[Occupational rehabilitation of patients with mental or psychosomatic disorders. Study of 389 cases].

The study of all the patients sent to the PPU by the disablement insurance in 1975 and 1976 (389 cases). The comparison of the propositions made in medical reports and the actual allowances provided by the disablement insurance reveals the following data: in 326 cases (84%) out of 389, medical doctors only conclude to the presence or absence of a certain rate of disability (from 0 to 100%), to the exclusion of any rehabilitational measure. In 63 cases (16%) measures were proposed such as a professional rehabilitation or a placing in a specialized institution for insertion to a new profession. For this second group, only in 27 cases was the disablement insurance able to follow the medical propositions. Only in 3 cases the measures taken can be considered fully successful, and in 1 case partially successful. All the other measures taken failed. A first examination of the group of 326 cases (propositions only for disablement) which concerned 297 cases also shows a certain number of discordances between the medical propositions and the allowances provided by the disablement insurance. The majority of the discordances has to do with an undervaluation of the rate of disablement by the medical experts. Moreover, the examination of the diagnosis shows that readaptation measures are more easily proposed to patients with psychosomatic complaints, to young men, and allowances rather to women and to patients with obvious psychiatric disorders. The authors discuss some assumptions regarding these observations.

Influence of the setting on a psychotherapeutic relationship in analytically oriented brief psychotherapy.

In this brief review, the author stresses the interest of short-term psychotherapy in the study of psychotherapeutic processes: through modifications of time and space factors of the cure (limited duration - face-to-face) one can appreciate better the influence of the setting on the nature of the process. By comparing the psychotherapeutic situation (face-to-face) to the psychoanalytic situation (back-to-face), the author highlights some specific features of psychotherapies, in particular the influence of the infraverbal language and its consequences on the cure. In a face-to-face situation for example, the subject unconsciously tries to incorporate the therapist in his ego, and this very powerful merging mechanism may hinder the evolutive process of the treatment. Some of the aspects examined apply to any doctor-patient-relationship.

Psychoanalysis and brief psychotherapy: some new considerations on the psychotherapeutic process.

This introductory paper describes the technique of brief analytic psychotherapy as has been developed in Lausanne. The author has tried to demonstrate that a single modification of the setting in comparison with psychoanalysis stresses the importance of the 'hic et nunc' in the therapeutic relationship. If this phenomenon becomes apparent to the therapist and is adequately interpreted, a natural associative process can develop where the analysis of transference and countertransference takes a primordial place. This process is comparable to psychoanalysis as far as certain transference movements are concerned, but the working mechanisms are different.

Introductory paper describing succinctly the technique of brief analytical psychotherapy as developed by one of the authors in Lausanne. The author tries to demonstrate that a single modification of the setting in comparison with psychoanalysis bends the therapeutic relationship towards a stressing of the importance of the "hic et nunc". Evaluation of psychotherapies and Osgood's Semantic Differential. A tentative approach.

Setting up a semantic differential according to Osgood for evaluating analytical psychotherapies: a selection of scales and inducive words plus a factor analysis based on answers obtained from two test groups--the one composed of ambulatory psychiatric patients, the other composed of normal individuals. Preliminary validity tests of the results for applicability in clinical situations: examples of individual semantic configurations reflecting in a plausible manner certain clinical characteristics; comparison of the configurations of a group of neurotics with those of a group of schizophrenics; example of the evolution of one configuration during the period of psychoanalytical treatment. These first tests led us to believe that the semantic differential could contribute effectively to the evaluation of psychotherapies.

[Comparative study of 2 groups of paranoid syndromes appearing at different ages].

Clinical study of two groups of females beyond age of 65, institutionalized for delusional manifestations of schizophrenic nature and presenting also, at the time of examination, a pronounced paranoid state. In the first group: the manifestations had arisen before the age of 45. In the second group, after the age of 65. This study has demonstrated certain psychopathological characteristics suggesting the presence of personality problems definitely more profound in patients of the first group: autistic state, asthenia, thought disorder, incoherence and vagueness of delusional subjects, ordinarily much more unreal are characteristics of the first group in comparison to the second. This seems to bring evidence that these two paranoid states (paranoid schizophrenia in adult age and paranoid state in senility) are, at first sight, pathological entities based on personality problems of very different intensity.
