[Spontaneous hemorrhage and arterial hypertension as complication of Adrenal Pseudocyst. Report of a case].

Adrenal Pseudocyst are uncommon and asymptomatic tumors. We report an unusual case who had previous high blood pressure and acute hemorrhage presented with abdominal pain and shock. Diagnosis was made with ultrasonography and computed tomography revealed the presence of large retroperitoneal hematoma around the superior pole of the left kidney. Urgent surgery was made with a complete excision of a 10 cm. tumor with preservation of adrenal tissue and the left kidney. Histopathological diagnosis was: Adrenal Pseudocyst. Blood pressure normalized after surgery.

[Intrascrotal metastasis in a renal cell carcinoma].

The present article reports a case of intrascrotal metastasis of renal adenocarcinoma. This is an unusual case. A 66-year-old male patient undewent right radical nephrectomy and cavotomy for renal cell carcinoma with renal vein infiltration and thrombus in cava. Six months later the patient present with a nodulous enlargement intrascrotal and roots of penis. And he died 15 moths after nephrectomy. Usually intrascrotal metastases are a late event in the course after detection of a renal carcinoma.

[Cystic testicular lesions in infancy].

The present article reports a case 11 month-old infant with a right intratesticular cyst. We analyze the etiology, differential diagnosis and management off all cystic lesions of the pediatric testis. Patient age at presentation, examination features, tumor markers and sonographic appearance may assist in making a presumptive and occasionally definitive diagnosis preoperatively. The differential diagnosis include intratesticular simple cyst, epidermoid cyst, tunica albuginea cyst, testicular teratoma, juvenil granulosa cell tumor-gonadal stromal tumor, cystic dysplasia of the rete testis, cystic lymphangioma, and testicular torsion. Usually enucleation is the best treatment. A thorough understanding of potentially cystic testis lesions in children leads to the best management choices and often to preservation of a substantial portion of the affected testis.

[Right renal agenesis and ureter ectopic abouchement in cystic dilation of seminal vesicle].

The association of a seminal vesicle cyst with ipsilateral renal agenesis was initially reported in 1914 by Zinner, and it is a very rare congenital anomaly. The association of this anomaly with an ectopic ureter entering the seminal vesicle cyst is even less common. In these cases, clinical symptoms appear in patients 15 years or older due to the distention of the seminal vesicle caused by the secretions of the reproductive tract. Perineal discomfort and recurrent epididymitis are the most often presentations. Urogenital upper tract image studies are mandatory in the management of a cystic pelvic mass and magnetic resonance imaging (MRI) is the best of this radiographic studies, that also reveals the ectopic ureter draining into the seminal vesicle. We report an additional case of this rare congenital anomaly where only MRI provided a correct preoperative diagnosis and a right surgical approach.

Self-expanding polytetrafluoroethylene covered nitinol stents for the treatment of ureteral stenosis: preliminary report.

PURPOSE: We evaluated the treatment of ureteral stenosis using a self-expanding nitinol stent covered with expanded polytetrafluoroethylene (ePTFE). MATERIALS AND METHODS: Between January 2001 and April 2003, 37 ePTFE covered metal stents were implanted in 20 patients with a total of 29 ureteral stenoses due to various causes. In general the stents were introduced by using a retrograde approach under combined endoscopic and fluoroscopic guidance with patients under spinal block and sedation. Followup assessments in 18 patients, including urography, were done 3, 6, 12, 18 and 24 months after the procedure. Followup endoscopic examinations were performed at 6 and 12 months in 8 patients. RESULTS: Immediate ureteral patency was achieved during all insertion procedures and maintained in most patients. Four patients died of the neoplastic process 3 to 12 months after implantation. Four stent migrations occurred in 3 patients (22.2%), which was resolved by implanting a new stent. Nonobstructive mucous hyperplasia was observed in 5 patients (27.7%) at the end of the stent, although there was no internal calcification. In all cases the stents remained patent until the current time or until patient death. CONCLUSIONS: We found that ePTFE covered nitinol stents were safe and effective for ureteral stenosis. Their resistance to calcification was high with nonobstructive hyperplasia developing in only a few cases and only at the stent ends. The ideal material for covering stents would produce no mucous hyperplasia.

[Traumatic intrathoracic herniation of the left kidney].

The detection of a intrathoracic kidney is an exceptional discovery. There is a large period of time between the injury and its clinical presentation revising the literature, so it is possible that the herniation of the kidney into the chest was caused by a rise in intra-abdominal pressure but going through a preexistent congenital way not caused by the traumatism. In order to dismiss this possibility and to prove the traumatic origin of the diaphragmatic rupture, surgeons have examined the intraoperative characteristics of the rupture's borders. The elevation of the kidney has been said to be progressive because of the positive intra-thoracic pressure and the negative intra-abdominal one. Our objective is to report an extraordinary case of abdominal traumatism whose clinic and radiological proofs let us get a rapid diagnosis and surgical demonstration of the direct herniation of the left kidney into the chest through a diaphragmatic rupture caused by the same traumatism.

