Burkitt's lymphoma in Ethiopian children.

Burkitt's lymphoma is the commonest malignancy seen in childhood in Ethiopia. Thirty-nine children admitted to the Ethio-Swedish Children's Hospital from January 1971 to December 1988 were studied. The male to female ratio was 1.3:1, 24(63%) were from non-malarious areas, 29(76%) came from areas with altitude above 1500 meters above sea level, 36(95%) came from areas with mean annual temperature above 16 degrees C and 35(61%) patients came from areas with mean annual rainfall above 1000 mm. The commonest presentation was abdominal mass followed by jaw swelling. Eight (20%) patients died in hospital. The geographic distribution of the tumour seems to be different from that seen in some parts of Africa. Studies with regard to Epstein-Barr virus linkage in Ethiopia are warranted.

Analysis of surgical admissions to the Ethio-Swedish Children's Hospital (1984-1988) in Addis Ababa.

Surgical problems in children result in significant morbidity and mortality. A retrospective analysis of all surgical patients admitted to the Ethio-Swedish Children's Hospital (ESCH) over a five year period from 1984 to 1988 was made. There were a total of 2,281 surgical patients admitted, accounting for 22% of all hospital admissions (total = 10,364). The gastrointestinal and musculoskeletal systems were the most common systems involved. Acute appendicitis accounted for 13.9% (N = 318), cleft-lip and palate 8% (N = 183), and burns 6.9% (N = 157) of all surgical admissions. Accidents and trauma accounted for 25% of the surgical admissions (N = 564). Of these, the most common conditions were burns, car accidents, accidental falls, and foreign body aspirations. The over all mortality rate was 4% (N = 98). Acute appendicitis, intussusception, acute laryngotracheobronchitis (ALTB), and burns were associated with a high mortality. Of the neonatal admissions, one third died shortly after surgery, probably due to anaesthetic, fluid and electrolyte imbalance. Examination of the general pattern of surgical admissions revealed that many of the conditions were preventable, or amenable to medical therapy if detected early. Health education of the public is therefore necessary in order to reduce the morbidity and mortality of these conditions.

[Diagnostic assessment and documentation of psychosomatic diseases].

Besides the notion of psychosomatic medicine as a way of viewing, there is need of a definition of so-called psychosomatic diseases from the aspect of demarcation against general bio-psycho-social interactions. To this end, evidence of a framework of conditions has to be provided which integrates organopathological, experience-related psychological and social parameters. The traditional medical history must be broadened to cover these connections and for this purpose an orientation system is presented.

An unusual presentation of hepatoblastoma.

A biopsy-proven case of epithelial-type hepatoblastoma in a 12 year old Ethiopian boy who presented with an epigastric mass, jaundice, and a cutaneous biliary fistula is described. The literature on hepatoblastoma is reviewed, and therapy discussed.

Subcutaneous nodules in rheumatic fever.

Acute rheumatic fever is described in a six year old Ethiopian male child who first presented with subcutaneous nodules and later developed carditis.

Quaternary structure of erythrocruorin from the nematode Ascaris suum. Evidence for unsaturated haem-binding sites.

The quaternary structure of erythrocruorin from the nematode Ascaris suum was studied. The native protein had a sedimentation coefficient, at a protein concentration of 1 mg/ml, of 11.6 +/- 0.3 S and an Mr, as determined by sedimentation equilibrium, of 332,000 +/- 17,000. SDS/polyacrylamide-gel electrophoresis gave one band with a mobility corresponding to an Mr of 43,000 +/- 2000. The Mr of the polypeptide chain was determined to be 41,600 +/- 1,500 by sedimentation equilibrium in 6 M-guanidinium chloride and 0.1 M-2-mercaptoethanol. Cross-linking with glutaraldehyde followed by SDS/polyacrylamide-gel electrophoresis yielded a maximal number of eight bands. The haem content of Ascaris erythrocruorin was observed to vary from one preparation to another. This finding was shown to be due to non-realization of the full binding capacity for haem. By titration with haemin, the haem content was found to attain a maximal value of 2.86 +/- 0.14%, corresponding to a minimal Mr per haem group of 21,000 +/- 1,000. Our findings indicate that Ascaris suum erythrocruorin is composed of eight identical polypeptide chains, carrying two haem sites each.

