[Documentation of surgical performance--does more really help more? Comparison of the effects of maximum and limited documentation depth of clinical patient data on theoretical revenue volume of a surgical clinic after introduction of the DRG-based reimbursement system].

INTRODUCTION: The forthcoming introduction of a DRG-based account system in Germany aims at higher transparency and economic efficiency, particularly in the sector of in-patient health care. The availability of documentation of the highest quality, taking into account all potentially relevant diagnoses, appears to be the best method for achieving maximum revenue in individual surgical units. The aim of the study was to determine the relevance of various degrees of documentation depth on calculated DRG-based revenue. Furthermore, we evaluated whether improvements in the quality of documentation can be realized in current hospital organization. METHODS: In a prospective study, clinical data from 402 in-patients were collected and revenues were calculated based on the Australian-Refined DRG system. Various qualities of documentation were defined. In order to find the medical sectors most sensitive to "under-documentation", homogenous cases were classified into 23 treating groups, according to diagnosis. RESULTS: In 267 cases, maximum revenue was determined only by one main diagnosis, while better results could be achieved in 137 cases (34%) by extended documentation quality. Half of this gain could only be achieved by an independent medical documentation specialist. An upper limit of documentation intensity (number of diagnoses) could be defined. Maximum gain did not require maximum number of diagnoses. CONCLUSIONS: Documentation depth has an important influence on the calculated revenue of surgical therapy based on AR-DRG system. The quality and depth of the documentation is not, in itself, sufficient. In order to be really effective, it requires the highest degree of professionalism from hospital staff.

The LBI-method for automated indexing of diagnoses by using SNOMED. Part 2. Evaluation.

We present a simple, formal, lexicon-based method for automated indexing of diagnoses based on the Systematized Nomenclature of Medicine (SNOMED), called LBI-method. Part 1 gave an introduction to the LBI-method and presented its realisation as application system SALBIDH. Part 2 presents the design and the results of an evaluation study to judge the quality of the LBI-method. In this evaluation study the quality of automated indexing as well as the quality of the retrieval of patient data by using automated indexed diagnoses was examined. The results show that the retrieval based on SNOMED indices is at least as good as the retrieval based on ICD classes despite a lot of indexing errors. From this we gather that our system is not yet good enough for immediate routine use but that an appropriate indexing quality and, as a result, a higher retrieval quality can be achieved after few improvements of the LBI-method, especially after revision of the lexicons.

A simple on-line information system for quality assessment.

Many aspects of medicine would be well served by a simple method to assess the outcome of care in specified groups of patients. This study examined charts of patients with stomach cancer on a surgical service. Two digits were added to the ICD-9 number in the routine discharge data; one for the nature and severity of case and the other for the outcome of care. The digits were designed for on-line registration at discharge. Information was also obtained on resource consumption in the various groups of patients. Most of the variables had to be evaluated implicitly as there were no explicit judgement criteria and few empiric data available for comparison. Implicit evaluation of the results was significant and prompted steps for improving care. With current systems, the information obtained from traditional hospital statistics is limited and partly misleading. By slight modification, however, hospital statistics may provide valuable information for assessing quality of care and resource allocation during hospitalization.

Investigation of the significance of heavy metal contents of blackbird feathers.

The lead, cadmium and zinc in untreated blackbird (Turdus merula L.) feathers is predominantly of exogenous origin. The endogenous concentration is of minor importance. The degree of surface metal pollution depends on exposure time. The exogenous fraction of heavy metals cannot be completely removed by washing procedures. The difference between washed and unwashed feathers is demonstrated by SEM micrographs.

Automated serial ECG-analysis within an epidemiological study.

Since 1980 we have performed a cohort study in order to develop diagnostic and predictive algorithms for myocardial infarction. These algorithms shall be based on cardiovascular risk factors, ECG-measurements and serial measurement changes. An intermediate analysis of 5524 study participants describes observed measurement changes stratified by age and sex. The values thus obtained may serve for reference purposes and illustrate the influence of age and sex on intraindividual ECG-measurement changes. Specific suggestions are made for future program developments in order to overcome some present problems in serial ECG-analysis.

Prospective study of suspected associations between certain drugs administered during early pregnancy and congenital malformations.

From 1964 to 1976, a cohort study was performed in West Germany to study the possible influence of various factors on pregnancy and child development. Results of the evaluation of 13,643 pregnancies are given with respect to possible teratogenetic effects of antiemetic drugs and sex hormones administered in early pregnancy. There was no evidence of an increased risk of major malformations following the intake of certain antiemetic drugs and progesterone. Also, the use of a hormonal pregnancy test was not significantly associated with an increase of major malformations.

[Value of vascular morphology for the selection of catheters in selective celiacography and mesentericography].

By means of 192 lateral aortograms the vascular morphology of the coeliac and superior mesenteric artery were correlated to age, sex and body weight. Independently of these parameters 90% of the exit angles of the coeliac artery were situated between 15 degrees and 90 degrees with a median of 45 degrees. In persons with less than 65 kg body weight the median origin angle of the superior mesenteric artery corresponded to 45 degrees, too, whereas in heavier patients they were bigger (60 degrees). The vascular diameter and the further course of the superior mesenteric artery significantly depended on the constitutional type. In 40.5% we saw a cranial eccentric stenosis of the coeliac artery. Frequency and intensity were not influenced by age, sex or body weight, but accompanied by smaller origin angles of the coeliac artery and bigger ones of the superior mesenteric artery. In conclusion different catheter shapes in dependence of the constitutional type are recommended for combined coeliac and mesenteric angiography.

