[Study of ocular hemodynamics and general somatic status of patients with primary open-angle glaucoma].

Ocular hemodynamics and general somatic status were studied in 50 patients with primary open-angle glaucoma. Complex ophthalmological and therapeutic examinations were carried out, macro- and microcirculatory hemodynamics of the eye was examined, case histories and therapeutic status of patients were analyzed with due regard for previous local hypotensive and general vascular therapy. Revised recommendations for glaucoma patients aimed at correcting their life style include diet, regular vascular and metabolic therapy, normalization of orbital circulation in order to improve ocular hemodynamics and increase the tolerance of the optic nerve to intraocular pressure fluctuations.

[Characteristics of the clinical picture of combined injuries of the organ of vision and eye appendages under conditions of peace-time and war injuries].

A total of 100 patients with combined injuries of the organ of vision and eye appendages, inflicted in peacetime and under war conditions, were examined. The clinical picture of combined injuries in the wounds inflicted at the battlefield (gunshot and resultant from mine explosions) and of traffic, communal, criminal, and occupational injuries was studied using traditional ophthalmological and common clinical instrumental methods of examination. Common regularities of the traumatic process were revealed in combined injuries: mutual aggravation of injuries to the eye and its appendages, a considerable incidence of bilateral injuries, and simultaneous impacts of several damaging factors. The most unfavorable prognosis as regards visual functions in the group observed was for patients with war injuries, particularly those caused by mine explosions.

[Features of treating patients with combined damage to the orbit and eye in the late period in peacetime and war conditions].

Analysing the diagnosis and treatment of 100 patients with combined injuries of the organ of vision and orbital zone of the face provided clinical features of lesions in peace-time or battle accidents. Battle accidents were found to be prognostically worse for visual functions. The use of immunotherapy for inflammatory complications of a traumatic process is demonstrated to be advisable. A program of stepwise treatment of combined lesions of the eye and the orbit with the priority of preserving the function of the vision organ. At stage 1 it is necessary to take measures for restoration of visual functions to avoid their decline or loss. At stage 2 reconstructive interventions in the orbit and visceral cranium, directed to the provision of topographic and trophic conditions for further restoration of eye functions should be performed. One-stage surgical intervention may be used if there are no real danger of vision loss. The peculiarities of reconstructive treatment of multifractional fractures of the orbit in the battle accident and fragmental fractures of the orbit in peace-time accident are described. Patients with combined injuries of the vision organ and the orbit in specialized institutions should be cared.

[Carcass tarsomargoplasty in involutional (senile) entropion of the lower eyelid].

The authors analyze the literature and their own data on the structure of the lower lid in health and involution and present their version of the development of the lower lid entropion and a method for surgical correction of this abnormality, carcass tarsomargoplasty, developed by them. The gist of this surgical intervention is fortification of the lower eyelid carcass structure by implantation into the depth of the eyelid of dosed shaped plates, molding of the lower eyelid shape, and fixation of the pretarsal portion of the skin (in the lower segment of the eyelid to the tarsal plate). Eleven patients were operated on; functional and cosmetic results were good, as evidenced by a follow-up of 11 months.

[Methods of the study of basal lacrimal secretion].

The authors propose a nontraditional method for the study of basal lacrimal secretion, based on biometry of the height of the lacrimal flow meniscus during common biomicroscopy. The advantages of this method are shown: it is noninvasive, contact-free, available, and informative for assessing basal lacrimal secretion. The authors defined the normal value of lacrimal meniscus height in health and detected noticeable changes of basal secretion in secondary dry syndrome. Basal lacrimal secretion is suppressed in patients with pathologic changes of the anterior segment of the eye (relapsing erosions, pterygium, chronic conjunctivitis in contact correction, etc.). Prospects for practical use of the method are outlined.

[Reconstruction of the lower orbital wall in the remote posttraumatic period].

The authors share their experience gained in the treatment of 11 patients with different fractures of the lower orbital wall in remote periods after the injury (1 month to 4 years). A specific feature of late injury in all the patients we observed were disorders of binocular vision presenting as diplopia due to cicatricial changes in the orbit. X-Ray examinations showed deep fractures of the lower orbital wall involving the posterior half of the orbital space in all cases. An operation had to be performed in all the patients: inferior orbitotomy via a transcutaneous access combined with anterior highmorotomy and submucosal refracture of the upper wall of the maxillary sinus within the framework of improperly consolidated fracture along the borders of the orbital hernia. The choice of this operation was dictated by specific localization of the fractures (zones difficult to access), solidity of scar growth at the site of the fracture (because of long period elapsed after the injury), and by limited possibility of intervention via an isolated transorbital access. An accessory element: refracture of the lower deformed wall and its reposition in the proper state helped repair the defect in the wall with a homocartilage on the side of strips and the orbit. Binocular vision and mobility of the eyeball in the orbit were repaired in all the patients, the degree of posttraumatic enophtalmos was appreciably decreased.

[The possibilities for surgical correction in the combined treatment of patients with cicatricial deformities of the eyelids].

A total of 217 patients with posttraumatic cicatricial deformations of the eyelids were treated. The authors resort to a functional approach when assessing posttraumatic injuries of the eyelids and propose their own anatomic and functional classification of cicatricial deformations of the eyelids, based on the principle of anatomic and functional entity of the eye and its defense system. This classification specifies the basic functions of the eyelids as components of the defense mechanism of the eye, offers criteria for assessing the cicatrices proper, and presents the basic characteristics of combined cicatricial deformations of the eyelids and complications thereof. The authors used this classification to plan the treatment strategy, scope of surgical intervention, and order of stages, and to define the indications for the use of various methods of blepharoplasty in patients with cicatricial deformations of the eyelids.
