Trends in emergency psychiatry in the last two decades.

Emergency psychiatric services provide essential care. Their utilization may also be assumed to reflect the health care needs of a population. The authors observed trends in an emergency psychiatric service over a 17-year period by comparing 400 randomly selected patient visits in 1977 with date from 1960 and 1970. They found a decrease in the number of patients seeking treatment, decreased patient age and socioeconomic status, increased use by men, increased night visits, little change in diagnostic categories, and a striking increase in hospitalization since 1970. These data support the importance of assessing trends in emergency psychiatric services over time to ensure that they meet the needs of the population being served.

Platelet MAO and amitriptyline treatment.

Some tricyclic antidepressants have been reported to inhibit monoamine oxidase (MAO) activity in vitro in addition to blocking the reuptake of norepinephrine and/or serotonin. While the inhibition of MAO is reversible, platelet MAO activity in depressed patients responding to amitriptyline treatment has been reported to be reduced after drug treatment. In a reverse design, we measured platelet MAO activity and drug levels in patients chronically being treated with amitriptyline and again 2 and 4 weeks after stopping the medication. Although tricyclic drug concentrations were initially within the therapeutic range and undetectable on placebo treatment, platelet MAO activities were unchanged.

Choline acetyltransferase: regional distribution in the abdominal ganglion of Aplysia.

Using a new microassay, we have determined the properties and the regional distribution of choline acetyltransferase in the abdominal ganglion of Aplysia. Enzyme concentrations in homogenates of groups of cells and in single identified cells indicate that neurons which function as neurosecretory cells, and which do not form chemical synapses with other cells or with peripheral structures, have little or no ability to synthesize acetylcholine; neurons which are involved in visceromotor integrations, and which connect with each other or with the periphery, have a substantial concentration of the enzyme.

Symptoms of interpersonal sensitivity in depression.

Six self-rated items of interpersonal sensitivity (IPS) were examined in 174 depressed outpatients. These items were "feeling critical of others," "your feelings being easily hurt," "feeling others do not understand you or are unsympathetic," "feeling others are unfriendly," "feeling inferior to others," "feeling shy or uneasy with the opposite sex." The population was grouped into tertiles based on their pretreatment IPS score. High levels of IPS were associated with earlier onset and greater chronicity of depression, higher Hamilton Rating Scale for Depression (HRSD) score, more severe depressed mood, guilt, suicidality, impaired work and interest, retardation, depersonalization, paranoia, and cognitive symptoms of depression. More frequent atypical features were found, e.g., overeating/weight gain, self-pity, phobic avoidance, and panic attacks. Response to a monoamine oxidase (MAO) inhibitor drug increased at higher levels of IPS, while the response to a placebo decreased.

MAOI treatment response: multiaxial assessment.

While studying the effectiveness of the MAOI isocarboxazid for the treatment of depression, we noted that many patients experienced a reduction of symptoms without equivalent improvement in other areas of their lives. We evaluated four outcome areas: symptoms, work, family functioning and social functioning. After 6 weeks on medication, symptoms improved the most, significantly more so than the other three areas. For the group of patients who completed 24 weeks on medication, all four outcome areas were further improved compared to the 6-week levels, with the improvement in work functioning reaching statistical significance. We conclude that the assessment of treatment outcome is more complex than the simple measurement of symptom reduction, and that different outcome areas are likely to improve at different rates and to different extents.
