A comparison of the prognostic value of antibody-dependent lymphocyte cytotoxicity and other EBV antibody assays in Chinese patients with nasopharyngeal carcinoma.

Antibody titres to EBV-associated antigens in Chinese NPC patients were analysed according to length of survival after diagnosis and to disease stage. Geometric mean titres of ADLC antibody were highest in the long-term survivors, whereas VCA, EA and EBNA antibody titres showed an inverse relationship to survival. High VCA, EA and EBNA titres were less frequent, and high ADLC titres were more frequent in long-term survivors than in intermediate or short-term survivors. The association of geometric mean titres of EBV antibodies with prognosis could not be entirely explained by stage of disease. A functional role for the ADLC antibody is suggested by the association of a high ADLC antibody titre with a good prognosis regardless of stage of disease.

General immunological status of nasopharyngeal carcinoma patients in Singapore.

Newly diagnosed NPC patients were found to have impaired general T-cell functions, as determined in vivo by the Mantoux test and in vitro by the PHA response assay. Treated remission patients were as hyporesponsiveness as newly diagnosed patients. PHA hyporesponsiveness was associated with the HLA profile of A2-B Sin 2 and with high antibody titres to EBV-related antigens, in particular to the early antigen. Impaired responses in the Mantoux or PHA tests were associated with poor survival. The impaired response could be partially or totally restored by in vitro treatment of the hyporesponsive lymphocytes with levamisole.

Etiological factors in nasopharyngeal carcinoma: a hospital-based, retrospective, case-control, questionnaire study.

A total of 379 Singapore Chinese patients with NPC were interviewed by use of a questionnaire covering the following items: occupation, level of education, language medium of education, personal and family history of nasal illnesses, types of medicines used, use of Chinese medicines for the nose and throat, use of soya sauce, Chinese tea, cooling drinks and alcohol, cigarette smoking (number and duration), cooking fuels and use of incense (frequency and duration) and of anti-mosquito coils. The same questionnaire was given to two groups of controls: 595 patients with diseases of the ear, nose and throat other than NPC and 1 044 patients with diseases other than cancer or otorhinolaryngeal disease. NPC patients differed significantly from both groups of controls in that they showed stronger associations with personal history of nasal illnesses, family history of nasal illnesses, use of Chinese medicines for the nose and throat and exposure to smoke from anti-mosquito coils.

Nasopharyngeal carcinoma and histocompatibility antigens.

New data are presented concerning the relationship between NPC and HLA antigens among Chinese. When attention is confined to newly diagnosed cases, it can be shown that, apart from the increased risk associated with the joint occurrence of A2 and B-Sin 2, there is also an increased risk associated with BW17 and a decrease in risk associated with A11. Among long-term survivors, however, BW17 is appreciably decreased, whereas A2 in the absence of B-Sin 2 or BW17 is increased. Among Malays, a non-Chinese group, there is an excess among NPC patients of a locus A blank, a blank which is probably associated with the AW19 complex.

Hepatic secretion and turnover of serum phosphatidylcholine-cholesterol acyltransferase in male and female rats.

The activity of serum phosphatidylcholine-cholesterol acyltransferase (LCAT), output of the enzyme by the perfused rat liver, and the effect of pretreatment with colchicine on LCAT were studied in male and female rats. It was observed that: 1. Serum LCAT activity in the female exceeded that of the male in fasted animals, whereas in fed animals, LCAT activity was higher in the male than the female. With both sexes, however, serum LCAT activity in fed animals was greater than that in fasted animals. Data are presented which suggest that the observed sex differences were due to concentration and/or composition of the substrate rather than to differences in the serum concentration of the enzyme. 2. The release of LCAT by perfused livers from fasted female rats exceeded that of the male animals. The output of LCAT was inhibited by pretreatment (male) with colchicine, which suggests that hepatic secretion of LCAT is dependent on vesicular transport. 3. The decay of serum LCAT activity in vivo following injection of colchicine was more rapid in fasted female rats than in male animals. These observations lead us to postulate that the turnover rate of LCAT is higher in female rats than in male animals.

Effects of free fatty acids on activity of hepatic microsomal 3-hydroxy-3-methylglutaryl coenzyme A reductase and on secretion of triglyceride and cholesterol by liver.

The output of triglyceride and cholesterol, and the activity of microsomal 3-hydroxy-3-methylglutaryl coenzyme A reductase (HMG-CoA reductase) were measured following perfusion of the isolated rat liver with a medium containing free fatty acids. The activity of the enzyme and the output of cholesterol and triglyceride are regulated by the quantity and structure of the free fatty acid. The activity of HMG-CoA reductase was linearly proportional to uptake of oleic acid (18:1) by the liver. Output of triglyceride and cholesterol in the presence of oleate was also stimulated although, under these experimental conditions, the maximal secretory rate for cholesterol was observed while reductase activity was still increasing. When equimolar quantities of palmitic (16:0), oleic (18:1), or linoleic acid (18:2) were infused, the observed activity of the enzyme was in the order 18:1 greater than 18:2 = 16:0. Output of triglyceride was in the order 18:1 = 18:2 greater than 16:0. Output of free and esterified cholesterol was in the order 18:1 greater than 18:2 greater than 16:0. The stimulation by free fatty acids of hepatic cholesterogenesis, as estimated by the activity of HMG-CoA reductase, probably is the result of stimulation by free fatty acids of secretion of triglyceride in the very low density lipoprotein, and the obligatory requirement for cholesterol as a component of the lipoprotein.

Impaired general cell-mediated immune functions in vivo and in vitro in patients with nasopharyngeal carcinoma.

General cell-mediated immune (CMI) functions in NPC patients were investigated by the in vivo Mantoux and in vitro lymphocyte response to PHA assays. Thirty-eight (50%) of 76 untreated NPC patients were hyporesponsive in the Mantoux assay compared to 27 (25%) of 110 control patients. Forty-three (65.2%) of 66 untreated NPC patients also showed lymphocyte hyporesponsiveness to PHA compared to 15 (15.5%) of 97 control patients. Combined deficiencies were observed in 35 (42.2%) of 83 NPC patients compared to only 2 (3.3%) of 61 control patients. No difference in the frequency of immunodeficiency was observed between "early" and "late" disease patients. CMI functions of treated "remission" NPC patients were found to be impaired to the same extent as those of untreated NPC patients.

Immunogenetic aspects of nasopharyngeal carcinoma. IV. Increased risk in Chinese of nasopharyngeal carcinoma associated with a Chinese-related HLA profile (A2, Singapore 2).

The results of this study of 110 Singapore Chinese with nasopharyngeal carcinoma (NPC) and 91 controls confirmed the association between the occurrence of HLA antigen Singapore 2 (Sin2) and NPC in the Chinese population, and indicated that their increased risk for NPC was confined to the joint occurrence of Sin 2 and A2 antigens. These findings suggested that the genotype of importance in susceptibility to NPC is the A2-Sin 2 haplotype.
