The effects of urea treatment in combination with curettage in extensive lip cancers.

Thirty cases of lip cancer in 28 patients, 23 of which were extensive or very extensive, were treated with local urea injections in combination with curettage. Out of the 30 cases there were 26 squamous cell carcinomas, three basal cell carcinomas extending to the mucosa from the skin of the lips, and one adenocarcinoma of the mucosa of the interior surface of the upper lip. This treatment was effective in all our patients. Recurrences appeared in four patients. These were easily treated in three out of the four patients who returned for treatment, by the same method. In three cases metastases appeared in the lymphnodes of the neck, which were surgically treated.

Effects of urea treatment in malignancies of the conjunctiva and cornea.

A series of 9 patients with extensive squamous cell carcinomas of the conjunctiva, affecting also the cornea in 5 of them, were treated with local applications of urea. 8 out of these patients were cured. In 1 patient with a concomitant extensive conjunctivitis, the treatment was ineffective. Urea treatment is very simple and without any complications, apart from a transient opacity of the cornea. Recurrences are easily and effectively retreated as in 1 of 2 of our cases. But if the eyeball has been destroyed, as in the second case, retreatment is impossible. At least 5 out of the treated patients would have had an enucleation of the globe or an exenteration of the orbit without the urea treatment.

Effects of urea treatment in combination with curettage in extensive periophthalmic malignancies.

46 patients with 47 cancers of the eyelids and the canthi, large or very large in their great majority, were treated with local urea injections in combination with thorough curettage. In 7 of the above patients, the caruncle and the conjunctiva were also affected. This treatment was effective in 100% of our cases. The full recovery of the skin which had been destroyed by the cancer, without any remnants of scar or other disfigurement, is a very remarkable phenomenon; the functional condition of the lids remained quite normal as well. So we can affirm that in extensive periophthalmic carcinomas, in which cure with conventional methods is very difficult or almost impossible, as in some reported cases, the afore-mentioned treatment gives the most beneficial results, without any of the disadvantages of these methods.

Urea in the treatment of epibulbar malignancies.

Eight patients with epibulbar malignancies of the eyes were sucessfully treated with urea. One of them presented a malignant melanoma, one a tumour of Kaposi's disease, and six had squamous cell carcinomas. At least four out of these eight patients would have required enucleation of the globe or exenteration of the orbit as an alternative to urea therapy.
