Eletriptan vs sumatriptan: a double-blind, placebo-controlled, multiple migraine attack study.

OBJECTIVE: To compare the efficacy of oral eletriptan, 40 mg and 80 mg, and oral sumatriptan, 50 mg and 100 mg, in the acute treatment of migraine. METHODS: Patients with a history of migraine (n = 1,008) were randomly assigned to receive placebo, 40 mg of eletriptan, 80 mg of eletriptan, 50 mg of sumatriptan, or 100 mg of sumatriptan to treat up to three attacks. Early headache response (at 1 hour) was the primary endpoint, in addition to the standard endpoint, 2-hour headache response. RESULTS: Headache response rates were 12% at 1 hour and 31% at 2 hours for placebo; 24% at 1 hour and 50% at 2 hours for sumatriptan 50 mg; 27% at 1 hour and 53% at 2 hours for sumatriptan 100 mg; 30% at 1 hour and 64% at 2 hours for eletriptan 40 mg; and 37% at 1 hour and 67% at 2 hours for eletriptan 80 mg. More patients receiving eletriptan 80 mg achieved a 1-hour headache response than did patients receiving sumatriptan 50 mg (p < 0.05). All doses of eletriptan were superior to sumatriptan at 2 hours for headache response and complete pain relief (p < 0.05). Significantly more patients on eletriptan 80 mg achieved headache response in all attacks than did patients receiving either sumatriptan dose. Eletriptan 40 mg was superior to both sumatriptan doses in functional improvement (p < 0.005). The superior efficacy of both eletriptan doses was associated with higher rates of patient acceptability than sumatriptan 50 mg (p < 0.05). Eletriptan and sumatriptan were well tolerated. CONCLUSION: Oral eletriptan (40 mg and 80 mg) is effective, safe, and tolerable in the acute treatment of migraine and yields a consistent response.

An Australian study on the sociocultural context of menopause: directions for contemporary nursing practice.

This research project by Berger (1997) investigates the physical, psychological, and sociocultural menopause experiences of a group of seventy women aged 45-70 years from Brisbane, Australia. Thus far the narrative provided for mid-life women shows a preoccupation with medical/biological perspectives that emphasize negative images of menopausal women as estrogen deficient and diseased. This new study is considered to be one of the few more recent attempts that investigates menopausal women within a sociocultural context and explores women's views about menopause as a normal, natural transition part of the ageing process. A qualitative approach (comprising focus groups and interviews) was aimed at women in mid-life who were willing to share their insights of this complex phase, to document individual variations and broad patterns. Findings provided enhanced understanding of many menopause aspects (such as appropriate treatment) that remain controversial. Content analysis uncovered three emerging themes: 1) contrary to popular beliefs menopause usually signified wellness; 2) apart from biological changes, the sociocultural context played a pivotal role in modifying menopause experiences; and 3) women did not feel adequately supported by health professionals. The findings signify that nurses are ideally placed to assist menopausal women in a variety of health care settings.

Yield optimization and time structure of femtosecond laser plasma kalpha sources

The generation of femtosecond Kalpha x rays from laser-irradiated plasmas is studied with a view to optimizing photon number and pulse duration. Using analytical and numerical models of hot electron generation and subsequent transport in a range of materials, it is shown that an optimum laser intensity I(opt) = 7x10(9)Z4.4 exists for maximum Kalpha yield. Furthermore, it is demonstrated that bulk targets are unsuitable for generating sub-ps x-ray pulses: instead, design criteria are proposed for achieving Kalpha pulse durations </=100 fs using foils of approximately 2 &mgr;m thickness.

[Neuropsychological aspects of dementia diagnosis].

In the interdisciplinary diagnostics of dementia neuropsychology is applied in early detection, in differential diagnostics and in progressive course assessment. With standardized neuropsychological testing severity and nature of cognitive dysfunction is assessed. In particular memory disorders, aphasia, apraxia, agnosia and disorders of executive functions are assessed by these methods. In early stage of Alzheimer's Disease episodic and semantic memory as well as certain language functions are affected. Apractic and agnostic disorders often appear only in the later course of the disease. Depending on localization and nature of lesion cognitive dysfunction patterns tend to be more lateralized in vascular dementia (e.g. aphasic, apractic or visual-constructive disorders). In the various forms of fronto-temporal dementia executive functions and/or personality are mainly affected.

Avoidance of issues in family caregiving.

