[Occupational disease 2108. Degeneration pattern in magnetic resonance tomography of the lumbar spine in patient with differential weight-bearing activity].

The addition of vertebral disc degeneration to the job-related disease register raises the question of vertebral disc degeneration patterns according to loading strain. The readings of the lumbar vertebra of construction workers and nurses were compared with those of a group without workload. In the groups examined, aged 35 to 50, monosegmental damage was found in only 17% of the patients with high workload, as opposed to 29% of those with no workload, mostly with monosegmental damage at level L5/S1. Damage to the upper segments of the lumbar spine with intact discs in between was found exclusively in patients with high workload. Multiple segment damage in the age range examined was found in subjects with activities that add to the load of the spinal column. The value of MRI in assessing and evaluating illness originating from the vertebral discs is currently being discussed.

[Infectious spondylitis. A retrospective evaluation of MRI markers].

AIM AND METHODS: The aim of the present study was to evaluate the MRI criteria of infectious spondylitis (spondylodiscitis). The MR images of 23 patients suffering from spondylodisitis (78% unspecific, 22% specific) were retrospectively analyzed. RESULTS: The height of the intervertebral discs involved was normal in 40%, reduced in 43% and increased in 17% of the cases. The most common findings can be summarized in an MR triad: 1) The vertebral bodies involved are hypointense in T1-weighted images (100%) with a lack of delineation of the intervertebral discs (53%). 2) The injection of Gd-DTPA yields an enhancement of the vertebral bodies involved and intervertebral discs (95% and 74% respectively). 3) The vertebral bodies and intervertebral discs are hyperintense in T2-weighted sequences (76% and 90% respectively). When present, a paravertebral or intraspinal extension of the infection was isointense compared with the adjacent involved vertebral body in the majority of the patients. A differentiation between unspecific and specific etiology based on the MR images was not possible. CONCLUSIONS: The vertebral bodies affected were usually hypointense in T1-W with enhancement after the administration of Gd-DTPA and hyperintense in T2-W. The discs involved were usually hyperintense in T2-W and demonstrated an inhomogeneous enhancement.

[Open segmental bone transport. A therapeutic alternative in post-traumatic and osteitis soft tissue and bone defects].

Between 1990 und 1992, 22 patients with bone and soft tissue defects were treated by open segmental bone transport according to Ilizarov, and 20 of these were followed up over a period of 18 months after removal of the external fixator. In 19 cases the bone defect could be filled by callus distraction only, while 1 patient needed additional spongeous bone transplantation because of delayed ossification. In 1 case amputation was necessary because of extensive fistula carcinoma of the resected bone and soft tissue. Closure of soft tissue defect was achieved in 14 patients simultaneously with docking of the bone segments. In 5 patients additional skin transplantation was needed, and in 1 case a latissimus dorsi flap had to be transferred because of unstable scar formation. Transporting vital bone simultaneously with the overlying tissue into a defect allows for limb salvage even in poor biological conditions and in patients with vascular problems. Open bone transfer has extended the range of methods available for the treatment of bone and soft tissue defects.

[Diagnosis and spontaneous course of non-traumatic localized myositis ossificans].

Non-traumatic localized myositis ossificans is a rare variant of pseudotumoral muscular ossification. In the early stage it may be misdiagnosed as soft tissue sarcoma or some kind of osteosarcoma. Myositis ossificans itself does not always require therapy; when diagnosed on the basis of the typical features revealed by meticulous non-invasive investigation, biopsy is not mandatory for confirmation of the diagnosis. The diagnostic procedures needed - radiography, ultrasound, MRI, and isotope bone scan - are discussed with reference to three clinical case reports.

[Management of the patient with craniocerebral injuries at the accident site and clinic admission].

Between January 1991 and December 1992, there were 686 rescue operations involving patients with craniocerebral trauma in the catchment area of Ulm. There were 376 patients who had to be graded as seriously injured according to the NACA classification. In 178 cases there was a severe craniocerebral trauma, and 131 of these patients were admitted to the traumatology department of the University of Ulm. The pattern of injuries was analysed; multiple injuries were found in 63 patients, with injuries to the extremities and the thorax being most-frequent. The primary preclinical treatment for patients with craniocerebral trauma is demonstrated; the indications for intubation and artificial respiration are discussed, and also the selection of drugs. Diagnostic procedures and immediate treatment must initially be directed at securing vital functions. Treatment of life-threatening haemorrhage has priority over neurosurgical diagnosis and therapy. The urgent indications for neurosurgical intervention are: space-occupying intracranial bleeding, open craniocerebral traumas, and space-occupying depressed fractures.

[The effect of the sperm collection rhythm on the sperm production of insemination bulls in subtropical climate conditions].

A contribution is presented towards stabilizing the sperm production of A. I. bulls under subtropical conditions. The results are outlined of a production experiment on the influence of different sperm collection frequencies on the sperm production as found in 48 insemination bulls of a central bull station in the Syrian Arab Republic in the course of one year. It was found that a rhythm of 2 collections per bull and week provides favourable conditions for a stable and high-level sperm production.

[Do organizational structures and factors influence the outcome of physiotherapy--a questionnaire survey related to whiplash-associated disorders].

BACKGROUND: The effectiveness of physiotherapy in patients with whiplash injuries of the cervical spine is a subject of critical discussion. Besides the inadequate situation with regard to the medical data available and the financial cuts in drug prescription, organizational factors and structures are assumed to be factors that have an influence on the effectiveness of physiotherapy. AIM: The aim of this study was to evaluate data on training, patient population, communication, prescription activity and therapies of physiotherapists. METHODS: In a survey of 213 physiotherapists in the City of Ulm, data were collected with the aid of questionnaires related to "therapy of whiplash-associated disorders". RESULTS: The results show deficits in the theoretical knowledge of physiotherapists, in the exchange of information and in therapy management. The survey further revealed that 22% of the patients decide themselves which therapy they will undergo. One can assume that these factors have a detrimental effect on the effectiveness of conservative therapy. CONCLUSION: An optimization of the organizational structures is thus urgently required and constitutes the only possibility, in the short to medium term, of improving the effectiveness of physiotherapy in patients with whiplash injuries of the cervical spine.

Endoprosthetic treatment of metastatic pathological fractures.

Due to a prolonged time of survival patients with lowrisk skeletal dissemination may develop a local relapse after surgical treatment of a pathological fracture. In a series of 180 patients with bone metastases of breast cancer, a local metastatic recurrency was observed 8-36 months after operative treatment in 12 patients. All of them had a receptor-positive primary and were free from visceral metastases; only one had received radiotherapy postoperatively. Patients with low-risk disease therefore should undergo more radial surgical procedures than patients with poor prognosis, in whom reconstitution of stability represents the most important therapeutic goal. In cases with low-risk disease and histologically proven residual tumour a postoperative irradiation should be administered as a rule.
