Streptococcal pharyngitis in Italian children: epidemiology and treatment with miocamycin.

A prospective multicentre study of Italian children with acute pharyngotonsillitis was carried out to determine the incidence of beta-haemolytic streptococcal infection and its epidemiological characteristics and to evaluate the effectiveness of a recently marketed macrolide, miocamycin. From February 1988 to March 1989, 865 children (aged 5 months to 14 years) were enrolled in 11 paediatric departments. Of these 31.9% had a positive throat culture for beta-haemolytic streptococci (26.6% group A streptococci, 5.3% group B, C, D or F). In 68.1% of the patients the throat culture did not yield any pathogen and the disease was considered to be probably viral. No epidemiological (age, season of occurrence, sex) or clinical characteristics (fever, pharyngeal hyperaemia, exudate etc.) could differentiate children with streptococcal pharyngotonsillitis from those with a probably viral one. Breese's score correctly identified only 19.1% of the cases with group A streptococcal infection. Miocamycin was given 50 mg/kg/day in 2 divided doses for 10 days in 225 children with streptococcal pharyngitis. The drug exhibited good clinical efficacy (resolution or improvement of acute signs and symptoms) in 96.3% of the children and a microbiological efficacy (eradication of the causative bacteria) in 85.3%.

Single photon emission computed tomography with 99mTc HM-PAO in the study of focal epilepsy.

We used 99mTc HM-PAO SPECT to study 50 patients with partial epilepsy: 47 interictally and 3 during a seizure. All 3 patients studied during a seizure presented an area of increased tracer uptake. Of those in whom recordings were taken during seizure-free intervals 35 (74%) showed perfusion asymmetries: 27 (57%) with decreased and 8 (17%) with increased uptake. We discuss the findings and compare them with those of similar studies using SPECT and positron emission tomography.

[295 cases of brucellosis treated with minocycline].

295 cases of acute and subacute cases of brucellosis were treated with minocycline. It is concluded that because of its in vitro activity, its penetrability in macrophages and granulomas and its tolerance, minocycline is the antibiotic of choice for brucellosis.

[Bronchopneumopathies during a course of brucellar infection].

The clinical polymorphism of brucellosis is well known fact. It is thought personally that respiratory system localizations are more frequent than have so far been reported in the literature and serious differential diagnosis problems arise with tuberculosis, neoplasia and pneumoconiosis. After discussing such problems, some cases of pleurobronchopneumopathies during brucellosis which were cured are reported.

[Preliminary study on immunoglobulins G in the cerebrospinal fluid of neurological patients].

The preliminary data from a study of the cerebrospinal fluid IgG of 76 neurological patients are reported. The radial immunodiffusion method was employed. The absolute and percentage increase of IgG with respect to total proteins in the various diagnostic classes involved in the series was considered. The most significant increases were encountered in M.S., in neuropathies and radiculopathies and in tumor involving the subarachnoid spaces. The high percentage is, however, characteristic of M.S. The method is thus particularly useful in diagnosing M.S. and in differentiating it from other neurological diseases.