Energy cost of membrane depolarization in hog carotid artery.

Past studies have shown that during stable stepwise activations of vascular smooth muscle with varying concentrations of high-K+, both cytoplasmic free-Ca++ and membrane depolarization vary. On the other hand, during stepwise activations with varying concentrations of external Ca++ in the presence of constant external Ca++ in the presence of constant external depolarizing high-K+, cytoplasmic free-Ca++ varies, while membrane depolarization remains relatively constant. In this study, the rates at which suprabasal energy metabolism (oxygen consumption and lactic acid production) increased with increasing isometric tension maintenance were measured under both circumstances. Suprabasal energy metabolism with increasing membrane depolarization (increasing external K+) exceeded that with constant depolarizing-[K+] and varying Ca++ by less than 2.5% at all levels of activation, which was not statistically significant (P greater than 0.70). We conclude therefore that the steady state metabolic energy cost of membrane depolarization per se during contractile activity in vascular smooth muscle from a tonic conducting vessel (hog carotid artery) is negligible. Although the possibility cannot be excluded, we find no metabolic evidence that increased cytoplasmic free-Ca++ itself activates an ATPase associated with Ca++ sequestration and/or extrusion beyond that present in the relaxed state. Activation of hog carotid artery with an isosmotic K+-for-Na+-substituted medium fails to stimulate aerobic glycolysis at all levels of K+ substitution. Experiments were performed at the muscle length optimal for isometric tension generation and at 37 degrees C.

A computed tomographic prolective trohoc study of chronic schizophrenics.

The maximal width of the third ventricle, the maximal distance between the outer tips of the anterior horns, and the number of enlarged cerebral sulci on the two highest CT slices were measured in 68 chronic schizophrenic patients on cranial computed tomograms in order to detect a possible enlargement of the cerebrospinal fluid (CSF) filled intracranial spaces. These results were compared with values obtained from a control group which was formed in accordance with definite exclusion criteria and matched-pair parameters (sex, age and maximal inner diameter of the skull). In a prolective trohoc study no difference was found in the size of the CSF spaces of schizophrenics and the controls. The psychopathological condition of the patients, which was classified in a semistandardized dialogue, also showed no correlation with the ventricular size or the number of enlarged cerebral sulci.

[Computed tomography of cranio-facial injuries (author's transl)].

For the detection of the finest changes in bone structure--like arrosions, porotic alterations, small fracture and fissure lines--computed tomography (CT) is yet surpassed by X-ray tomography: The current resolving power of CT is smaller, our Delta Scanner's layer thickness of 13mm is too large. Nevertheless, the advantages of CT in fronto-basal injuries augment essentially the diagnostic possibilities: with few layers and without great problems in setting projections, most of the possible injuries may be easily surveyed. The traumatized patient ist only minimally discomforted by the whole-body scanner. The lens dose is far less than the values of X-ray tomography reaches. Dislocated fracture fragments, brain traumatisation with and without hemorrhage, defects of the cribriform plate with concimitant brain prolaps, trauma of intraorbital structures, injuries to the nasal cavity and paranasal injuries may easily be detected. These findings influence importantly the surgical intervention and enable one to set up a planning of time and intercollegiate treatment. The postoperative care ist improved and inflammatory complications may be pointed out. In diagnosis of frontobasal injuries we therefore recommand: 1. X-ray survey and eventually X-ray radiograms of the nasal cavity and paranasal sinuses. 2. Computed-tomography (CT). 3. For fine detailed study of bone structures like fissures and smalles fractures X-ray tomography will still remain indispensable.

The aerobic metabolism of porcine carotid artery and its relationship to isometric force. Energy cost of isometric contraction.

1. The O2 consumption and aerobic lactate production of porcine carotid artery have been studied in relation to active isometric tension maintenance in order to estimate the tension cost, i.e., ATP hydrolysis rate per unit force maintained. 2. The relationship between total metabolism and isometric force was measured at several muscle lengths in order to distinguish tension-dependent and -independent components of ATP utilization. 3. At the maximum isometric tension observed, 2 kg wt/wt/cm2, 1.3 mumole ATP/M in per gram wet weight out of a total metabolism of 2 mumole/min is tension de pardent. 4. The absolute levels of force, O2 consumption, lactate production and the ratio of oxidative to glycolytic metabolism varied considerably with the stimulant; however, the tension-dependent ATP utilization was invariant with respect to the mode of stimulation.

Cross bridge ATP utilization in arterial smooth muscle.

Measurements of oxygen consumption and lactate production were used to estimate the ATP production in porcine carotid arteries. Changes in the total energy metabolism were correlated with changes in force which was altered by changing the muscle length under maximum stimulation. This tension-dependent metabolism could be used to distinguish between cross bridge and other ATP utilization. The tension-dependent metabolism was equal under K+ -depolarization or histamine stimulation. Tension cost was 300x smaller than skeletal muscle while the cross bridge cycle duration 30x longer.
