Renal injury: similarities and differences in male and female rats with the metabolic syndrome.

The metabolic syndrome is complicated by nephropathy in humans and rats, and males are more affected than females. We hypothesized that female rats had reduced expression of glomerular oxidized low-density lipoprotein (oxLDL) receptor 1 (LOX-1), attendant glomerular oxidant injury, and renal inflammation. Three groups, obese males (OM), obese females (OF), and lean males (LM) of first-generation (F(1)) hybrid rats derived from the Zucker fatty diabetic (ZDF) strain and the spontaneous hypertensive heart failure rat (SHHF/Gmi-fa) were studied from 6 to 41 weeks of age. OM had severe renal oxidant injury and renal failure. Their glomeruli expressed the LOX-1, and exhibited heavier accumulation of the lipid peroxide 4-hydroxynonenal (4-HNE). OM had compromised mitochondrial enzyme function, more renal fibrosis, and vascular leakage. Younger LM, OM, and OF ZS (ZDF/SHHF F(1) hybrid rat) rats, studied from 6 to 16 weeks of age, showed that unutilized renal lipids were comparable in OM and OF, although young OM had worse nephropathy and inflammation. In conclusion, glomerular LOX-1 expression is coupled to deposits of 4-HNE and glomerulosclerosis in OM. We presume that LOX-1 enhances glomerular uptake of oxidized lipids and renal inflammation, causing greater oxidant stress and severe glomerulosclerosis. In OF, renal protection from lipid oxidants appears to be conferred by blunted glomerular LOX-1 expression and renal inflammation.

Systemic and topical corticosteroid treatment of oral lichen planus: a comparative study with long-term follow-up.

BACKGROUND: Topical corticosteroids are the mainstay treatment for oral lichen planus (OLP), but some authors suggest that systemic corticosteroid therapy is the only way to control acute presentation of OLP. METHODS: Forty-nine patients with histologically proven atrophic-erosive OLP were divided into two groups matched for age and sex. The test group (26 patients) was treated systemically with prednisone (50 mg/day), and afterwards with clobetasol ointment in an adhesive medium plus antimicotics, whereas the control group (23 patients) was only treated topically with clobetasol plus antimycotics. RESULTS: Complete remission of signs was obtained in 68.2% of the test group and 69.6% of the control group, respectively (P = 0.94). Similar results were obtained for symptoms. Follow-up showed no significant differences between the two groups. One-third of the patients of the test group versus none in the control group experienced systemic side-effects (P = 0.003). CONCLUSIONS: The most suitable corticosteroid therapy in the management of OLP is the topical therapy, which is easier and more cost-effective than the systemic therapy followed by topical therapy.

Prior geographic mobility and job search length.

The effect of previous geographic mobility on the length of an unemployed person's job search in the United States is examined. "This study proposed that knowledge of regional wage differentials and other market conditions give a worker with prior geographic mobility experience a better stock of information than one who has worked all of his or her work life in the same geographical location. It is proposed that this superior stock of information has a significant impact on the amount of time it takes an unemployed worker to locate and accept a job." Data are from the 1981 Panel Study of Income Dynamics.

Sampling of amniotic fluid for L/S ratio determination after rupture of membranes.

After rupture of membranes (RM), amniotic fluid (AF) leaking from the external os can be collected by means of a sterile bladder catheter inserted into the vagina and connected to a plastic bag for determination of the L/S ratio. This study was designed to ascertain whether contamination of AF with cervical or vaginal mucus alters the L/S ratio determined by Gluck's method. 50 gravidas in term or preterm labor had their amniotic sac punctured by a long gauge-20 needle under amnioscopic sight. Two samples of AF were collected from the needle. After RM, a sterile plastic bladder catheter was placed in the vagina and connected to a plastic bag ordinarily used for urine collection. Two additional samples of AF were taken from the plastic bag. All 4 samples were marked with code numbers and submitted for L/S ratio determination. There were no significant differences in the L/S ratio between samples obtained by the two different techniques. It is concluded that sampling of AF by means of a vaginal catheter and a plastic bag does not alter the L/S ratio, and is therefore practicable for assessing fetal lung maturity after RM.
