[Obliteration of the bulla of the guinea pig with a Kieler bone chip].

The obliteration of the sinus frontalis after removal of the chronically inflamed and irreversibly transformed mucous is a common surgical treatment. There is no definite answer to the question of the most suitable material for obliteration. The suitability of the Kieler Knochenspan was, therefore, tested in animal experiments. After trepanation and removal of the mucous of the bulla of 30 guinea pigs, the cave was filled up with Kieler Knochenspan. The animals were killed 2 weeks, 1, 2, 3, and 4 months after implantation. The implanted nonvital spongy substance of the Kieler Knochenspan was first enclosed in granulation tissue. Then, depending on the duration of implantation, this granulation tissue turns into bony substance first peripherically and then centrally. Immunologically conditioned reactions were never observed. Thus, the transformation of the Kieler Knochenspan in the pneumatic system of the guinea pig is the same as in human tubular bones. So the conclusion is drawn that Kieler Knochenspan might be used in the obliteration of the human frontal sinus.

[Metastasizing basalioma of the temporal area or continuous extension? Surgical approach and results].

By means of histological sections the rare diagnosis of a basalioma of the temporal area with regional metastases in the parotid gland and in the neck could be verified. Subsequently the surgical treatment and the postoperative results are described. The operation consisted of an en bloc excision of the basalioma, exstirpation of the parotid gland and radical neck dissection. Since the tumor had also infiltrated the facial nerve, the nerve had to be sacrificed and reconstructed using the superficial peroneal nerve. In order to obtain immediately a satisfactory esthetic result in the face region, in addition to the facial nerve graft, the corner of the mouth and the eyelids were taken up using solvent dehydrated dura mater and parts of the temporal muscle and its fascia. A face-lifting supported this procedure. The primary closure of the temporal skin defect was successfully performed using a regional skin flap from the scalp.

[The sublingual and submandibular glands after preganglionic parasympathetic blockade. A histological, electron optical, autoradiographic and scintigraphic study on the guinea pig].

After preganglionic parasympathetic block three histological and histochemical phases could be determined in the only cholinergically innervated mucous cells of the sublingual gland. The first phase is characterized by overproduction and oversecretion. Possibly this results from acetylcholine overproduction, caused by the block, termed as "degeneration secretion" however of preganglionic origin. Hypersensitivity of the gland parenchyma appears to be present also, which with catecholamins as well as acetylcholine cause the ensuing "paralytic secretion". At the end of the first phase the desquamation of parts of cells, and rarely entire cells, in form of apocrine and holocrine secretion occurs and a uniform cell population is developing. When this cell population develops the second phase starts. The lack of stimulation because of acetylcholine exhaustion is discussed as the cause of the second phase. In the third phase a self regulatory involution commences. In the double innervated serous submandibular gland the effect of chorda tympani section was less severe.

[Vascular considerations and technique-planning for pectoralis major myocutaneous island flaps (author's transl)].

The relevant distribution of the blood vessels originating from the thoracoacromial artery and the technique used to raise the pectoralis major myocutaneous island flap are described. The nutrient vessel to this flap is the thoracoacromial artery and its pectoral branches. After giving off its axillary vessels this artery with its vein crosses the upper edge of the pectoralis minor muscle and passes between the pectoralis major and minor muscles. There its branches extend over an area bordered by the second and sixth rib and the mammary and parasternal lines. The skin underlying this area is supplied by branches of the internal mammary artery and on its deep surface by branches of the thoracoacromial artery. Preservation of the thoracoacromial vein as well as artery is also necessary in this creation of a viable pectoralis major myocutaneous island flap.

[Flap-reconstruction in mouth and oropharynx. A clinical comparison of methods (author's transl)].

The possible methods for plastic reconstruction after resection of malignant tumors in the mouth and oropharynx are reported. A total of 165 patients, operated upon between 1973 and 1980, has been reviewed. The tumor was located 26 times in the tongue, 4 times within the base of the tongue, 27 times in the floor of the mouth, 10 times in the floor of the mouth and in the tongue and 80 times in the tonsils. For reconstruction 8 times the deltopectoral flap, 51 times the forehead flap, 42 times the tongue flap, 7 times the myocutaneous sternocleido-mastoideus island flap were used. 8 times other methods were applied, like for instance skin grafts, and in 41 cases a primary closure of the defect was possible. Complete necrosis of the flap were rare, more frequent in contrast were partial dehiscences with or without a fistula. Thus in the forehead flap 5 times a necrosis occurred whereas in 15 cases a dehiscence was seen. Even better results were achieved for the deltopectoral- and tongue flap. In contrast, the skin island of the myocutaneous sternocleidomastoideus flaps all became necrotic, but only once a temporary fistula developed. Of the pectoralis myocutaneous island flaps the first two became necrotic, probably due to lack of surgical experience. Taking into account the surgical expenditure, the functional as well as cosmetical results, the methods may be scaled according to clinical value as follows: The pectoralis major myocutaneous island flap and the tongue flap equally range on the first place, followed by the myocutaneous sternocleidomastoideus island flap, and on the 3. and 4. place by the deltopectoral and forehead flap.

[Deviations of septum cartilage after unilateral separation of Mucoperichondrium in rabbits (author's transl)].

