Migraine: the platelet hypothesis after 10 years.

The proposal that migraine is a blood disorder and caused by a primary abnormality of platelet behaviour was first put forward in 1978. This paper outlines the basis on which the proposal was made and the way in which the platelet hypothesis can account for the many facets of the disorder. It also reports further studies of platelet composition and function which have been undertaken by a large number of independent workers during the past ten years. The results of their investigations provide strong additional support for the platelet hypothesis in migraine.

Migraine: a platelet disorder.

The hypothesis that migraine is caused by a primary abnormality of platelet behaviour was investigated in a total of 77 migraine patients and control subjects. Platelets taken from patients with classical migraine during a headache-free period demonstrated significantly higher spontaneous platelet aggregation and platelet adhesion than platelets from controls. Platelet 5-hydroxytryptamine release within three days of a migraine attack was significantly less than that measured during a migraine-free interval. Platelets from migraine patients differ significantly in their behaviour from normal platelets, and these differences can explain the onset and recurrence of attacks.

Migraine. A platelet hypothesis.

This paper presents evidence for my hypothesis that the diverse features of a migraine attack, both during the prodromal and the headache phases, become explicable if migraine is caused by a primary abnormality of platelet behaviour. This hypothesis places migraine among the most common disorders of the blood.
