Lymphocyte subpopulations in hypertrophied adenoid in children.

Adenoid hypertophy is a common feature of childhood. It is currently accepted that it is caused by the antigen-stimulated increased activity of lymphocyte B (D. Bani, O. Gallo, O. Fini-Storchi, Intraepithelial lymphocyte subpopulations and dendritic accessory cells in normal and hypertrophic adenoids, Laryngoscope 10 (1994) 869-873). The adenoid decreases its size with age but the accompanying function alterations are not fully understood (L. Zawadzka-Glos, M. Chmielik, M. Wasik, Cell mediated response in hypertrophied tonsils in children, Nowa Pediatr. 4 (1997) 12-13). The understanding of the adenoid structure that undergoes some changes during the growth period is essential for evaluation of indications for adnoidectomy and assessment of its potential results. The aim of this study was to evaluate lymphocyte subpopulations in adenoid according to age. The analysed material was adenoids removed on the grounds of hypertrophy, which caused obstructive symptoms and/or otitis media with effusion onset. In the present study, we did not find any statistically significant differences among lymphocytes B, Th, and Ts subpopulations, respectively, in the adenoids of any of the age groups. We have found a statistically significant CD3(+) HLA-DR(+) cell percentage decrease in the group of children from 5 to 10 and above 10 years of age, respectively. We have also found a statistically significant increase in the percentage of NK (CD3(-) CD16(+) 56(+)) lymphocytes in relation to age. On the grounds of the current study, it may be stated that some changes in lymphocyte subpopulations in the adenoid take place with age.

Acoustic rhinometry assessment of the nasal cycle in neonates.

In this study the nasal cycle in newborns was assessed using acoustic rhinometry with miniprobe (RHIN 2100). Enrolled in the study were 67 newborns aged 2 to 4 days. Each subject was assessed for 3 h, examinations taking place every 30 min. At least six measurements, three from each nostril, were taken at each examination. The miniprobe RHIN 2100 (SR Electronics ApS) was used for evaluation of nasal geometry. The minimal cross-sectional area (MCA) and the total volume of the anterior 45 mm (VOL45) were calculated and used for statistical analysis. The mean volume of MCA and VOL45 in the examined group of children was 0.08 cm3 and 1.19 cm3 respectively. Nasal cycle was assessed by constructing the plots of acoustic rhinometry parameters MCA or VOL45 for time and statistically using Pearson's correlation coefficients. Most of the examined infants (70-85%) showed no significant fluctuations in nasal patency.

Cytokines in children with otitis media with effusion.

We assayed 38 middle ear effusions from 23 children aged 4-13 years (mean 7) undergoing tympanostomy tube placements. All fluid was assayed for tumor necrosis factor (TNF) alpha, interleukin (IL) 1beta, IL-8, and IL-10. Cytokine concentrations were measured by means of an enzyme-linked immunosorbent assay. Detectable levels of IL-1beta, IL-8, and IL-10 were found in all of the effusions. TNF-alpha was detected in 18 of the middle ear effusions (47.4%). The mean concentration of TNF-alpha, IL-1beta , IL-8, and IL-10 was, respectively, 0.423 +/- 1.39, 30.58 +/- 68.7, 7001.9 +/- 6743, and 56 +/- 58.7 pg/ml. There was a strong, statistically significant correlation between the concentrations of TNF-alpha and IL-1beta (r = 0.87, P = 0.001) and between IL-1beta and IL-8 (r = 0.53, P = 0.001). There was no correlation between the concentrations of IL-10 and other cytokines examined or between tympanic membrane pathology and the concentrations of TNF-alpha, IL-1beta , IL-8, or IL-10. The presence of IL-10 in middle ear effusions may be one of the causes of a lack of clinical features of acute inflammation and may lead to a chronic inflammatory state.

[Tumor DNA content as a prognostic indicator in squamous cell carcinoma of the larynx].

Celluar DNA content has been found to be an important prognostic factor in many head and neck tumours, yet within the larynx few studies have examined its effect on survival. 31 patients with squamous carcinoma of the larynx treated in years 1983-85 by total laryngectomy were studied. The tumours were classified according to the TNM classification 1987 and graded histopathologically according to Jaccobson's classification. Quantitative DNA content was determined with the use of image cytometry performed by an image analyzer (CAS-200). Quantitative DNA analysis correlated with tumor grade and patient outcome within 6 years of follow-up.

Distortion products otoacoustic emissions in diagnosis of hearing loss in Down syndrome.

This study was carried out to investigate the features of hearing impairment in subjects with Down syndrome. Forty-seven children and 14 adults with Down syndrome were included in the study. In all cases, a complete otorhinolaryngological examination was performed, followed by audiological assessment. Depending on age, intellectual level and middle ear status the following examinations were performed: pure-tone 'play audiometry', tympanometry, acoustic reflex, auditory brain response (ABR) and distortion products otoacoustic emissions (DPOAE). The results were compared with age matched control groups. Among the group of children with Down syndrome, we have frequently found impairment of the conductive function of the middle ear which was expressed by pathological tympanometry. Tympanometry of B and C type was detected in 56% of ears. The amplitude of DPOAE was lower in children with Down syndrome than in the control group. This difference was more expressed in adults with Down syndrome. DPOAE examination results in subjects with Down syndrome without conductive hearing loss indicate early age related inner ear impairment.

