Effective audit: reporting to the National Confidential Enquiry into Perioperative Deaths.

OBJECTIVE: To investigate the effectiveness of computer based and manual district and unit information systems for identifying hospital deaths eligible for reporting to the National Confidential Enquiry into Perioperative Deaths (NCEPOD). DESIGN: Prospective six to 10 week study of hospital (death register, immediate coding of medical records) and district information systems followed by six month assessment after modification to entry of data. SETTING: Acute units within Lewisham and North Southwark Health District. PATIENTS: All 290 patients dying in hospital during the six weeks, for whom the medical records were obtainable in 231. MAIN OUTCOME MEASURES: Sensitivity and specificity of the information systems in ascertaining eligible surgical deaths (patients dying in hospital who had during 30 days previously had a surgical procedure while under the care of a consultant in a surgical specialty) tested against validated list of screened medical records. RESULTS: Of 231 medical records, 30 (12 from Lewisham, 18 from North Southwark) met the national inquiry's criteria. The computer based systems of both units detected less than 60% of eligible deaths (sensitivity 53%, specificity 83%); the death register detected about 60% (sensitivity 61%, specificity 89%); manual systems detected all eligible deaths. Subsequent modification to ensure immediate coding of records into the computerised systems during follow up failed to show any improvement. IMPLICATIONS: Routine hospital information systems may miss up to half the deaths eligible for NCEPOD.

The pyruvate dehydrogenase complex from the parasitic nematode Ascaris suum: novel subunit composition and domain structure of the dihydrolipoyl transacetylase component.

The pyruvate dehydrogenase complex (PDC) from muscle of the adult parasitic nematode Ascaris suum plays a unique role in its anaerobic mitochondrial metabolism. Resolution of the intact complex in high salt dissociates the pyruvate dehydrogenase subunit but leaves the dihydrolipoyl dehydrogenase subunit (E3) and two other proteins with apparent M(r)s of 45 and 43 kDa bound to the dihydrolipoyl transacetylase (E2) core. These proteins are not observable on Coomassie brilliant blue-stained gels of other eukaryotic PDCs, but the 45-kDa protein is similar in apparent M(r), pI, and sensitivity to trypsin to the Kb subunit of the bovine kidney PDH alpha kinase. Acetylation of the ascarid PDC with [2-14C]pyruvate under conditions designed to maximize the incorporation of label into protein yielded only a single radiolabeled subunit, E2. These results confirm earlier reports that the ascarid PDC lacks protein X, an integral component recently identified in other eukaryotic PDCs. About 1.6 to 1.8 mol of 14C was incorporated/mole of E2, suggesting that the ascarid E2 contained two lipoly-bearing domains. Domain mapping of the 14C-acetylated ascarid E2 by limited tryptic digestion identified two lipoyl-bearing fragments with apparent M(r)s of 50 and 34 kDa and two core fragments with apparent M(r)s of 46 and 30 kDa. The ascarid E2 domain structure appears to be similar to that of other E2s. However, it appears that the subunit-binding domain (E2B) of the ascarid E2 may be significantly larger or be flanked by larger than normal interdomain regions. An enlarged E2B domain may be necessary to accommodate the additional binding of E3 to the E2 subunit in the ascarid complex, in the absence of protein X.

Adenocarcinoma of the ethmoid olfactory mucosa: a histopathological and ultrastructural study with evidence of virus-like particles.

A histopathological, morphometric and ultrastructural study was made of enzootic nasal neoplasia in a group of 25 Verata goats. A nasal adenocarcinoma was diagnosed. The neoplasm contained two clearly defined zones, one cystic and the other compact. The tumour stroma was composed of abundant loose connective tissue, in which mononuclear infiltrate was clearly identifiable. The observed viral particles were morphologically similar to Visna-Maedi. These particles had an eccentrically located electrodense core. The diameter of the virus was about 90 nm and it showed envelope numerous spikes on the surface. The virus is assumed to be the causative primary agent of the tumour.

Characterization of cDNA clones for the beta subunit of pyruvate dehydrogenase from Ascaris suum.

The pyruvate dehydrogenase complex occupies a unique position in the anaerobic mitochondrial metabolism of the parasitic nematode Ascaris suum. This paper describes cDNA clones for the beta subunit of the pyruvate dehydrogenase component of this complex. A cDNA library has been constructed in lambda gt11 from poly(A)+ RNA isolated from adult ascarid body wall muscle. The library was screened with antiserum prepared against the beta subunit of pyruvate dehydrogenase. Full-length clones of 1.2 kb have been characterized. The first 15 amino acids determined from the purified protein match exactly those predicted from the cDNA sequence. The deduced protein sequence contains a 26-amino-acid presequence that has characteristics of mitochondrial targeting sequences. The mature protein is predicted to contain 334 amino acids and is 62% identical to the predicted sequence of the corresponding human subunit. Full-length in vitro transcripts have been translated in vitro to yield a 39-kDa polypeptide consistent with the open reading frame present in the cDNA sequence. The 90 nucleotides at the 3' end of the cDNA sequence have the potential to form a cruciform structure that may play a role in the synthesis of the enzyme.

Structural and ultrastructural differentiation of cell types in the gills of the tench (Tinca tinca, L.).

This study used 22 tench of 25-30 cm in length and weighing roughly 200 g. After sacrifice, the gill arch was extracted and fixed in 5% glutaraldehyde for structural, ultrastructural and morphometrical study. Tench gills consist of primary lamellae which are perpendicular to numerous secondary lamellae in which the respiratory barrier is located. A study was made of the structures which make up that barrier and of the multi-layered epithelium situated in the interlamellar space, which is closely linked to maintaining of internal balance in the fish.

