A rare intrahepatic portacaval tubular shunt in a patient with spastic paraparesis.

Although there are numerous communications between the portal and systemic venous systems, intrahepatic portosystemic venous shunts are not frequently encountered in clinical practice. Here we report a patient who presented with spastic paraparesis, who was found to have chronic liver disease with tubular intrahepatic portacaval shunting.

A new aneurysm wrapping material: polyglactin 910 + fibrin sealant.

Aneurysms experimentally induced by using the silver nitrate coagulation method in 10 Wistar Albino rats are wrapped with Polyglactin 910 and Fibrin Sealant. 6 weeks later the rats are sacrificed and compared with the control group. In the group in which Polyglactin 910 and Fibrin Sealant were used as the wrapping material, non-specific inflammatory granulation tissue development around the aneurysms is observed. We suggest that a Polyglactin 910 and Fibrin Sealant combination can be used as a wrapping material in the treatment of aneurysms where clipping is not possible.

The role of pentoxifylline in the treatment of erectile dysfunction due to borderline arterial insufficiency.

OBJECTIVE: To investigate the effects of pentoxifylline on penile blood flow and potency in patients with erectile dysfunction due to borderline arterial insufficiency. PATIENTS AND METHODS: Following a routine diagnostic evaluation, 36 patients with borderline arterial disease diagnosed using penile duplex ultrasonography were included in the study. Of these patients, 20 (mean age 54 years, range 40-66) received 1.2 g of pentoxifylline daily, divided into three doses orally, for 2 months and 16 (mean age 54 years, range 34-65) received a placebo. Peak systolic velocities (PSVs) before and after the treatment were compared with those in patients receiving a placebo. The clinical improvement in erectile function was also evaluated. RESULTS: Twelve of 20 patients receiving pentoxifylline had an increase in PSV at the end of the treatment period. The mean change in PSV achieved by pentoxifylline treatment (6.25 cm/s) was significantly higher than that achieved by the placebo (0.38 cm/s). Seven patients had a positive response (successful coitus achieved after treatment with pentoxifylline). No serious side-effects occurred. CONCLUSION: Pentoxifylline was well tolerated and could increase penile arterial inflow, shown objectively as an increase in PSV on penile duplex ultrasonography. These findings should encourage further studies of the efficacy of pentoxifylline in the treatment of impotence.

Sonourethrography in anterior urethral stricture: comparison to radiographic urethrography.

Twenty-three male patients with known or suspected urethral stricture disease were evaluated using sonourethrography and standard retrograde x-ray urethrography for comparative analysis of two techniques. Results were evaluated statistically. These two methods can substitute each other in determining the stricture area length of anterior urethra. Due to advantages, if both of these methods are used combining each other, we believe that they will be much more sensitive.

[Tracing and measuring errors in the roentgenographic cephalometry].

In the present study, thirty cephalometric films which had been used in three different investigations using almost the same parameters were selected and divided into two groups. Twenty-three parameters in the first group and nine parameters in the second, which were used in two and three investigations respectively, were statistically compared. Paired t test was used in the first group and variance analysis in the second group. In conclusion, a lot of parameters exhibited statistically significant variability, while some of them were not.

A-waves and electrophysiologically established diagnoses.

The aim of this study was to investigate the presence of A-waves in cases referred to our electromyography laboratory with various diagnoses and documentation of electrophysiologic diagnoses with A-waves. In cases where at least 3 motor nerves were analysed, during F response studies, A-waves having stabile latencies and amplitudes from minimum one third of submaximal and supramaximal stimuli were evaluated. Electrophysiologic diagnoses and the related nerves of cases with A-waves were recorded. A-waves were obtained from 38 out of 1604 cases (2.36%). Most of the cases (57.8%) with established A-waves consisted of patients with radiculopathies. A-waves were detected mostly in tibial nerve. Electrophysiologic analysis of 5 cases with A-waves has given normal results. Although mechanism of A-waves is not known completely, we suggest when obtained it must be remarked during routine electrophysiologic examination.
