Catheter induced distal intra-Hisian right bundle branch block in a patient with persistent proximal intra-Hisian complete A-V block.

We describe a patient with complete intra-Hisian A-V block and narrow QRS who developed catheter-induced right bundle branch block during an electrophysiological study. Selective His bundle pacing was performed during complete intra-Hisian A-V block and right bundle branch block pattern; the distal His bundle pacing normalized the QRS complex, while a more proximal His bundle stimulation showed a right bundle branch block configuration with the persistence of the proximal intra-Hisian lesion responsible for the complete A-V block. These electrophysiological findings suggest that the catheter-induced right bundle branch block in our patient was due to a focal lesion in the distal part of the main His bundle. To our knowledge, this is the first report of documented multilevel lesions within the His bundle in man.

[The stress systolic time intervals in the diagnosis of ischaemic heart disease. Study of the recovery in patients with persistent ischemia (author's transl)].

The changes in systolic time intervals (STI) LVET, PEP/LVET) between rest conditions, maximal exercise and after seven minutes after upright bicycle stress test were measured in 31 consecutive subjects: 14 "control" normal subjects, and 17 subjects with ischemic heart disease. The purpose of this investigation was to evaluate the specificity and sensitivity of the stress - STI in the identification of patients with positive exercise ECG and of patients with markedly positive exercise ECG that shown a late recovery persistent ischemia. The changes of STI are different in the control group and in ischemic heart disease group pts with negative exercise stress test and pts with positive or markedly positive exercise ecg. In the literature the usefulness of stress STI in the diagnosis of coronary disease is not univocally accepted. Our results indicate that STI are not a parameters that have an available correlation ith the stress myocardial ischemia even at distance of haemodynamic variations of maximal exercise and recovery and in presence of late recovery persistent ischemia.

[The hemodynamic effects of nifedipine in congestive cardiomyopathy (author's transl)].

The Authors present the haemodynamic effects of nifedipina (Adalat-Bayer) in nine patients with congestive cardiomyopathy. Heart rate (HR), systemic vascular resistances (SVR) and mean aortic blood pressure (MABP) were significantly modified in all nine patients following the administration of the drug. Six of them showed an increase of cardiac index (CI) and a decrease of myocardial oxygen consumption (MVO2); three of them presented a decrease of CI and an increase of MVO2. This discrepancy can be due to the different percentage of increase and to the maximal of valves of HR in the two groups. Nifedipina can be useful in the treatment of congestive cardiomyopathy with elevated STR (afterload) and peripheral hypoperfusion (more than pulmonary congestion).

[Chronobiological pattern size of cardiac rhythm (author's transl)].

The basic knowledge of statistical methods in chronobiology are exposed, for the analysis of chronobiological features of cardiac rhythm. The cosinor's method, with definition of chronobiological parameters (acrophase, mesor and amplitude) allows a microscopic approach to study of biological rhythms. The AA. relate some chronobiological studies: a) on heart rate in normal subjects and in patients with myocardial infarction (MI); b) on premature ventricular beats in MI; c) on atrial and ventricular rates in complete AV block. These results demonstrate that there is an evident circadian rhythmicity for all analyzed parameters. The AA. analyse the physiological and clinical meaning of the chronobiological approach to cardiac rhythm for the chronopharmacological and pathophysiological studies and suggests the implications in pathology of adaptation to shift work.

[The scimitar syndrome. A clinical contribution and review of the literature (author's transl)].

The scimitar syndrome groups a combination of several congenital anomalies in the right lung with an abnormal right pulmonary venous drainage in the inferior vena cava or the right atrium. After a survey of 130 cases in the literature, clinical, angiografic and hemodynimac features of the syndrome are defined. The Authors report two new cases diagnosed in adults, characterised by a clean discordance between the clinical features and the angiographic ones. The former case is completely asymptomatic and the radiographic feature is typical, while the latter shows a characteristic symptomatology but there isn't a clean "scimitar image". But notwithstanding the absence of this "scimitar image", the Authors think that the anomalous pulmonary drainage in the right atrium and the several anomalies in the right lung can be attributed to the scimitar syndrome, physiopatologically.