Experimental lead nitrate poisoning: microscopic and ultrastructural study of the gills of tench (Tinca tinca, L.).

A microscopic, ultrastructural, and morphometric study was made of the gills of tench (Tinca tinca, L.) subjected to acute experimental lead nitrate poisoning. Twenty-one adult tench were subjected to poisoning and a further 22 were used as controls. Lesions were characterized by the appearance of edema and epithelial hyperplasia and necrosis, both in cells forming part of the filtration barrier and in those in the interlamellar space. These processes developed in the course of the experiment, leading to the death of tench after 12 to 15 days of exposure to 75 ppm lead nitrate, at which point the concentrations of lead in the gills had reached their maximum.

Expanded use of the right and left internal mammary arteries for myocardial revascularization.

While the use of the left internal mammary artery (IMA) has become routine in many cardiac surgery units there is some reluctance to utilize the right IMA. The reasons for this are the higher morbidity and mortality observed in the initial experience with bilateral IMA grafting, the limited length of the right IMA, and its potential injury in reoperations when used to revascularize the left anterior descending artery. In this series of 50 patients the right IMA was used as a conduit for myocardial revascularization. One hundred and fifty-nine coronary vessels were grafted (3.2 per patient) 67.3% with IMA and only 32.7% with vein grafts. The right IMA was used for 51 anastomoses and the left for 56, either as simple, sequential or Y type grafts. The refinement of techniques for constructing the IMA grafts used in this series show that the right IMA can reach in most of the cases the anastomotic sites of the right coronary artery beyond the acute margin of the heart, and it is hoped that the lateral positioning of the IMA pedicles will lessen the chance of graft injury at reoperation. On the basis of early clinical results (2% mortality and 4% sternal complications), the application of the right IMA seems justified electively for the revascularization of both left and right coronary artery systems or as a free graft in conjunction with the left IMA.

Carpentier's flexible ring versus De Vega's annuloplasty. A prospective randomized study.

To compare the efficacy of Carpentier's tricuspid annuloplasty with De Vega's, we prospectively randomized 159 patients, operated upon between January, 1977, and January, 1980, to one of the two techniques: 76 patients were assigned to the Carpentier group and 83 to the De Vega group. The criterion for inclusion in the study was the presence of moderate to severe tricuspid regurgitation. There were no significant differences in mean age, male proportion, type of mitral lesion, incidence of aortic valvulopathy, and other preoperative and perioperative characteristics between the two groups. However, organic tricuspid damage on macroscopic intraoperative examination was more common in the Carpentier group. At the end of follow-up (average 64 months) in patients with satisfactory left heart hemodynamics, there was a significant difference in the incidence of moderate or severe tricuspid insufficiency between the two groups (De Vega, 14/41; Carpentier, 4/40; p less than 0.01). In 76 patients (40 with Carpentier's annuloplasty and 36 with De Vega's technique), contrast right ventriculography was performed postoperatively and the degree of tricuspid regurgitation assessed semiquantitatively. In both groups, control of tricuspid regurgitation was poor in patients with either high total pulmonary resistances or organic tricuspid damage. If patients with these characteristics are excluded, then significant tricuspid regurgitation was encountered in only one patient in the Carpentier group but in nine of 19 in the De Vega group (p less than 0.01). In conclusion, in the treatment of tricuspid regurgitation, better results are obtained with Carpentier's than with De Vega's annuloplasty, especially if there is no organic tricuspid damage and pulmonary resistances decrease postoperatively.

[Course of psychopathologic and extrapyramidal motor symptoms during long-term treatment of schizophrenic patients with psycholeptic drugs (author's transl)].

65 chronic-schizophrenic outpatients were treated with fluphenazinedecanoate for 24 months. Hallucinations and delusions remitted between week 24 and 36, formal disturbances of thinking occurred in 50% of the patients up to the 24th week, mood disorders and disorders and schizophrenic changes in personality could only gradually be controlled. In the first 3 months of treatment the incidence and intensity of rigor grew up to the critical point after 24 weeks of treatment; by the 60th week of treatment it had completely vanished. Akathisia reached its peak after the first and the 36th week and two different populations could be distinguished. 8% of the patients showed dyskinetic reactions up to the 12th week; 20% of the patients showed hyper- and dyskinesia after 72 weeks of treatment. 30 to 40% of the patients required anticholinergics between the 12th and 48th weeks; after the 84th week this medication was no longer necessary.

[Mortality in accidents in Spain: contrast of various statistical series].

The data on deaths due to accidents in Spain during the period 1975-79 is analyzed and compared according to three routine sources of information: death certificates, codified, tabulated and presented by the National Statistics Institute (INE): traffic accident reports from the Directorate General of Traffic (DGT): and work-related accidents according to reports drawn up by the National Institute of Job Health and Safety. The comparison reveals that the data from death certificates are not useful for studying work-related accidents in Spain. As for traffic accidents, the death statistics bulletins offer a more thorough coverage, especially for children and elderly persons; nevertheless, the detailed reporting of the circumstances surrounding the accidents and the earliness with which this data is made available make it advisable to use the data from the General Directorate of Traffic for monitoring the overall tendencies of traffic accidents and for evaluating the effects of the prevention programmes. However, the death statistics bulletins permit the formulation of more thorough estimates of the population base and diverse areas of the country with regard to deaths caused by accidents.