[Endoscopic treatment of ureteral lithiasis. Our experience with 360 retrograde uretero-renal endoscopies in the last ten years].

Presentation of results and complications obtained in 360 procedures of transuretral ureterorenoscopy (URS) in ureteral lithiasis performed in our unit from january 1990 to august 2000 in 354 patients: 55% female and 45% male. A rigid ureteroscopy Storz 10.5 Ch. was used and intracorporeal lithotripsy was necessary in 17.31% of cases. URS indications were always treatment of ureteral lithiasis, pelvic ureter localization in most cases (70.33%) being 93.05% the percentage of overall success and with a significant decrease when the calculus was located in the upper third of the ureter. Serious complications were only 3.05% of cases and endoscopic surgery was necessary in three cases of ureteral stenosis. In our experience, URS is our technique of choice for treatment of lower and medium third of the ureter where the percentage of success we have obtained were 98.99% and 95.83% respectively.

[Hypercalcemia, leukemoid reaction, and thrombocytosis as paraneoplastic presentation of transitional cell carcinoma of the kidney].

The most common etiologies of hypercalcemia are hyperparathyroidism or malignancy, most often of the lung, breast or hematological system, but rarely occur in transitional cell carcinoma. The appearance of this metabolic disorder and other paraneoplastic signs like leukemoid reaction or thrombocytosis is a potential marker for malignant behaviour and poor prognosis in advanced tumours. Simultaneous presentation of that three paraneoplastic signs have only been described in two patients with wellknown bladder carcinoma. Most paraneoplastic syndromes appear only during the late stages of malignancy when the diagnosis has long been established, but sometimes may be an early sign of the malignancy. We describe a case of a previously unknown infiltrating transitional cell carcinoma of the kidney without bony metastasis, that presented itself with hypercalcemia, hyperleukocytosis resembling a leukemoid reaction and thrombocytosis. We think recognizing the paraneoplastic syndrome is very important because it is an alarm sign of malignancy.

[Hypertension emergency during surgery of renal carcinoma associated with non-diagnosed pheochromocytoma].

OBJECTIVE: To present an uncommon association of renal carcinoma with an undiagnosed ipsilateral pheochromocytoma that caused severe cardiovascular disorders during surgical treatment of the renal carcinoma, and emphasize the need for careful assessment of these adrenal nodules before resection although they may appear to be clinically irrelevant. METHODS: A 61-year-old male with mild arterial hypertension controlled with drug therapy for 40 years was diagnosed as having a right renal hypernephroma and a probable adrenal adenoma by IVP, abdominal ultrasound, CT, bone scintiscan and renal arteriography. Preoperative values were within the normal limits. The patient developed severe hypertension intraoperatively, which was controlled by administration of lidocaine, nitroglycerine, sodium nitroprusside and labetalol. Surgery was interrupted and completed when the patient was stable. RESULTS: The pathological analysis showed clear cell renal adenocarcinoma and pheochromocytoma. Postoperative evaluation showed dopamine, adrenaline, noradrenaline and vanilmandelic acid were normal. A 131 MIBG scan was normal. CONCLUSION: Although an adrenal mass may appear to be clinically irrelevant, a careful study should be performed to discard a functioning tumor and to avoid the serious complications that might arise during surgery.

Adrenal Gland Neoplasms↗

[Granulomatous orchitis].

Granulomatous orchitis is an inflammatory change of the testis. This is a rare lesion of unknown etiology. Usually are unilateral. The clinical appearance varies, and it is difficult to differentiate from testicular cancer. The diagnosis usually being made on histological examination after orchiectomy. Our paper report one case of granulomatous orchitis in a 27 year old patient. A review of the literature is made on the diagnosis and pathogenesis.

Effect of venlafaxine hydrochloride in different preparations of isolated guinea-pig and rat organ tissues.

A study was undertaken to know better the effects of venlafaxine hydrochloride on the responses of isolated rat vas deferens to noradrenaline and dopamine, those of isolated rat uterus to serotonin and histamine, and those of isolated guinea-pig ileum to acetylcholine and histamine. Venlafaxine hydrochloride increased the response of rat vas deferens to noradrenaline but not to dopamine. Venlafaxine did not alter the response of rat isolated uterus to serotonin. In rat uterus, venlafaxine did not modify the response to histamine but was able to increase it in guinea-pig ileum. An anticholinergic effect was observed with the lowest concentration tested. Although venlafaxine is a selective serotonine reuptake inhibitor in the central nervous system, serotonin uptake was not seen in the rat uterus. The anticholinergic effects observed in the present study might be consistent with some of the side-effects associated with venlafaxine.

[Giant renal angiomyolipoma].

