Caries prevalence of preschool children in Baja, Hungary, in 1975 and 1982: effect of a supervised monthly NaF mouthrinsing program.

The aim of the study was 1) to compare the caries prevalence of preschoolchildren in Baja with data from 1975 and 2) to evaluate the effect of 0.2% sodium-fluoride mouthrinsings practiced during the last 4 yr. In 20 kindergartens of Baja 1462 children between 3 and 6 yr of age were investigated. All the children were participants in an oral hygiene motivation program and fluoride rinsings, performed generally monthly, but at least 10 times in a year. The dental investigations of the primary teeth were carried out by the same team as in 1975. The frequency of caries-free children increased from 18.8% in 1975 to 24.8% in 1982. The dmft mean values decreased only in the 5- and 6-yr-old children.

Collaborative WHO xylitol field studies in Hungary. I. Three-year caries activity in institutionalized children.

The aim of this 3-year field study was to assess the value of partial substitution of sucrose with peroral xylitol (14-20 g/day) as a caries-preventive measure (X group) in comparison with systemic administration of fluoride (F group) and restorative treatment procedures solely (C group). An F dentifrice was used unsupervised in the X and F groups, the former containing 10% xylitol. The C group used customary, predominantly F-free dentifrices distributed by the local health authorities. The final material consisted of 689 institutionalized children (6-11 years). Caries was scored yearly in duplicate by two continuously calibrated teams. At base line the X group had a significantly higher caries prevalence than the F and C groups. The 3-year DMFS increment was 4.2 in the X group, 6.5 in the F group, and 7.7 in the C group. The corresponding ratio (RS) between caries incidence and the tooth surface population at risk was RSx, 4.9; RSF, 6.6; and RSC, 8.6. It is concluded that dietary xylitol in solid sweets resulted in a lower increment of caries than obtained in the F and C groups (p less than 0.001, covariance analysis, with base-line prevalence, number of permanent teeth, and visible plaque index as covariants).

Collaborative WHO xylitol field studies in Hungary. VII. Two-year caries incidence in 976 institutionalized children.

The aim was to assess caries increment as influenced by partial substitution of sucrose by xylitol (X group) over a 2-year period in comparison with systemic fluoride (F group) and restorative treatment only (C group). The study differed from the 3-year field study of the same series primarily in that existing base-line differences were eliminated because the protocol required that all the new subjects entering the institutions in the 1st year were to be included for a 2-year trial. During this period the number of dropouts was 243 (19.9% of all subjects), the final material consisting of 976 children (6-12 years old). The 2-year DMFS increment was 3.8 in the X group, 4.8 in the F group, and 6.0 in the C group. The corresponding ratio (RS) between caries incidence and the tooth surface population at risk was RSX, 4.5; RSF, 5.5; and RSC, 7.5. The xylitol regimen resulted in a lower increment of caries than measured in the F and C groups (p less than 0.001; convariance analysis, with base-line prevalence, number of permanent teeth, and visible plaque index as covariants.

Identification of a surface antigen, common to murine lung and Lewis lung carcinoma, by monoclonal antibody.

Lewis lung carcinoma (3LL) is a spontaneous murine lung carcinoma which preferentially metastasizes into the lung. Monoclonal antibodies (MAb) against 3LL cells were prepared by fusing spleen cells from rats immunized with 3LL and the P3-NSI/I-Ag4 (NSI) plasmacytoma. Two hybridomas were selected which secreted antibodies capable of preferential binding to 3LL cells and not to other malignant cells. Immunohistochemical staining of sections prepared from various mouse organs with one of the MAbs (22.2), revealed exclusive staining of the lung tissue. The data suggest that a lung-specific, 3 LL-associated antigen, is detected by the 22.2 MAb on 3LL cells. The 22.2 antigen is a surface molecule, and is stable following trypsin treatment and fixation by glutaraldehyde or picric acid. Cells that were prepared from a subcutaneous tumor or from lung metastases express high amounts of 22.2 antigen compared to cells of the culture 3LL line (3LLC). Upon reinoculation of 3LLC cells into mice there was a gradual increase in 22.2 antigen expression. It was possible to select from the heterogeneous 3LLC cell population several clones which express high amounts of antigen. These clones differed also in their metastatic patterns. However, the metastatic capacity of these clones did not correlate with expression of the 22.2 antigen. Thus, it appears that expression of the 22.2 antigen is not a necessary condition for formation of lung metastases by 3LL.

