Cytokeratin profiles and mucin secretion in combined hepatocellular-cholangiocarcinoma. A case report.

A case of a combined hepatocellular-cholangiocarcinoma (HCC-CC) is presented showing mucin production in both the HCC and the CC component. Immunohistochemical staining for cytokeratins 7 and 19 was performed and it is concluded that immunoreactivity for cytokeratin 7 and 19 is an additional criterion to the detection of mucin in making the diagnosis of a combined HCC-CC of the transitional type.

Subcutaneous diffuse neurofibroma of the neck: a case report.

A case of a rare and unusual variant of neurofibroma, diffuse neurofibroma (paraneurofibroma), in a young patient is presented. The clinical, radiological and histopathological features of this case are reported. The magnetic resonance imaging (MRI) features of the diffuse neurofibroma are comparable with those described in other neurofibromas.

Rhodococcus equi infection in 3 AIDS patients.

Three cases of AIDS complicated by Rhodococcus equi infection are reported. At least one of the patients acquired his Rhodococcus infection in Africa. Despite the fact that the R. equi strains were susceptible to tetracycline, erythromycin, amikacin, co-trimoxazole, rifampicin and vancomycin, these antibiotics were clinically not successful. A clinical improvement was observed in only one patient during teicoplanin and imipenem-cilastatin treatment. Multicentre clinical trials are needed to determine the optimal treatment of R. equi infections in AIDS patients.

Laparoscopic resection of an adenoma of the urachus in combination with a laparoscopic cholecystectomy.

In a 40-year-old male patient with symptomatic cholecystolithiasis, ultrasound examination disclosed a large cystic mass on the dome of the bladder. Laparoscopic resection of this mass was carried out in combination with a laparoscopic cholecystectomy. Histology disclosed an urachal adenoma. Postoperative recovery was uneventful. We conclude that urachal adenomas can be managed safely by laparoscopic means.

Histoplasma capsulatum infection in three AIDS patients living in Africa.

Three AIDS patients are reported, 2 European and 1 African. All 3 developed Histoplasma capsulatum infection while living in Africa. Two developed a pulmonary infiltrate, and 2 had skin lesions. One patient was successfully treated with itraconazole, 400 mg daily; in a second patient, itraconazole was partially successful; the third patient was lost to follow-up before adequate treatment could be started. Histoplasma infection is probably an often undiagnosed complication in African AIDS patients.

Mucinous cystadenocarcinoma of the pancreas with liver metastases. An unusual presentation of a rare tumor.

We present a case of mucinous cystadenocarcinoma of the pancreas with liver metastases. Initially, the patient presented as pancreatitis and pseudocyst which was marsupialised into the stomach in 1983. Eight years later, a tumor with liver metastases was detected. The liver metastases mimicked multiple abscesses. Over a period of forty-four days, CT features changed considerably and there was explosive enlargement of the lesions in the postoperative period. The reason for this dramatic increase in size was the anaplastic component of the tumor. To our knowledge, no such case has been reported in literature.