Feasibility of supracricoid laryngectomy based on pathological examination.

Partial horizontal supracricoid laryngectomy with cricohyoidopexy at the Department of Otolaryngology, Medical University of Białystok, involves resection of the entire thyroid cartilage with the pre-epiglottic and paraglottic spaces. Experience has shown that this technique will permit adequate phonation, respiration and deglutition in selected advanced cases of supraglottic and glottic carcinoma. The purpose of this study was to verify histologically the indications for this surgery by examining sections of whole-organ laryngeal specimens. Post-laryngectomy specimens from 90 patients with otherwise previously untreated supraglottic (48), transglottic (22) and glottic (20) carcinomas were reviewed retrospectively. The majority (66) of the specimens were staged as pT4. Findings showed that 22 of the specimens analyzed (mostly supraglottic tumors) could have been eradicated by supracricoid laryngectomy alone. The present study confirmed the principles of supracricoid laryngectomy for selected large tumors.

Triple procedure; analysis of outcome, refraction, and intraocular lens power calculation.

AIMS: A total of 97 triple procedures performed over a 6 year period were studied retrospectively to determine the best approach to calculate intraocular lens power. METHODS: The cases were divided into two diagnostic categories. RESULTS: After 1 year best corrected visual acuity was 20/40 or better in 37.5% of the cases of the 'modified group'. This group consists of patients with the diagnosis Fuchs' dystrophy, non-guttate endothelial dystrophy, and Reis-Buckler dystrophy. Analysis of visual acuity was made using logMAR. A final postoperative refraction within 2 dioptres of predicted refraction was achieved in 76.5% of patients in the modified group. CONCLUSION: In future, in the absence of a keratometry, a keratometry value of 7.49 mm will be used for calculation of the power of the implant as analysed in this study.

Comparison of the Storz bifocal zonal and the 3M diffractive multifocal intraocular lenses.

Visual acuity and contrast sensitivity after cataract surgery were evaluated in best-case eyes that received a Storz bifocal zonal intraocular lens (IOL) or a 3M diffractive multifocal IOL. A group of eyes with a monofocal IOL was used as a control. Postoperatively, all eyes had a best corrected visual acuity of 20/40 or better. At near, wearing the best distance correction, 86.0% of the Storz group and 94.5% of the 3M group had a visual acuity of 20/40 or better. Both the Storz and 3M groups had a significantly greater loss in visual acuity in dim light at the lowest contrast levels than the monofocal group. Under bright light, the Storz group's contrast sensitivity was not significantly different from that of the monofocal groups; however, the 3M group's contrast sensitivity decreased at all levels of contrast. The Storz lens performed slightly but significantly better than the 3M lens in bright light (96% and 25% Regan levels), perhaps because of the difference in IOL optics or small differences in the incidence of posterior capsule opacification, for which this study was not controlled. In the Storz and 3M groups, distance visual acuity was better than distance visual acuity using a minus addition, which makes the patient use the near portion of the optic.

Ventriculosaccular carcinoma of the larynx.

A series of 76 cases of laryngeal carcinoma was examined in transverse slices using a motorized slicing machine. Eleven cases of ventriculosaccular carcinoma were detected by this method and the topography and gross appearances of the tumours in the larynges are described. Microscopy of the tumour indicates a keratinizing squamous cell carcinoma the mode of growth of which seems to be by concentric expansion through the whole margin rather than by invading tongues of tumour. In no case was there invasion of laryngeal cartilages or extension to the surgical cut surface of the specimen. The clinical, radiological and biopsy features in 10 of the 11 patients are described. In follow-up studies, which were 12 years or longer in five patients, none had lymph node metastases or recurrences of the laryngeal carcinoma of any sort. The following features may suggest the diagnosis of ventriculosaccular carcinoma before laryngectomy: (1) a ventricular tumour, (2) a supraglottic bulge above it, (3) a paraglottic swelling on CT scan radiology covered by a smooth laryngeal lining and (4) biopsy appearances of a well differentiated 'folded carcinoma'.

Scanning electron microscopy of human ossicles.

Human middle ear ossicles removed from cadavers and during surgical procedures have been examined with scanning electron microscope (SEM). Two kinds of bone tissue compose ossicles: periosteal and endochrondral bone. Periosteal bone occurs in the form of external covering on the ossicles. In the middle of the ossicles there is endochrondral bone which shows various spatial arrangement of collagen bundles. Lamellar bone occurs also in this bone around vascular channels but rarely in the form of typical osteons. Surface appearance of periosteal, coarse-fibred bone and lamellar, fine-fibred bone is almost the same. Lamellar bone is composed of coarse collagen bundles arranged tightly in the rows. The space between the rows is filled by fine fibres which link the coarse bundles.