Effect of fluoridated milk on caries: 3-year results.

The effect of fluoridated milk consumption has been investigated in a follow-up study of 269 children aged 3-9 years. Children consumed 200 ml of milk daily, fluoridated in the groups 3-5 years with 0.4 mg F-, and in those aged 6-9 years with 0.75 mg F-, for 300 days yearly. The caries data were analyzed with special regard to changes in the DMF values of the permanent first molars, the yearly examination data of the corresponding age groups compared and correlated with the length of fluoridated milk consumption. Statistical analysis of the permanent first molar data showed significant negative correlation between DMF means and length of fluoridated milk consumption in children 5-6 years old at the time of evaluation. The caries reduction after 3 years was 74% and statistically significant (P less than 0.001). In children 7-9 years old the caries reduction of the permanent first molars was less, and statistically not significant. A slight reduction of DMFS mean values in the front teeth of this group could be observed.

Production of hybridomas secreting monoclonal antibodies against components of normal and malignant cells.

The advantages of the lymphocyte hybridization approach has been demonstrated in two systems where monoclonal antibodies of defined specificity have been produced against complex antigens as the acetylcholine receptor and murine carcinoma cells. In both cases it was possible to obtain monoclonal antibodies with specificities that could not be detected in "conventional" antisera. Rare determinants of the AChR were described, one of which is associated with the binding site of cholinergic drugs. A new antigenic marker common to the 3LL metastatic carcinoma and to the lung tissue was defined. A system is described which increases the frequency of positive hybridomas resulted from the utilization for fusion of antibody forming cells that were enriched by antigenic challenge in adoptive transfer system.

Prognostic evaluation of secretory otitis media as a function of mastoidal pneumatisation.

Fifty-two secretory otitis media (S.O.M.) ears with protracted course were compared roentgenologically (Schuller projection) with 52 S.O.M. ears which recovered after insertion of a single ventilating tube. Measurements were done first by comparing the pneumatised area millimetrically and later semiquantitatively according to the size of the mastoid cells--grading the mastoid cells from 1 (eburnize) to 10 (very big mastoid cells). S.O.M. ears with protracted chronic course showed an average mastoid area of 312 mm2, their cell size showing on the average a diploic or small-diploic cells--corresponding to grade 3.5. On the other hand, S.O.M. ears which recovered promptly had an average mastoid area of 440 mm2, and their mastoid cells began to show as actual cells (small up to medium cells), corresponding to grade 5.7. The difference, both millimetrically and grade-wise, between the two groups was found to be very significant (P greater than or equal to 0.01). We may conclude that the prognosis of S.O.M. is related to the size of their mastoid cells or alternatively to the amount of air in the middle ear cleft as a whole.

Clearance by the mucociliary system in 'simple chronic otitis media'.

This is a study of the rate of middle ear clearance in chronic otitis media and atelectatic ears. The clearance rate was measured with the aid of non-soluble saccharin, introduced into the middle ear through an existing perforation in 'chronic ears', or through a ventilating tube in atelectatic ears. Cholesteatomatous ears were excluded. Altogether 122 ears were examined and about two-thirds of the subjects felt the sweet taste in their mouths thereafter. The average time it took for the saccharin to be transported from the middle ear to the taste buds was 33' 22" minutes in non-infected (i.e. dry) chronic ears (38 patients or 66 per cent). Wet or infected ears with chronic otitis media (30 patients, or 58 per cent) showed a slower transport rate, averaging 54' 22" minutes--the difference was significant at the 0.01 level. 8 (63 per cent) of the atelectatic ears transported the saccharin at an average rate of 50' 25". This study demonstrated that most ears with 'simple' chronic otitis media and atelectatic ears have a patent eustachian tube and that their mucociliary system can transport foreign particles through it. When the ear is infected, transport tends to be slowed down.