Haemocyanin mRNA from arthropod and mollusc origin. Evidence for a multi-unit structure in mollusc haemocyanin mRNA.

mRNA from two molluscs, the snail Levantina hierosolima and the cuttlefish Sepia officinalis, and one arthropod, the scorpion Leiurus quinquestriatus, were found to contain species that hybridize with an oligodeoxynucleotide sequence corresponding to the six-amino-acid sequence His-His-Trp-His-Trp-His postulated to carry binuclear copper in arthropod haemocyanins. The duplexes formed between the oligodeoxynucleotide and mRNA from Leiurus and Levantina had similar 'melting' temperatures, namely 46.5 and 44.5 degrees C respectively. The hybridizable mRNA species were identified with mRNA that codes for haemocyanin, Hc mRNA. Electrophoresis under denaturing conditions of Leiurus Hc mRNA gave a single band corresponding to 2.3 kb (kilobases), consistent with the value of 75 000 reported for the Mr of the polypeptide chain of arthropod haemocyanin. Electrophoresis of mollusc Hc mRNA yielded RNA bands corresponding to 9.4, 6.7, 4.0 and 1.7 kb for Levantina and 9.5, 2.8 and 1.7 kb for Sepia. The 9.4 and 9.5 kb species represent authentic Hc mRNA and are consistent with an Mr of 350 000 for molluscan haemocyanin polypeptide chain. The faster-moving RNA bands are attributed to Hc mRNA cleavage by nucleases during isolation of mRNA. Analysis of the electrophoretic band pattern indicates a multi-unit structure for mollusc Hc mRNA.

[Extraction of the electrodes by continuous traction--an effective procedure in treating pacemaker wearers with secondary inflammatory complications].

Using a continuous traction method we succeeded in extracting 24 bacterially contaminated and fixed in the heart electrodensounds around 4 years after pacemaker implantation without complications worth mentioning in 7.5 days (1-27). The tissue attached to 20 sound points contained 9 times portions of the myocardium, 9 times remainders of the endocardium, 12 times particles of the connective tissue and 17 times suppurative inflammatory tissue. After extraction of the sound and aimed chemotherapy in all 24 patients the inflammatory pathological conditions rapidly and free from recidivations receded.

[Clinical and morphologic aspects of balloon catheter nephrostomy].

During 23 years in the County Hospitel St. Georg Leipzig 500 nephrostomies were performed. A definitive balloon catheter nephrostomy was performed 58 times. Exclusively the open transparenchymatous establishment was carried out. If no malignant basic disease shortens the survival time, the renal fistulae are tolerated for many years without greater restriction of the renal function. The longest time of observation of a fistulated solitary kidney is about 17 years. The complication rate is small and the psychic stress of the patient as well as the quality of life obtained are tolerable. A preoperative intensive talk with the patient with adequate explanation and alternative demonstration is necessary in every case. An epithelial lining of the channel of the fistula does not take place even after years. The squamous cell epithelium of the external skin and the urothelium of the renal calyces grow only a few centimeters into the fistula. This is lined by inflammatory granulation tissue.

[Necessity and difficulties of involvement of family members in the psychotherapy of psychosomatic patients].

Based on the particular problems which appear in psychosomatic patients the aim of the psychotherapeutic treatment of these patients is described. By inpatient group psychotherapy, a personality development is started that subsequently has to be continued in the real life situation of the patients. It is demonstrated which difficulties are brought about by the continuation of this development during subsequent outpatient treatment and how these difficulties can be overcome more easily by involving members of the family. The physical recovery aimed at may thus be achieved by a better compensation of defects in the primary psychic development of the psychosomatic patients.

Erythrocruorin from the water-flea Daphnia magna. Quaternary structure and arrangement of subunits.

