Results of a double-blind placebo controlled trial of a new serotonin uptake inhibitor, sertraline, in the treatment of obsessive-compulsive disorder.

Eighty-seven patients with a DSM-III diagnosis of obsessive-compulsive disorder (OCD) without depression were entered into a double-blind, placebo-controlled study of the efficacy of sertraline, a new serotonin uptake inhibitor. After a 1-week washout period, patients were randomly assigned to receive either placebo or sertraline. After a 2-week titration period in which the once-daily sertraline dose was increased from 50 mg/day to a maximum of 200 mg/day, dosage was maintained until the end of the eighth week, then patients were titrated off medication over the next 2 weeks. Efficacy was measured by the Yale-Brown Obsessive-Compulsive Scale (Y-BOCS), NIMH General Obsessive-Compulsive Scale, Maudsley Obsessive Compulsive (MOC) Inventory, and Clinical Global Impressions (CGI) Severity and Improvement scales. Results on the MOC Inventory showed trends in favor of active drug that were not statistically significant compared with placebo. Results of the Y-BOCS total score, the NIMH score, and the global severity and improvement scores demonstrated a statistically significant superiority of sertraline compared with placebo.

Autosomal recessive spastic paraplegia: evidence for demyelination.

Neurological evaluation of a family living in rural Louisiana showed that six living members have splasticity, hyperactive stretch reflexes, and pathological reflexes. Ten deceased family members had been similarly affected. All affected persons were offspring of consanguineous marriages, and sex distribution was equal, indicating that inheritance was autosomal recessive. Although undetected clinically, conduction abnormalities suggestive of demyelination in the visual pathways were revealed by special tests.

Treatment of infestation with Phthirus pubis: comparative efficacies of synergized pyrethrins and gamma-benzene hexachloride.

In recent years there has been a steady increase in the incidence of infestation with Phthirus pubis, a sexually transmitted louse. A recently introduced nonprescription liquid pediculicide, whose major ingredient is 0.3% pyrethrins synergized by 3.0% piperonyl butoxide (RiD), was compared for efficacy and safety with a prescription-only pediculicidal lotion whose major ingredient is 1% gamma-benzene hexachloride (Kwell). Thirty adult man and women with P. pubis infestation were assigned randomly to treatment with either the synergized pyrethrins or gamma-benzene hexachloride. A single 10-min application of the synergized-pyrethrin liquid produced the same results as a single 12-hr application of the gamma-benze hexachloride lotion: total eradication of adult lice and nymphs, cessation of pruritus, and no treatment-induced side effect. A follow-up visit a week after treatment verified all eradications.