OBJECTIVE: To present a cae of giant renal angiomyolipoma that required surgical treatment owing to its size and concomitant intratumoral hemorrhage. METHODS: The characteristics of the case are presented and discussed. RESULTS/CONCLUSIONS: Giant renal angiomyolipoma is one of the most frequent causes of Wünderlich syndrome and is diagnosed by ultrasound and CT. The decision to operate is based on the size of the lesion and/or symptomatology, as in the case described herein, which required a simple nephrectomy.

[Intestinal obstruction secondary to peritoneal carcinomatosis after iatrogenic perforation during TUR for transitional cell carcinoma of the bladder].

OBJECTIVE: A case of intestinal obstruction following transurethral resection (TUR) of a locally advanced transitional cell carcinoma of the bladder is presented. METHODS/RESULTS: The unique features of the present case are described. Laparotomy was performed and peritoneal carcinomatosis was found. Renal function became impaired and the abdominal condition persisted. The patient died five days postoperatively. CONCLUSIONS: Intestinal obstruction following TUR of transitional cell carcinoma of the bladder is an uncommon complication that is mainly due to peritoneal seeding following iatrogenic perforation of the tumor or the bladder wall during resection. Still, TUR is undeniably useful in the diagnosis, staging and treatment of bladder neoplasm.

[Self-expanding metallic endoprosthesis in the treatment of recurrent ureteral stenosis].

OBJECTIVE: To describe two cases of recurrent fibrotic stricture of the uretero-vesical junction treated with a metallic self-expanding endoprosthesis which has successfully achieved patency of the stenotic area in the medium-term. METHODS: Two cases of difficult ureteral stricture are described. Both cases presented obstruction of the terminal ureter by abundant fibrotic tissue in the bladder wall following radical prostatectomy in one case and TUR in the other for non-infiltrating bladder carcinoma. Several attempts to recover patency by endoscopic resection and placement of a double-J stent in these patients had previously failed. The patients were therefore submitted to resection of the fibrotic area and insertion of a self-expanding endoprosthesis (Wallstent), which was eventually enclosed by fibrous tissue at its vesical portion. Both cases underwent resection again and another endoprosthesis (Memotherm) was inserted to extend the previous prosthesis 1 cm within the bladder lumen. RESULTS/CONCLUSIONS: When the endoprosthesis is placed in the uretero-vesical junction with its tip flush with the bladder wall, it is eventually enclosed by fibrous tissue, which invariably leads to recurrent obstruction. We have found that extending the prosthesis 1 cm within the bladder lumen can overcome the foregoing complication, although the long-term results have as yet to be established.

[Testicular changes produced by alcohol].

Acting as a direct toxic through different mechanisms, alcohol causes many hormonal changes which induce sexual function impairment and fertility problems. We have used 36 white Wistar Lewis rats, divided in three large groups: 10% alcoholic, 25% alcoholic and control group for 55 and 95 days. The parameters studied were the rat's ponderal evolution, testicular volume and weight, testes and body weights ratio, as well as testes density and seminiferous tubule area. We found a decreased ponderal gain in animals receiving high doses of alcohol. Decreased testicular volume and increased density in long-term alcohol regime. At high dose and medium-term increased testicular weights were seen as compared to total weight of the rat. The area of the seminiferous tubule is reduced in all alcoholic groups.

[Relation between renal transplantation and work situation].

OBJECTIVE: The present study analyzed occupational rehabilitation following renal transplantation and its impact on social costs. METHODS/RESULTS: A survey was conducted on 493 patients who had undergone renal transplantation during 1979 to 1995. The results show that 27.76% were working after kidney transplantation versus 27.52% during dialysis. Retirement was the most common explanation given for not working (29.4%) when the percentage of those over age 65 accounted for 8%. CONCLUSION: Analysis of the occupational status in relation with sex, age, level of education, profession and type of dialysis disclosed that women aged 25-34 years, with a high level of education, who describe themselves as entrepreneurs and have had peritoneal dialysis, have a higher likelihood of working after transplantation.

[Sexual changes in men with urological disorders].

OBJECTIVE: To evaluate the changes in sexual function due to urological pathologies and their treatment in the male. The present study analyzed erection, ejaculation, frequency of sexual intercourse and orgasm. METHODS: A questionnaire comprised of 30 items were sent out to 1016 males over 18 years of age with urological disease that had been treated from 1990 to 1991. Data processing was carried out using the Statgraphics and Statistica programs. The x2 and Wilcoxon Z tests were utilized for statistical analysis. RESULTS: All of the parameters studied were found to be impaired in a high number of patients in relation to age, pathology and treatment. Benign prostatic hyperplasia and its treatment caused the most impairment. CONCLUSIONS: Erection, ejaculation, frequency of sexual intercourse and orgasm decreased significantly after a urological condition. In our view, patient preoperative data and analysis of the foregoing parameters are warranted.
