[Fibrinolytic treatment. First results during the early postoperative period].

Fibrinolytic treatment during the early post-operative period is associated with a real danger of hemorrhage. On the other hand, in palliative shunt procedures in Fallot's disease, especially in small children, the anastomoses may thrombose. In such cases the therapeutic lysis of the thrombus improves the child's condition and may defer further surgery. The authors describe five cases of children with Fallot's disease and thrombosis of Blalock-Taussig anastomoses who were successfully treated by streptokinase. They conclude that fibrinolytic treatment should be considered in early thrombosis following anastomosing operations.

[Model of formation of chronic volume loading in the right heart].

The authors describe an experimental model for examining the changes occuring in the haemodynamics of the small circuit as a result of created artificial defect of the interatrial septum. An original operative technique is used, by which the complete clamping of the right lung artery and vera is avoided and lung complications are prevented. The performed control haemodynamic studies up to one vear after the operation support the efficiency of the proposed model.

[Systolic and diastolic load of right heart and right bundle block].

Analysing the Ecg and VCG of 818 subjects, healthy or with different congenital and acquired cardiopaties, the frequency of right bundle branch block (RBBB) is established to decrease with the increase in age of children, and is observed to be 8.9% in healthy adults. The right bundle block is observed in 82.5% of the patients with atrial septal defect and quite often in patients with right ventricle tension loading, gradually decreasing with the level of the left ventricle loading, as well as with the development of extreme right ventricle hypertrophy. Out of 50 experiments with dogs with acute and chronic right heart tension loading it is evident that the number of certain and marginal (uncertain) cases with RBBB steadily increases. At the same time this increase cannot be connected with the type of loading, in dogs in which dilatation of the right ventricle is presented as well as in others with light or marked hypertrophy. Besides, under the same conditions and durations of loading some hearts exhibit RBBB, while others do not. What has been stated before gives enough grounds to accept that RBBB in healthy subjects, as well as in patients with congenital and acquired cardiopaties manifests the electromotive tension (EMT) of persistent from foetal and children-age more pronounced physiological hypertrophy (as normal variant) of the basal part of the right ventricle. Whether the picture of RBBB is retained, pronounced or disappears is determined by the correlation between the quantity and the time-manifestation of EMT of the left ventricle and of the corps part of the right ventricle.

[Terminal bundle-branch block (TBB) in acute and chronic overloading of the right heart].

The authors performed a model loading of the right heart ventricle in 40 dogs in 17 dogs--acute tension loading by creating a partial ligature (banding) of the pulmonary artery; the dogs were examined in respect to the changes hemodynamics and ECG for a period of three hours. In the second group of 10 dogs a chronic tension loading of the right heart was created by a partial ligation of the pulmonary artery (banding); the dogs were followed--up in respect to hemodynamic and vectocardiographic changes for a period of one year. In the other group of 10 dogs a chronic loading of the right heart: was formed by operative route and interatrial defect was obtained; the dogs were followed-up in respect to hemodynamics and vectrocardiographic changes for a period of one year. The authors described the changes in the intraventricular conduction of vectro cardiograms, made by the three plane system of Akulincev, and these vectrocardiograms were compared with the type and degree of loading as well as with the morphologic changes occurred in the heart. The authors indicated the nonspecificity of TBB as a sign of volume loading of the right heart ventricle.
