[Microsurgical treatment of posterior capsule opacification].

PURPOSE: To evaluate irrigation/aspiration and posterior capsulectomy in the treatment of the posterior capsule opacification (PCO). SETTING: Department of Ophthalmology, School of Medicine, Comenius University, Bratislava, Slovakia. MATERIAL AND METHODS: Between 1.1.1999 and 31.7.2000 we treated 75 patients (79 eyes) for PCO. Cataract surgery was performed in the years 1990-1999. The artificial intraocular lens (IOL) was implanted in 62 eyes. The interval between cataract surgery and PCO surgery ranged from 4 m.-10 y. The regenerative type of PCO was present in 41, fibrotic in 23 and combined in 15 eyes. Irrigation/aspiration (I/A) was performed in 44 eyes. Capsulectomy with a vitrectomy probe was performed in 35 eyes. I/A procedure was 42x performed with a bimanual system. Capsulectomy was performed in combinations of various insertions of the infusion line (without, anterior chamber maintainer-AChM, pars plana) and capsulectomy (limbal or pars plana approach). It was evaluated: occurrence of peri- and postoperative complications; achieved best corrected visual acuity. RESULTS: Complications during I/A: posterior capsule tearing with vitreous prolaps 4x, IOL subluxation 3x, partial zonular dialysis 2x-1x with vitreous prolaps. During capsulectomy performed through the limbus was observed vitreous prolaps in two eyes. After capsulectomy, performed through pars plana without infusion line, was observed the chronical corneal oedema in one eye. The best corrected visual acuity improved for 2 and more lines in 79.5% of eyes after I/A procedure and in 74.3% of eyes after capsulectomy. The achieved visual acuity lower then 6/60 was caused by changes not related to the cataract and PCO surgery. After testing of various combinations, the most suitable capsulectomy procedure seems to be: infusion line through the limbus, capsulectomy through the radial scleral tunnel incision. In this fashion it is possible to perform the procedure as no-stitch surgery. CONCLUSIONS: Instead of possibility of occurrence of various peri- and postoperative complication after I/A and capsulectomy used for treatment of PCO these procedures complete possibilities in the treatment of PCO. For the regenerative type of PCO is suitable I/A procedure. For the fibrotic type of PCO is suitable capsulectomy through pars plana with AChM. By this procedure it is possible to remove also nearly all vitreous opacities.

Stratification of multiple myeloma according to serum beta 2-microglobulin and serum albumin levels.

In 71 patients with multiple myeloma the authors evaluated the clinical significance and the prognostic relevance of Bataille's stratification system based on serum beta 2-microglobulin and serum albumin levels. This staging system, which classified the patients to low, intermediate and poor risk groups (stages 1-3) showed a rather small prognostic significance, and it stratified our myeloma patients unsatisfactorily. On the basis of the results of our own analyses, we have developed our own modification of a pilot stratification system. We have verified its good clinical applicability and its powerful prognostic prediction. In this study, the good prognostic significance of serum beta 2-microglobulin and albumin levels (with the best cut-off levels of 4.0 mg/l and 35.0 g/l) and a good correlation between both analysed markers were confirmed.

[Serum beta 2-microglobulin in multiple myeloma. II. Its significance in monitoring the disease].

In a group of 71 patients with multiple myeloma the importance of beta 2-microglobulin (S-B2M) serum levels was evaluated with regard to their importance for monitoring of the disease. No significant relationship was found between B2M levels and monoclonal serum immunoglobulin, only in one third of the patients parallel changes of the two proteins were observed. One third of the patients had permanently normal S-B2M values and thus could not be evaluated with regard to the therapeutic results, 9% of the patients had very low S-B2M values throughout the disease regardless of the high activity of the latter and the marked increase of myeloma mass (stage III A). "Non-corrected" values of S-B2M proved useful in the evaluation of therapeutic results in patients with primarily elevated S-B2M values and satisfactory renal function but not in patients with elevated serum creatinine values. Normal or only slightly variable S-B2M values were part of the plateau phase of the disease, while during the relapse a rise of varying speed and extent occurred. S-B2M appears a suitable, though in some patients only supplementary, indicator for the long-term follow-up of the course of multiple myeloma.

[Serum beta 2-microglobulin in multiple myeloma. I. Relation to selected indicators, clinical stage and disease prognosis].

The authors evaluated in a group of 89 patients with monoclonal gammapathy (18 patients with monoclonal gammapathy of undermined significance, 34 patients examined at the time of diagnosis of multiple myeloma (MM) and in a group of 71 patients with MM examined in different stages of the disease) the serum beta 2-microglobulin. It was revealed that the mentioned indicator is of no differential diagnostic value, it is not related to sex nor to the immunochemical type of monoclonal immunoglobulin. A relationship of serum beta 2-microglobulin to age, serum urea and serum creatinine, to the severity of anaemia, serum albumin, sedimentation rate of red cells, degree of infiltration of bone marrow by myeloma plasmocytes and the stage of the disease, evaluated by the systems of Durie-Salmon and Medical Research Council, was found. The authors tested the importance of serum levels of this indicator for the prognosis of the disease.

Application of ELISA method to determine antigliadin antibodies in children with coeliac disease.

The application of the ELISA method to the detection of antigliadin antibodies (AGA), class IgG and IgA in children with coeliac disease (CD) is described. All the patients of the group of 17 with untreated CD and another group of 19 with CD formerly treated and now gluten exposed show elevated AGA values. The group of 14 CD patients permanently treated with gluten free diet show AGA increase in 42.6% cases. A correlation between the advanced grades of mucosal atrophy and pathological elevated AGA IgA values is found in this group. Patients with acute CD put on gluten free diet display a faster and more marked drop of AGA IgA than AGA IgG in the course of 3 to 12 months. Higher AGA values are found for children with inflammatory bowel diseases. The control group have higher AGA values in 6.45% of instances. Due to its sensitivity (100% by the assessment of both the AGA immunoglobulins) and 93.55% specificity AGA determination represents a valuable screening test both when indicating enterobioptic examination and following the gluten free diet of CD patients.

[Determination of beta 2 microglobulin in malignant tumors of the ovaries].

Beta 2-microglobulin (beta 2-MG) is used as a tumour marker in some malignant diseases. The authors assessed the concentration of beta 2-MG in serum of 30 patients with malignant ovarian tumours and in 29 patients with other benign gynaecological diseases, in particular benign ovarian tumours. Although raised values of beta 2-MG were found in ovarian carcinomas, as compared with the control group of non-malignant gynaecological diseases, a statistically significant increase was recorded only in carcinomas grade III and IV.

Increased serum beta-2-microglobulin level in patients with inflammatory bowel disease.

Beta-2-microglobulin (B2m) serum levels were measured by newly developed electroimmunoassay in a group of patients with inflammatory bowel disease. Increased serum B2m concentrations were found in 66.7% out of 93 subjects with Crohn's disease and in 56.0% out of 25 subjects with idiopathic proctocolitis. According to the author's view, serum B2m can be regarded as a useful adjunct to other immunological markers in diagnostic evaluation and monitoring of Crohn's disease. The high B2m level may reflect elevated biosynthesis and release from immunologically activated lymphocytes in areas of inflammation.
