Non-alimentary aspects of the irritable bowel syndrome.

The irritable bowel syndrome (IBS) is characterized by alimentary and non-alimentary symptoms; the aim of this study was to assess the clinical value and discriminant function of non-alimentary symptoms in the irritable bowel syndrome. Alimentary and non-alimentary symptoms, estimated daily dietary fibre intakes, ability to express personal feelings, anxiety and depression ratings, and life events and difficulties inventories were compared in 128 hospital out-patients with IBS and 113 age and sex matched control subjects not seeking health care, randomly recruited from the community. Alimentary symptoms correlated closely with those non-alimentary symptoms often associated with the hyperventilation syndromes, difficulties in expressing personal feelings and anxiety, and depression ratings. Life events and difficulties, and estimated daily dietary fibre intakes did not differ significantly between patients and controls. With the exception of abdominal bloating, no significant gender differences were observed in IBS symptoms in either the IBS or control groups. The irritable bowel syndrome is one facet of a more general condition of illness behaviour which includes the hyperventilation syndrome and an inassertiveness in expressing personal feelings. Though non-alimentary symptoms did not improve diagnostic accuracy beyond that achieved by a combination of alimentary symptoms, their recognition provides alternative approaches to the management of refractory IBS.

Job sharing in general practice.

A questionnaire survey of 500 consecutive patients consulting their general practitioners was undertaken to compare job sharing part time partners and full time partners in respect of patients' perception of and satisfaction with the availability of the doctor they wished to consult. Comparison of linear analogue scales of patients' satisfaction with the availability of their chosen doctor showed no significant difference between job sharing partners and full time partners. Nevertheless, significantly fewer patients were able to see the full time partner of their choice within two days than were able to see the job sharing partner of their choice within the same period. In this study patients were as satisfied with the availability of job sharing partners as they were with that of full time partners; the findings highlight important considerations for practices wishing to appoint partners with a limited commitment.

Use of the Michigan alcoholism screening test in general practice.

The Michigan alcoholism screening test was used to assess the prevalence of alcohol problems in 142 patients aged between 18 and 60 years consulting a general practitioner. Previous and/or current alcohol problems were identified in 33% of men and 4% of women. Unemployment was found to be significantly associated with alcohol problems in the men of the study group. The higher proportion of patients with alcohol problems requesting consultation compared with those without such problems did not appear to be associated with a specific presenting complaint.Screening for alcohol problems should be undertaken in all patients consulting a general practitioner if early detection and intervention are to be achieved.

Control of severe thyrotoxicosis with potassium iodide and propranolol.

Conventional preparation of thyrotoxic patients for surgery using thiouracils and iodides may not be possible because of either, inability to obtain satisfactory therapeutic levels of the drugs or idiosyncratic reactions to the drugs. An alterative regime using potassium iodide and propranolol in combination has previously been shown to be both safe and effective in the pre-operative control of mild to moderate thyrotoxicosis. The use of this combination in a patient with severe thyrotoxicosis is described. Potassium iodide and propranolol can be used successfully to prepare patients with all degrees of thyrotoxicosis for surgery.
