[Current trends in the treatment of flail chest].

The authors make a wide-ranging review of the literature, examine the epidemiological aspect of fail chest and underline the importance of its correct anatomopathological classification for a more qualified physiopathologist, diagnostic and therapeutic approach. They make a historical excursion on volet's treatment and underline the complementarity of surgical and resuscitation therapies.

[Angioplasty of the deep femoral artery using a semirigid PTFE prosthesis. Experimental studies].

Underlining the importance of Profundus Femoral Artery and its capacity to make up for lower limb's haemodynamics during obliterative arteriopathy, the Authors conceive a funnel-shaped intravascular prosthesis, Goretex constructed (PTFE), to introduce, in case of obstruction, in the artery lumen with a suitable instrument. The research is performed through ten oldster dogs whose average weight is twenty-four kilograms. Isolated the deep Femoral Artery and carried out an arteriotomy of about two centimetres on the Common Femoral Artery, we bring the prothesis in Profundis Femoral Artery and we fix it to the wall of the Common Femoral Artery with 8/O microsurgical dots. After thirty days, the arteriographic test shows the patency of the endoprosthesis with normal peripheral flow in nine animals out of ten. By virtue of this preliminary experiment, the Authors point out the semplicity of this intervention, also feasible in local anaesthesia, the good tolerability of the prosthesis and its perfect integration with the surrounding arterious wall, without shifts or dissecting laminar flows between the external prosthesis wall and the vessel lumen of the animal.

[Chyloperitoneum. Presentation of 2 cases].

The authors report two cases of chylous ascites recently observed. After a wide review of the literature, they point out the rarity of this disease and its particular therapeutic aspects.

[Ultrasonography in abdominal emergencies].

From February 1986 to March 1988 113 abdominal US exams were performed in emergency situation to evaluate the accuracy of this methodology: 13 were blunt traumas, 18 postoperative complications. A real-time scanner with a linear probe of 5 MHz was employed. The results were confirmed by surgical and/or clinical and instrumental evaluation. In 81% of the examinations, ultrasonography allowed a diagnosis to be made. Gallbladder and biliary pathologies were the most common findings. The results (sensibility 96%, specificity 88%, accuracy 95%) confirm the reliability of ultrasonography in abdominal emergencies, as shown in literature.

Primary ureteral carcinoma. Experience in a general surgical service.

Five cases (3M, 2F) of ureteral transitional cell carcinoma are reported. they have been observed in the last two years in a general surgical service. All the patients have been treated surgically. Gross hematuria was present in all the patients. Three cases had multicentric tumor sites. Three tumors were located in the distal ureter. Four cases showed a non functioning kidney at IVP. In two, because of obstruction, retrograde pyelography was unsuccessful. Ultrasound was able to demonstrate hydronephrosis. In the two cases submitted to angio-CT the spread of the tumor was correctly diagnosed. Frequency, etiology, clinical, diagnostic findings and surgical therapy as well have all been taken into account, based on the literature. Histopathologic aspects, grade and stage, are considered too. The authors emphasize the necessity to find and understand the cause of an unexplained hematuria, since ureteral carcinoma is not as rare as previously thought. Being grade and stage closely related to prognosis, the diagnostic delay must be reduced.

[A case of thymoma: histological and diagnostic aspects and surgical considerations].

The histological and diagnostic features of a thymoma case observed recently have been investigated in the present study. Comparing the case under investigation with the literature, the Authors conclude that the proper surgical approach is that indicated by Jaretsky. By means of a longitudinal sternotomy associated with minimal cervical incision, the whole mediastinal cellular tissue together with that of the thyroid inferior poles is easily removed. Moreover oncological and myastenic recurrences have a very low incidence in cases surgically treated with this procedure.

Surgical treatment of multinodular goiter: incidence of lesions of the recurrent nerves after total thyroidectomy.

The problems concerning surgical goiter treatment are discussed, particularly the morbidity of total thyroidectomy and possible injury to recurrent nerves in comparison to the partial resection of the gland. We discuss our case material and review the literature. We conclude that total thyroidectomy seems to be the most effective surgical procedure with lower morbidity than subtotal thyroidectomy.

[Is blood transfusion a risk factor in colorectal surgery for cancer?].

Personal experience in the treatment of 93 cases of cancer of the large bowel and rectal localization is reported 32 patients (A group) received blood transfusions, 61 patients (B group) hadn't any transfusions or autotransfusions. Postoperative morbidity was 34.37% in A group and 14.75% in B group. The Authors stress the basic importance of autotransfusions as well in patients surgically treated for colorectal cancer.

Role of surgery in ductal carcinoma of the pancreas.

Data of 111 patients with ductal carcinoma of the pancreas examined over a decade (1979-1989) at the 1st Department of Surgery, "La Sapienza" University of Rome, are presented. 21.6% of them underwent pancreatic resection and 40.5% biliodigestive diversion. Resectability was 26.5% for tumors of the head, 11.8% for tumors of the body and tail, nil for diffuse tumors. Overall operative mortality was 13.5%. Only stage I patients were shown to be resectable for cure and benefited from surgery with 21% probability of 5-year survival.

Variations of telomerase activity in cultured mouse fibroblasts upon proliferation of polyomavirus.

Telomerase plays a central role in various biological phenomena such as cell differentiation and proliferation, apoptosis, malignant transformation and virus infection, for instance HIV and papillomavirus. In addition, it has recently been shown that, in human fibroblasts transformed by monkey polyomavirus SV40, telomeres became stabilized as a consequence of telomerase activation. However, no information exists on the effects of acute infection by murine polyomavirus on the telomeres maintenance and telomerase activity in the host cell. In this paper we report on a differential activity of telomerase in productively infected cells. The results showed a decreased activity of the enzyme as assessed by the TRAP assay. The decrease had already occurred at a non-lytic time of infection and was observed both after infection and naked DNA transfection. Therefore nuclear decapsidation is not involved in the determination of the phenomenon that is attributed to the proliferation of the virus.

[Current status of the emergency surgical management of closed trauma of the thorax].

The authors discuss problems related to emergency surgical management of non penetrating thoracic injuries. With reference to their own experience and current literature, they underline the etiologic, pathophysiologic, diagnostic and therapeutic features of the most frequent indications to the surgical treatment.

[Leiomyosarcoma of the rectum].

The Authors report a case of leiomyosarcoma of the rectum. Recent literature is reviewed and some problems related to clinico-pathological, diagnostic and therapeutic aspects of this neoplasia are also discussed.