Effective communication between older people and their family carers is necessary for providing appropriate and quality care. However, family carers and carereceivers may avoid discussing issues of concern and this may adversely affect the quality of the caring relationship. This study investigated the content of, and avoidance of issues in communication between 84 spousal and filial carers and carereceivers. The study findings indicate that family carers and carereceivers do avoid discussing issues of concern. Nurses working with families are well placed to promote more effective communication in the caring context to augment more satisfying caring relationships for both carers and carereceivers.

The design of arm pressure covers to alleviate pain in high G maneuvers.

HYPOTHESIS: Arm pain has been noted especially when increased coverage anti-G suits are worn in combination with PBG (pressure breathing during G) in high G centrifuge test runs. Arm pain has forced the termination of test runs and has the potential to cause a serious decrement in a pilot's ability to function effectively during high-G flight. Based on the theory that arm pain is caused by fluid pooling in the arm, four new pressure cover systems for the arm were designed and tested. METHODS: Seven test subjects wore each design in a series of runs in a dynamic flight simulator (DFS). Subjects experienced both gradual and rapid onset rate runs with G levels ranging from +3.0 G to +9.0 G. Data were collected on the maximum G's achieved, pain location, and pain intensity. Subjective responses on the comfort and function of each design and overall arm cover design preferences were also obtained via a questionnaire. RESULTS: Analysis of the DFS data, using a two-factor ANOVA (subjects, designs), revealed that each of the new designs provided significantly better protection from arm pain than the control condition (no protection) and that each of the designs significantly improved the maximum G level attained during rapid onset acceleration in the range of +1 G. CONCLUSION: No positive correlation was found between subject ranking of the four new design concepts at the conclusion of the study in terms of the pain reduction they provided and the pain levels reported during DFS testing.

Characterization of novel peptoid agonists for the CCK-A receptor.

The successful design of peptoid CCK-B receptor antagonists using rational approaches suggested that it might be feasible to develop similar non-peptide small molecule agonists with potential therapeutic applications. We now report the characterization of such a compound with full agonist activity at CCK-A receptors on rat exocrine pancreatic acinar cells. The compound, PD149164, stimulated a similar maximal response to CCK8 from the exocrine pancreas in anaesthetized rats in vivo, and from isolated pancreatic acini in vitro it also generated intracellular Ca2+ oscillations similar to those evoked by CCK8. These effects were inhibited by the CCK-A antagonist L-364,718. Interestingly, the enantiomer of PD149164, PD151932, was a CCK-A antagonist and blocked PD149164 stimulated effects on the exocrine pancreas. The data indicate that it is possible to develop both agonist and antagonist activities in enantiomers of small non-peptide molecules.

Deciding to move home: a preliminary analysis of household migration behaviour in Scotland.

"This paper examines data from a mainland Scotland survey of household migration into owner-occupied properties identified from the Register of Sasines. Plotting of movements reveals migration patterns, but of arguably greater interest are the increasingly segmented and largely unexplored household decisions which drive migration. The purpose of the survey was to reveal something of these migration processes. Overview results presented here confirm that most moves are short distance, reasons for moving differ according to distance travelled, and although employment remains important for long-distance moves, its significance has declined in favour of quality of life considerations."

Stimulation of gastric somatostatin mRNA abundance by substance P in capsaicin-treated rats.

The spinal afferent neurons serving the stomach influence a variety of different gastric functions that together can be considered protective; it is not known whether the stomach can adapt to the loss of these neurons. We now report that in conscious rats pretreated with capsaicin to lesion small-diameter afferents, but not in control rats, i.v. infusion of substance P for 6 h increased the abundance of mRNA encoding somatostatin in antrum; there was no change in a reference mRNA, glyceraldehyde phosphate dehydrogenase. Substance P had no effect on somatostatin mRNA in the gastric corpus in either control or capsaicin-treated rats. An increased sensitivity of antral somatostatin cells to substance P may be one of the adaptive changes that occurs in the stomach of capsaicin-treated rats.

Placental isoferritin (PLF) in comparison with MCA and CEA in advanced breast cancer--first data from a pilot study.

The development of new and effective marker substances has optimized tumor-marker-guided follow-up programs to monitor generalization of disease and to assess the therapeutic outcome. Isoferritins of placental origin were first determined in the serum of patients with lymphoproliferative disease by way of the recently developed monoclonal antibody CMH-9. We have set up an Austro-Israeli working group and analysed 64 patients in terms of the sensitivity of placental ferritin (PLF) compared with the standard markers carcinoembryonic antigen (CEA) and mucinous-like cancer-associated antigen (MCA) in patients with metastatic breast cancer. We have additionally evaluated the importance of combined marker determination. Analysis of the data in view of site of metastatic spread yielded satisfying results both for PLF (sensitivity 70.4%) as well as MCA (sensitivity 76.9%) for visceral metastases; a combination of these two markers revealed a striking sensitivity of 88.4%, which, however, could not be improved by adding the third marker (CEA). With regard to non-visceral metastases, CEA and MCA were clearly superior.

