Photocoagulation therapy of proliferative retinopathy in young onset type 1 (insulin-dependent) diabetes.

Fifty-one young onset Type 1 (insulin-dependent) diabetic patients with proliferative diabetic retinopathy before the age of 25 years were treated over a 3 year period by xenon photocoagulation. A few were treated additionally by argon laser, cryo- or diathermy-coagulation. The results of therapy with respect to both fundus disease and visual acuity were highly dependent on the fundus changes at the beginning of photocoagulation. Ater 3 years, 65% of eyes with peripheral neovascularization alone did not develop any new vessels. In contrast, only 20% with disc new vessels did not develop further neovascularization. Vitreous haemorrhage further reduced this percentage. The presence of blindness 3 years after treatment was rare in eyes with peripheral neovascularization alone (3%). However, 52% of eyes with disc neovascularization and vitreous haemorrhages were blind 3 years later.

[Proliferative diabetic retinopathy and pregnancy].

Twenty pregnant diabetics with proliferative diabetic retinopathy are described. Out of a total of 37 eyes, 18 were coagulated during pregnancy. Coagulation treatment before pregnancy was performed in 14 eyes. The morphologic aspect of the fundi at the end of pregnancy was unchanged or even better in 31 of the 37 eyes, as compared with the time before pregnancy. The best results were obtained in eyes which were sufficiently coagulated before pregnancy. Visual acuity at the end of pregnancy in 35 eyes was much the same as during pregnancy. It is concluded that from a ophthalmological point of view the inindication for therapeutic abortion in diabetics with dication for therapeutic abortion in diabetics with proliferative diabetic retinopathy is no longer tenable.

[Coagulation therapy of proliferative diabetic retinopathy in juveniles (author's transl)].

51 selected juvenile diabetics (diabetes before the age of 12 and proliferative retinopathy before the age of 25) were treated over a 3-years-period by xenon-photocoagulation, in a minor percentage additionally also by argonlaser-, cryo- or diathermy-coagulation. Results of therapy regarding both fundus disease and visual acuity are highly dependent from the morphologic aspect of the fundus at the beginning of coagulation. Eyes with peripheral neovascularizations alone have no more neovascularizations in 65% after 3 years, this is true for eyes with papillary neovascularizations in only 20%. Vitreous hemorrhage further reduces this percentage. Blindness after 3 years is rare in eyes with peripheral neovascularizations alone (3%). On the other side 52% of eyes with papillary neovascularizations and vitreous hemorrhages are blind after 3 years.

[Measurement of subretinal fluid in retinal detachment (author's transl)].

In surgery for retinal detachment, such as shortening of the eyeball, volume reducing, and indentating operations, knowledge of the volume of the subretinal fluid is important. A method based on ultrasonography was developed to measure the volume of subretinal fluid preoperatively. After establishing a topography of the fundus, the height of retinal detachment can be measured by ultrasonography at 49 defined points. The diameter of the eyeball is measured at 12 different points. From the height of retinal detachment and the diameter of the eyeball it is possible to calculate the subretinal space, which is divided into 60 segments. The total volume of subretinal fluid is calculated by adding the volumes of these 60 segments. The calculus is programmed on a computer.

[Carcinoembryonic antigen in retinoblastomas (author's transl)].

In 47 patients (46 children and 1 adult) with unilateral and bilateral retinoblastoma, the titer of the carcinoembryonic antigen (CEA) was determined. The CEA titer of children with active retinoblastomas was 1.44 +/- 0.26 ng/ml (x +/- SEM). Those patients whose retinoblastoma was inactivated by therapy did not show a significantly lower CEA titer. In the group of children with one eye removed because of a unilateral retinoblastoma, the CEA titer was significantly (P less than 0.1) lower. The globes of 25 children were examined histologically. In those cases with invasion of the optic nerve or choroid, the CEA titer was significantly higher (P less than 0.1) as compared with those where the tumor was limited to the retina alone.

[Experience with the new photocoagulator (Zeiss) (author's transl)].

The new Zeiss Xenon-photocoagulator has considerable advantages. The possibility of converging the light beam allows coagulating even prominent areas (folds) in lensless eyes without difficulty. From our experience Xenon coagulation with individually adapted coagulation offers advantages over argon-laser coagulation with relatively short coagulation times.

Retinal enzyme activities under hypoxic conditions prior to neovascularization.

In an effort to demonstrate biochemical changes occuring in malperfused retinas, oxygen-induced retrolental fibroplasia was produced in kittens. Immediately before the onset of vasoproliferations, that is in the stage of maximal hypoxia, the activities of lactate dehydrogenase (LDH), glucose-6-phosphate dehydrogenase (G-6-PDH), malate dehydrogenase (MDH) and isocitrate dehydrogenase (ICDH) were determined. The activity of MDH was found to be significantly decreased, whereas the activities of the other enzymes showed no major changes.

The role of lactic acid in retinal neovascularization.

In order to gain information on the possible role of lactate in the development of neovascularization following retinal malperfusion, the lactate content in vitreous-retina-choroid specimens of 02-treated kittens and streptozotocin-diabetic rats was determined. In both experiments there was no significant difference of lactate content between the diseased eyes and healthy controls.

[Sclerodermie en coup de sabre with spontaneous filtering bleb (author's transl)].

In a 43-year-old female patient with sclerodermie en coup de sabre on the right forehead, the following changes of the right eye were observed: Partial lack of hairs of the eyebrow; distortion of the upper eyelid, similar to a coloboma; paralysis of the inferior oblique and the superior rectus muscles; decrease of corneal sensibility; formation of a spontaneous filtering bleb in the upper limbal region; ocular hypotension with changes of the papilla.