The subunit structure of erythrocruorin from the cladoceran Daphnia magna was studied. The native protein was found to have a sedimentation coefficient (S2(20), w) of 17.9 +/- 0.2 S and a molecular weight, as determined by sedimentation equilibrium, of 494 000 +/- 33 000. Iron and haem determinations gave 0.312 +/- 0.011% and 3.84 +/- 0.04%, corresponding to minimal molecular weights of 17900 +/- 600 and 16 100 +/- 200 respectively. Sodium dodecyl sulphate/polyacrylamide-gel electrophoresis gave one band with mobility corresponding to a molecular weight of 31 000 +/- 1 500. The molecular weight of the polypeptide chain determined by sedimentation equilibrium in 6 M-guanidinium chloride and 0.1 M-2-mercaptoethanol is 31 100 +/- 1300. On a molecular-weight basis, Daphnia erythrocruorin is composed of 16 identical polypeptide chains carrying two haem groups each. The native structure is stable between pH5 and 8.5. At alkaline and acidic pH, a gradual decrease in the sedimentation coefficient down to 9.8S occurs. Above pH 10 and below pH4, a slow component with S20, w between 2.7S and 4.0S is observed. The 2.7S, 4.0S and 9.8S species are identified as single-chain subunits, subunit dimers and half-molecules respectively. We propose a model for the molecule composed of 16 2.7S subunits grouped in two layers stacked in an eclipsed orientation, the eight subunits of each layer occupying the vertices of a regular eight-sided polygon. Support for this arrangement is provided from electron microscopy and from analysis of the pH-dissociation pattern.

Erythrocruorin from the crustacean Caenestheria inopinata. Quaternary structure and arrangement of subunits.

The subunit structure of erythrocruorin from the crustacean Caenestheria inopinata was studied. The native protein was found to have a sedimentation coefficient of 12.0 S and a molecular weight, as determined by sedimentation equilibrium, of 302,000. Iron and heme determinations gave 0.346 and 3.98% corresponding to minimal molecular weights of 16,100 and 15,500, respectively. Sodium dodecyl sulfate-polyacrylamide gel electrophoresis gave one band with mobility corresponding to a molecular weight of 30,000. The molecular weight of the polypeptide chain was determined to be 30,500 by sedimentation equilibrium in 6 M guanidine hydrochloride and 0.1 M 2-mercaptoethanol. Dissociation of the 12S molecule was observed at acidic and alkaline pH. A dissociation species of 2.7 S was isolated and its molecular weight determined to be 28,000 by sedimentation equilibrium. On a molecular weight basis, the native molecule is composed of ten 2.7S subunits, each of which consists of a single polypeptide chain carrying two hemes. We propose a model for the molecule composed of ten spheres, each representing a 2.7S subunit, arranged in two layers stacked in an eclipsed orientation, in five spheres of each layer occupying the vertices of a regular pentagon. Support for this arrangement is provided by a comparison of projections of the model with molecular profiles seen in the electron microscope.

[Bladder paraganglioma].

It is reported on a 76-year-old male with an extraordinarily large paraganglioma of the urinary bladder. As lethal complication a urine diapedesis in the necrotic areas of the tumour with subsequent urine phlegmona was found. Up to now such a complication had not been described.

Oxygen binding properties of stripped (calcium ion and magnesium ion free) hemocyanin from the scorpion Leirus quinquestriatus.

The binding of oxygen to stripped Ca2+,-Mg2+-free) hemocyanin from the scorpion Leirus quinquestriatus was studied over a wide range of pH and temperature. Oxygen binding was cooperative in the entire range of pH where the native hemocyanin structure is preserved. Probably 12, or even the totality of the 24 oxygen binding sites of the molecule, act as a cooperative unit. The oxygen binding affinity and the degree of cooperativity were both affected by pH. The dependence on pH of the half-saturation pressure-Bohr effect-was interpreted in terms of two oxygen-linked protons, one opposing and the other promoting oxygen binding. The effect of pH on the maximal slope of the Hill plot indicates that proton dissociation and cooperativity are linked phenomena. Analysis of the binding data using linkage theory shows that an efficient coupling of the homotropic (oxygen-oxygen) and the heterotropic (proton-oxygen) free energies of interaction can provide a satisfactory interpretation of the cooperative behavior. Our findings suggest that in arthropod hemocyanin, as in molluskan hemocyanin, cooperativity of oxygen binding can be attributed to ligand-ligand linkage. In molluskan hemocyanin, however, cooperativity is generated solely by alkaline earth ion-oxygen linkage. In arthropod hemocyanin, both alkaline earth ion- and proton-oxygen linkage can generate cooperative behavior.

[Angiofollicular lymph-node hyperplasia (author's transl)].

Four cases of angiofollicular lymph-node hyperplasia (Castleman-Lymphoma) are presented, accidentally discovered. They prefer localisation in mediastinum. Histological characteristics are lymph follicular hyperplasia, vascular-proliferation with endothelium hyperplasia and sometimes hyalinosis of follicular centres. The tumor is of benign significance. Its etiologic remains unknown. For clinic relations it is very important to coordinate this tumor with the differential diagnosis of mediastinal diseases.
