[Clinical and neurophysiological features and outcome prognosis criteria of Guillain-Barre syndrome in children].

Clinical and electroneuromyographical (ENMG) study of 24 children with Guillain-Barre syndrome (GBS) was conducted. Using ENMG, axonal and demyelinating GBS variants differing by course and rehabilitation time were distinguished. To predict a development of bulbar and respiratory disorders and to evaluate their severity and duration, diaphragm and adductive nerves conduction was investigated. Factors influencing motor function rehabilitation rate in GBS were the following: mean distal M-response amplitude during peroneal nerve stimulation, proximal to distal amplitudes ratio, M-response amplitude for femoral nerves. Three types of ENMG parameter rehabilitation in children with GBS (linear, stepped and undular) which characterize different rates of rehabilitation process are described.

[Diabetic polyneuropathy in children and adolescents].

Clinical electroneuromyography (ENMG) was carried out in 63 children aged 2-17 years with polyneuropathy (PNP). Correlations were studied between the results of the laboratory studies and ENMG evidence, duration, severity of diabetes mellitus (DM). Diabetic PNP was characterised as demyelinating motor-sensory or sensory PNP. ENMG signs of axonopathy and of a sensory block of the conductivity through the nerves of lower extremities at early stages of DM were estimated as prognostically unfavourable ones in terms of trophic complications development. Duration of DM had some influence on the nervous conductivity, while axonal function was more stable. The factor that determined the severity of PNP course was the age of DM onset. It resulted in the necessity of more accurate control because of the risk of PNP development in children with DM onset at the age of 12-13 years. At early stages of DM conductivity through peripheral nerves correlated with the level of glycosylated hemoglobin and with the frequency of hypoglycemic states.

[The clinico-electroneuromyographic characteristics of acute inflammatory polyneuropathies in children].

Clinical electroneuromyographic examination (ENME) was carried out in 59 children at the age of 1-12 years with acute demyelinating polyneuropathies (PNP). The patients were divided into two groups, namely: 24 children with Guillain-Barré [correction of Giyenne-Barret] syndrome (the first) and 35 ones with diphtheritic PNP (the second group). The main clinical symptoms were investigated in each group. As a result of ENME investigation the axonal and demyelinating types of PNP's beginning were identified. They were distinguished both by severity of course and by rate of restoration. Diphtheritic PNP was characterized by demyelinating type of pathology rise with addition of axonal component in severe cases. The conduction through the additional nerves was investigated to prognose the development of bulbar disorders in PNP.

[Echocardiography in the diagnosis and evaluation of the results of the surgical treatment of interatrial septal defects in adult patients].

Pre- and postoperative echocardiographic findings in 67 patients above 18 years of age with atrial septal defects were compared with hemodynamic data obtained at catheterization of heart cavities. No possible ways of accurate noninvasive assessment of arteriovenous discharge were found. Echocardiographic parameters of the followed-up patients returned to normal within 3 to 4 years after surgical correction of the defect. In some cases where mitral prolapse was found preoperatively, angiographically documented mitral regurgitation was seen at long-term follow-up.

[Echography of the right sections of the heart (methodological aspects of ultrasonic location)].

In location of the cardioelements from the intercostal spaces at the left sternal border, the anteroposterior directions of the ultrasonic beam are called standard positions of the sensor, while the right directions of the beam--the right positions of the sensor. In definite cases the use of the right positions of the sensor makes it possible to record the right cavities of the heart. Depending on the foreshortening of the locating impulse, considerable variability of the shape of triacuspid valve movement is noted. The pulmonary valve is usually represented on the echogram at a depth of 3--5 cm by a thin linear structure which moves away from the thoracic wall rapidly during the ejection period. It is presumed that the recording of the right parts of the heart will yield valuable diagnostic information in echocardiographic examination.

[Echocardiographic diagnosis of rheumatic mitral valve insufficiency].

Echocardiography was used for the examination of 45 patients with rheumatic mitral valve insufficiency. Echocardiographic data were found to permit making the diagnosis of mitral valve disease and differentiating within the group of mitral valve disease as such. A mitral disease with prevailing insufficiency is characterized on an echocardiogramme by a unidirectional diastolic movement of thickened mitral cusps, by an increased general excursion of the mitral valve, dilatation of the left ventricular cavity, increased systolic excursion of the ventricular septum, and a significant dilatation of the left atrial cavity. "Pure" mitral insufficiency is characterized by a dilatation of the left heart, vari-directional movements of the mitral cusps during the diastole, and a marked lack of their closure during the systole.

[Mucosal immunity in children with HIV infection and the possibility of correcting this immunity].

The influence of bacterial lysate [see text]PC-19 on the mucosal immunity of children with HIV infection was evaluated. The action of this topical immunomodulator was found to increase the synthesis of sIgA, which was indirectly reflected in a rise of the local immunity of the upper respiratory ways, preventing the aggravation of the chronic focal infection and the main disease. It should be pointed out that in a group of HIV-infected children and adolescents with an initially high level of salivary IgG the prescription of preparation [see text]PC-19 led to its considerable decrease. A decrease in the level of IgG and a rise in the content of sIgA in saliva under the action of the preparation correlated with a decrease in inflammatory changes in the nasopharyngeal mucosa. On the basis of results obtained in this study additions to the algorithm of the prophylactic medical observation of children and adolescents with HIV infection have been developed.
