Automated multiplex assay system for simultaneous detection of hepatitis B virus DNA, hepatitis C virus RNA, and human immunodeficiency virus type 1 RNA.

We have developed an automated multiplex system for simultaneously screening hepatitis B virus (HBV), hepatitis C virus (HCV), and human immunodeficiency virus type 1 (HIV-1) in blood donations. The assay, designated AMPLINAT MPX HBV/HCV/HIV-1 Test (AMPLINAT MPX), consists of virus extraction and target sequence-specific probe capture on specimen preparation workstation GT-X (Roche Diagnostics K.K., Tokyo, Japan) and amplification and detection by TaqMan PCR on the ABI PRISM 7700 Analyzer (Perkin-Elmer Applied Biosystems, Foster City, Calif.). An internal control (IC) is incorporated in the assay to monitor the extraction, target amplification, and detection processes. The assay yields qualitative results without discrimination of the three targets. Detection limits (95% confidence interval) are 22 to 60 copies/ml for HBV, 61 to 112 IU/ml for HCV, and 33 to 66 copies/ml for HIV-1, using a specimen input volume of 0.2 ml. The AMPLINAT MPX assay detects a broad range of genotypes or subtypes for all three viruses and has a specificity of 99.6% for all three viruses with seronegative specimens. In an evaluation of seroconversion panels, the AMPLINAT MPX assay detects HBV infection an average of 24 days before the detection of HBsAg by enzyme immunoassay. HCV RNA was detected an average of 31 days before HCV antibody. HIV-1 RNA was detected an average of 14 days before HIV-1 antibody and an average of 9 days before p24 antigen. The Japanese Red Cross has been evaluating the AMPLINAT MPX system since October 1999. The clinical performance indicates that the AMPLINAT MPX system is robust, sensitive, and reproducible, with a high percentage of valid assay runs (96.8%), a low false-positive rate (0.34%), and a low IC failure rate (0.24%).

Levofloxacin versus cefuroxime axetil in the treatment of acute exacerbation of chronic bronchitis: results of a randomized, double-blind study.

A randomized, double-blind, double-dummy, three-arm parallel design, multicentre study was conducted among adult patients with acute exacerbation of chronic bronchitis (AECB) in order to compare the efficacy and safety of two different doses of levofloxacin with cefuroxime axetil. A total of 832 patients were randomized to receive oral levofloxacin (250 mg od or 500 mg od) or oral cefuroxime axetil (250 mg bd) for 7-10 days. The primary efficacy analysis was based on the clinical response in patients with bacteriologically confirmed AECB, determined 5-14 days after the end of therapy (per-protocol population). Of 839 patients enrolled (at 71 centres in 14 countries), seven were not treated, giving an intention-to-treat (ITT) population of 832. In total, 281 patients received levofloxacin 250 mg, 280 received levofloxacin 500 mg and 271 received cefuroxime axetil. The cure rates in the ITT population were: levofloxacin 250 mg, 70% (196/281); levofloxacin 500 mg, 70% (195/280); cefuroxime axetil, 61% (166/271); those in the per-protocol population were: 78% (121/156), 79% (108/137) and 66% (88/134), respectively. Both doses of levofloxacin were at least as effective as cefuroxime axetil and were active against the main pathogens of clinical relevance (Haemophilus influenzae, Streptococcus pneumoniae and Moraxella catarrhalis). All three treatment regimens were equally well tolerated. In conclusion, the results show that levofloxacin (250 mg and 500 mg) od is effective and well tolerated in the treatment of AECB in adult patients.

Analysis of the exochelin locus in Mycobacterium smegmatis: biosynthesis genes have homology with genes of the peptide synthetase family.

Mycobacteria secrete the siderophore exochelin when grown under iron-limiting conditions. In order to understand iron uptake mechanisms in mycobacteria, we have taken a genetic approach to identify those genes involved in exochelin biosynthesis and transport in Mycobacterium smegmatis. Of the 6,000 chemically mutagenized clones of M. smegmatis mc2155 screened on agar plates containing chrome azural S, 19 mutants that had lost the ability to produce or secrete exochelin were identified. Thirteen of these mutants were complemented by a single M. smegmatis cosmid. Sequence analysis of this cosmid revealed nine open reading frames, three of which are homologous to genes encoding transporter proteins, which are likely involved in exochelin transport. Complementation and Tn10 mutagenesis analysis identified two new genes, fxbB and fxbC, which are required for exochelin biosynthesis. The fxbB and fxbC genes encode large proteins of 257 and 497 kDa, respectively, which are highly homologous to peptide synthetases, indicating that exochelin biosynthesis occurs by a nonribosomal mechanism.

Use of a siderophore detection medium, ethylene glycol degradation, and beta-galactosidase activity in the early presumptive differentiation of Nocardia, Rhodococcus, Streptomyces, and rapidly growing Mycobacterium species.