Patient dose and associated risk due to radiological investigation of the internal auditory meatus.

The radiation doses and associated somatic risks due to four radiological examinations for acoustic neuromata (AN) have been investigated. These examinations were (1) plain film radiography of the internal auditory meatus (IAM), (2) computed tomography (CT) of the IAM, (3) CT of the posterior fossa and (4) CT of the IAM with air contrast. Organ dose measurements were performed using lithium fluoride thermoluminescent dosemeters loaded in a patient equivalent phantom. Dose equivalents to various organs are presented, together with the effective dose equivalent and collective effective dose equivalent for each examination. Hypothetical fatal somatic risks for each examination studied here have been estimated from the effective dose equivalents. The estimated number of hypothetical fatal cancers induced by radiological examinations for AN is between approximately 110 and 820 times lower than the number of detected AN, depending on the method of assessing the radiation dose to the remainder organs. It is concluded that in this particular study the radiological examinations are of net benefit to this group of patients.

ATP stimulates calcium influx in primary astrocyte cultures.

The effect of ATP and other purines on 45Ca uptake was studied in primary cultures of rat astrocytes. Treatment of the cells with ATP for 1 to 30 min brought about an increase in cellular 45Ca. Stimulation of calcium influx by ATP was investigated using a 90 sec exposure to 45Ca and over a concentration range of 0.1 nM to 3 mM; a biphasic dose-response curve was obtained with EC50 values of 0.3 nM and 9 uM, indicating the presence of low and high affinity purinergic binding sites. Similar levels of 45Ca influx at 90 sec were observed with ATP, ADP and adenosine (all at 100 uM). Prior treatment of the cultures with LaCl3 blocked the purine-induced 45Ca influx. These findings indicate that one pathway for calcium entry in astrocytes involves purinergic receptor-operated, calcium channels.

Monitoring radiation dose to the hands in nuclear medicine: location of dosemeters.

The relatively high radiation dose which can be received by the hands of staff in nuclear medicine departments means that in many departments it is necessary to monitor such doses. A convenient method is to use a TLD sachet in a plastic strip around a finger. This study was done to determine whether a dosemeter worn at the base of the middle finger was adequate to monitor the dose to the surface of the whole hand. Dosemeters were worn at the finger tips, finger base and palm of both hands, on two people while preparing and dispensing radio-pharmaceuticals, and two others while giving injections using syringe shields. The pattern of distribution of radiation does to the hands was similar for all workers and for both types of work. A single, convenient site (base of middle finger) may therefore be used for monitoring radiation dose to the hand.

[Diabur-Test 5000 - a new test strip for urinary sugar control in diabetic patients (author's transl)].

Diabur-test 5000, a new test strip for estimation of urinary glucose, was compared with the hexokinase glucose-6-phosphate dehydrogenase method in more than 2500 urine samples. By combination of two test ranges glucose concentrations of up to 5% can be detected by the strip test. After a reading time of 2 minutes, very precise estimation of urinary glucose is possible in eight steps from negative to 5%. False estimations of more than one color step virtually do not occur. Ketone bodies, salicylic acid and several antibiotics do not influence the test strip. Ascorbic acid shows a slight influence only in concentrations above 40 mg/dl. This influence disappears with glucose concentrations of more than 0.5%. Good correlation with the reference method, wide range of readings and simple handling make the test strip suited for the laboratory and particularly for self control of diabetic patients.

6 beta-Hydroxycortisol: a noninvasive indicator of enzyme induction.

We measured urinary 6 beta-hydroxycortisol (6 beta OHF) excretion in normal children and in children receiving anticonvulsant therapy with phenobarbital or diphenylhydantoin, 6 beta OHF excretion increased 4- to 7-fold during anticonvulsant therapy. The marked increase in the ratios of 5 beta OHF to 17-hydroxycorticosteroid and 6 beta OHF to free cortisol suggests that the measured increase in urinary 6 beta OHF may serve as an index of induction of the microsomal mixed function oxidase system. This noninvasive approach allows rapid assessment of the effects of drugs and foreign compounds on microsomal hydroxylation of cortisol. This convenient probe can be applied, with particular ease, to further studies in children in whom characterization of the effects of drugs and xenobiotic compounds on hepatic hydroxylation is of interest.