In 49 rabbits the mucoperichondrium of their nasal septa was strictly unilaterally separated according to the Cottle procedure. In addition, in some rabbits only the surface of the septal cartilage was incised and in others the cartilage was cut vertically or horizontally. Four months after the operation the following alterations of the septa were seen: The separation of the mucoperichondrium from the cartilage or superficial incision of the cartilage led to a deviation to the operated left side. If in addition parts of the cartilage septum were mobilized, thus swinging horizontally or vertically, a deviation to the other nonoperated side was seen. Similarly, if two parallel vertical stripes of cartilage were excised, the ends of the remaining cartilage plate bent and the cartilage plate itself deviated to the nonoperated side. The postoperative deviations are due to released unbalanced forces of the cartilage itself and to the disequilibrium of scar tension resulting from the unilateral separation of the mucoperichondrium. To avoid these postoperative deviations the bilateral tunneling of the mucoperichondrium is required.

[Development of the vascularisation in ossicula trans- and implants (author's transl)].

The revascularisation in 30 rabbit incudes after autogenic transplantation and allogenic implantation as well as in 5 mallei of the calf after xenogenic implantation was investigated. Prior to allogenic implantation the ossicles had been treated in five different ways: 5 ossicles had been cooled down to - 20 degrees C or to - 196 degrees C respectively, 5 others had been treated with cialit (1:5000) and 5 had been freeze dried with subsequent gamma-irradiation. In the last five collectives during the process of revascularisation the sprouting tissue of the host follows the bony canaliculi of Havers. It either forms normal capillaries or it leads to a widening of the bony canaliculi of Havers; in this case capillaries with larger diameter predominate. In the ossicles after xenogenic implantation, however, a still persistent antigenic reaction causes a hyalinisation of the capillary walls with considerable reduction of their inner diameter.

[Tympanoplasty with human dura mater preserved in cialit (author's transl)].

In 26 cases allogenetic human dura mater, conserved in CialitTM 1:5000, was used to close perforations of the eardrum instead of commonly used autogenetic fascia of the temporal muscle. In preceding animal studies, it had been found that "Cialit dura" acts as guideline and stimulates the development of new host connective tissue. In 25 human ear drums the perforations healed up without any problems. The healing process took times of up to 8 weeks. Only in one case a new perforation was found. The moderate flexibility of "Cialit dura" proved as a special advantage during the surgery procedure. Human dura is easily to acquire and its conservation is simple and cheep. Therefore it can serve as a grafting material for tympanoplasty available at any time and in any size. For very large transplants and a lack of postoperative treatment the prolonged healing period could be of some disadvantage. For normal sized transplants, however, there seems not to be a difference in healing time between the commonly used fascia and the "Cialit dura".

[Earstructures in one case of anencephalie (author's transl)].

According to the majority of cases under observation the following anomalies could be found: extended, separated or manifold developed crura of the stapes, reduction in the circular windings of the cochlea down to 1 1/2 with a normal development of Corti's organum, reduction of the nerve structures of the nervus cochlearis to a munimum and an open facial canal.

[A clinical study on allogenetic ossicle-implantation (author's transl)].

Here we report a first three years clinical experience with allogenetic ossicles in middle ear surgery. The several allogenetic implants in tympanoplasty type III are coordinated to symbolic abbreviations for documentation. The technique and biology of Cialit-preservation is summarized. Findings and results vindicate the application of allogenetic ossicles outside large ENT hospitals too.

[Histomorphology and resorption of longtime-implanted cartilage chips conserved in merthiolat in rhinoplasty (author's transl)].

A histological study is presented on 18 cartilage grafts, conserved in Merthiolat TM, which had been implanted for reconstruction of the caudal border of the nose for time periods of 1 month up to 11 years. These cartilage graft preparations were compared to non graft cartilage from patients died in an age of 37, 51 and 53 years respectively. Our findings show that cartilage grafts are eroded from the edge by resorption granulations of the host. Besides resorption one case showed purulent chondritis and another one osteoneogenesis. The extent of the erosion and the resorption rate seem to depend strongly on the original quality of the cartilage graft. This is possibly due to degenerative focuses which could be found in all of the natural cartilage preparations. After our opinion these focuses regarded like "loci minoris resistentiae". Therefore cartilage material to be used for rhinoplasty ought to be checked for such degenerative focuses.

[Artificial mesh in the reconstruction of the posterior auditory canal wall (author's transl)].

Mersilene mesh was used in the reconstruction of the posterior wall of the auditory canal in 12 radical mastoidectomy and attico-antrotomy cavities. Reestablishment of a stable metal wall occurred in 7 patients. In 2 the mesh had to be covered with soft tissue in a second procedure. 3 implants required removal. This technique because of the 25% failure rate can only be recommended reservedly.

[Incus, controlled deepfrozen or in cialit conserved, after allogenetic implantation. An animal-experimental study with comparing histology (author's transl)].

The histological reactions to the implantation of allogenetic incus after conservation in Cialit or after controlled deepfreezing are described. After 5 months the incusses of both conservation technics show the same good tendency of revitalisation. Therefore the controlled deep-freezing technic does not offer an advantage compared to the Cialit conservation method at least for the present state of knowledge.

[The implantation of lyophilized and gamma-sterilized allogenetic incus and of lyophilized and gamma-sterilized xenogenetic malleus (calf) in rabbits and humans (author's transl)].

The histological reactions to the implantation of allogenetic and xenogenetic lyophilized and gamma-sterilized material into the middle ear of rabbits and men is described. In contrary to the behaviour of allogenetic material the xenogenetic shows a clear minor tendency of revitalisation. The reason is the antigen-antibody-reaction which takes place at the capillary network of the donor and leads to a hylinosis with following obliteration of the vascular rete. Therefore the angiogenetic osteogenesis cannot develop. The clinical usuability of such material is discussed.