A simplified scheme for the presumptive early identification of Nocardia, Rhodococcus, rapidly growing Mycobacterium, and Streptomyces species is presented. The Nocardia and Streptomyces spp. and the Mycobacterium spp. were positive. The spp. were positive for siderophore activity, but only 25% of the Rhodococcus spp. were positive. The Rhodococcus and Mycobacterium spp. were negative for beta-galactosidase, while the Nocardia and Streptomyces spp. were positive. The Nocardia and Streptomyces spp. and the Mycobacterium spp. were negative for ethylene glycol degradation, while 75% of the Rhodococcus spp. were positive. In combination, these tests were useful for differentiating Mycobacterium, Rhodococcus, and Nocardia species but did not differentiate Nocardia from Streptomyces species.

Purification of actin from Candida albicans and comparison with the Candida 48,000-Mr protein.

Actin was purified from Candida albicans cells by affinity chromatography by DNase-Sepharose and was recognized by immunoblotting with monoclonal antibody directed against chick muscle actin. The C. albicans 48-kilodalton protein recognized by sera from patients with invasive candidiasis was shown by DEAE chromatography and immunoblotting not to be identical with the purified C. albicans actin.

[Effects of adrenergic beta 2 agonists by oral route on theophylline blood levels].

Several drugs interfere with theophylline pharmacokinetics, inducing changes on it's serum levels. With the purpose of determining a possible influence of oral administered beta-2-adrenergics agents on theophylline serum levels ten patients with bronchial obstruction, whose ages ranged from 51 to 69 (average 59.90 +/- 6.67) years, were analysed. After a period of 24 hours without any medication, the patients received 600 mg of oral slow release theophylline per day, for 7 days, followed by 7 days of the same dose of theophylline combined with 16 mg of oral salbutamol. Blood samples were drawn after the 24 hours without medication and at the 4th hour after the morning dose of theophylline, at the end of each week. The serum levels were, respectively, 2.08 +/- 1.20 micrograms/ml, 15.18 +/- 6.87 micrograms/ml and 11.45 +/- 5.15 micrograms/ml. The levels of the phase (theophylline alone) were significantly higher than those obtained after the concurrent use of theophylline and salbutamol. It can be concluded that this association interferes on theophylline pharmacokinetics. We suggest that in patients taking these drugs the serum levels of theophylline shall be measured to permit an eventual correction of the dose of theophylline being administered.

[Pneumoconiosis in abrasive industry workers].

Occupational lung disease is uncommon in workers of abrasive industries. Twenty-two workers of these industries presenting radiological lung lesions were analysed. All were males with ages ranging from 29 to 59 years. Nineteen worked in the production of silicon carbide, three in the production of aluminum dioxide and one in both for a period of 8 to 27 years. In 20 patients the predominant radiological changes (ILO-80) were small opacities. Association of small and large opacities was observed in one case and isolated large opacities in another one. The small opacities were predominantly round and diffuse. The large opacities in both patients were classified as being of category B. It is concluded that the workers in abrasive industries are at a higher risk of developing occupational lung disease produced by silicon carbide.

[Forced mid-expiratory flow rate (FEF 25-75%): a critical analysis of its value in recognizing diseases of the small airways].

A total of 245 patients submitted to lung function test were studied. They were divided in 2 groups: A--FEV1.0/FVC greater than or equal to 75% and B--FEF25-75% less than 75%. These groups were further subdivided according to the age of the patients--subgroup 1 less than or equal to 50 years and subgroup 2 greater than 50 years. Patients with restrictive disorders were not included. The spirometric equipment employed was Hewlett-Packard Inc. Pulmonary Function Analyzer 47402-A. The results were obtained through a digital system and calculated from a graph impressed in a xy register. Results were automatically corrected to BTPS. In each group FVC and FEV1.0 were normal. In group A FVC and FEV1.0 were, respectively: A1-99.63 +/- 13.33 and 101.11 +/- 12.24, A2-97.60 +/- 11.06 and 98.81 +/- 12.18, expressed as percentage of predicted values. In group B the results were: B1-97.61 +/- 13.55% and 81.71 +/- 14.44%; B2-100.28 +/- 13.98% and 83.10 +/- 16.42%. The differences were not statistically significant at 5%. In subgroup A1 X FEV1.0/CVF was 83.41 +/- 5.63% and in subgroup A2, 79.42 +/- 4.23%. Maximum mid-expiratory flow rate was 88.64 +/- 24.39% in subgroup A1 and 65.26 +/- 17.88% in subgroup A2. These differences were statistically significant at 1%. In group B, FEV1.0/FVC was always less than 75%. In subgroup B1 it was 68.24 +/- 10.43% and in B2, 64.90 +/- 10.43% and values for FEF25-75% were 48.13 +/- 16.48% and 42.79 +/- 17.68%, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)
