[Metastatic involvement of pelvic lymph nodes in relation to the morphological differentiation grade and clinical status of prostatic cancer].

The relationship of cytological and histological grade and local tumour extent to pelvic lymph node involvement was assessed surgically in 92 patients, who underwent pelvic lymphadenectomy alone (31 cases) or together with radical retropubic prostatectomy for adenocarcinoma of the prostate (61 cases). A direct correlation was observed between the histological and cytological grade and clinical stage and node involvement. A total of 26% complications were observed after the lymphadenectomy.

Immediate mechanical reactions of isolated human detrusor muscle on x-irradiation.

The mechanical activity (isotonic registration) of isolated detrusor (smooth) muscle preparations from the human urinary bladder (surgical material) is investigated. The preparations reacted on X-irradiation (50 kV) with an immediate dose-dependent tonic contraction at threshold-doses of 100 to 1000 R (1 to 3 kR/min). The contraction was either reversible, semireversible or persistent, and the basal muscle tone was elevated. A tachyphylactic reaction was observed. The phasic activity of the preparations was temporarily increased, then inhibited after irradiation. The radiation-induced changes of the human and guinea-pig detrusor muscle are compared and their possible relation to the functional disturbances of the urinary bladder after radiotherapy is discussed.

Tumors of the epididymis.

Tumors of the epididymis, both primary and secondary, whether benign or malignant are very rare. The elements of the spermatic duct system, whether inside the testis or outside it in the epididymis and spermatic cord, exhibit only 1/20th of that of the germinal epithelium. In accordance with the mesodermal, or mesothelial origin of the whole spermatic ducr system, tumors of the epididymis and spermatic cord are - with few exceptions - mesenchymal tumors, the most frequent of these being the so-called adenomatoid tumor, a benign slowly growing tumor. With regard to the relative frequency of the various extratesticular tumors, a British panel has shown that one quarter of its cases are adenomatoid tumors, a second quarter sarcomas of children and juveniles (rhabdomyosarcomas and embryonic sarcomas), a third quarter sarcomas of adults and aged, while the last quarter includes simple benign mesenchymal tumors, mixed tumors, metastatic tumors, and others. There is only one extremely rare epithelial tumor of the epididymis, the papillary cystadenoma, which seems to be a partial menifestation or forme fuste of Hippel-Lindau's disease. So far only 40 cases have been described in the world literature. Therapy of intrascrotal but extratesticular tumors always ought to be surgical: if the tumor proves to be benign, simple excision suffices. In cases of malignancy, immediate inguinal lymphadenectomy and - as with testicular tumors - lymphography followed by retroperitoneal lymphadenectomy. Whether one should, in principle, then continue treatment either chemotherapeutically or radiotherapeutically, or do so only when there is evidence of metastasis, is a question whiich still remains open.

[The influence of renal artery occlusion on tumor growth of experimental nephroblastomas (author's transl)].

Nephroblastomas are induced in rats with N-Methyl-N-Nitroso-Urea, and selective renal artery occlusion is performed. This procedure has the same effect like occlusion by embolization. The effect of renal artery occlusion on the growth rate of nephroblastmas is controlled by angiography and gross and microscopic examinations up to 70 days following ischemia, the results are compared with a group of untreated nephroblastoma rats. There is a marked reduction of tumor size and a decrease in tumor proliferation. There is an immediate tumor cell death induced by acute and complete ischemia. Collateral blood vessels cause residual arterial blood supply of tumor parenchyma. There seems to be a correlation between collateral blood delivery and tumor size. Even 70 days after permanent ischemia there are areas of obviously absolute normal tumor cells. The conclusion of this experimental study demonstrated that growth rate of tumors can be reduced by ischemia although potentially malignancy still remains. Clinical embolization therapy is justified only in nonoperable patients with hypernephroma and with massive hematuria.

[Operative theatment of male pseudohermaphroditism (author's transl)].

The operative treatment of two patients with male pseudohermaphroditism is reported. The patients reported were girls whose psychosexual attitude indicated a male sexual change. During several procedures the vagina was removed, the urethra was extended according to the method of Denis-Browne and a functioning penis was formed using a skin flap plasty supported by rib cartilage or an artificial prosthesis.
