Comparative in vitro toxicity of mitoxantrone and adriamycin in human granulocyte-macrophage progenitor cells.

Mitoxantrone (MIT) has recently been introduced into cancer therapy as a possible substitute for the structurally related drug, adriamycin (ADR), because it causes less cardiotoxicity and fewer gastrointestinal side effects. However, the dose-limiting toxicity of MIT is pronounced neutropenia. The in vitro hematoxicity of both drugs in granulocyte-macrophage precursor cells (GM-CFCs) was analyzed using drug-exposure schedules analogous to the principles of the in vivo pharmacokinetics of MIT. Bone-marrow and peripheral-blood cells were exposed to 0.075-20 ng/ml MIT or ADR for 5, 20, 60, and 120 min, and for 14 days. The 14-day exposure resulted in Do values of 0.95 and 0.68 ng/ml for bone-marrow and peripheral-blood GM-CFCs subjected to MIT. Exposure to ADR resulted in Do values of 5.43 and 5.13 ng/ml, respectively. As was the case after 14-day exposure to MIT or ADR, short-term exposure again revealed that peripheral-blood GM-CFCs were more sensitive to both drugs. Moreover, at low concentrations, ADR was less toxic than MIT in both types of GM-CFCs, but was more toxic than MIT when a concentration of 20 ng/ml was used. The intracellular concentration of MIT, as measured by high-performance liquid chromatography, was constantly below 1 ng per 2 X 10(7) cells, even when it was applied at a concentration of 20 ng/ml for an exposure time of 2 h. The fact that such low concentrations of MIT are toxic for hemopoietic precursor cells may explain the myelotoxicity of this drug. However, the difference between the precursor-cell toxicity of MIT and that of ADR was small when their respective therapeutic doses were taken into consideration. Further analyses of their toxicity in stem cells and/or the microenvironment would appear to be needed.

Recombinant interferon alpha 2 stimulation of target-binding by natural killer cells.

Even though the enhancement of the lyitc capacity and the kinetics of lysis of natural killer cells (NK) by interferon has been well documented, an increase of the target-effector cell binding percentage is still disputed. We, therefore, modified the Grimm-Bonavida single-cell assay so that 400 to 600 cells per individual determination could be reliably evaluated. Using this assay, which makes possible separate determination of effector-target cell binding and target lysis, we demonstrated that, in addition to lytic capacity, target-effector cell binding is also increased by preincubating NK with 100 to 1,000 IU interferon alpha 2 per 10(6) cells. Our data indicate that interferon alpha 2 induces pre-NK cells to bind target cells and that it activates these pre-NK cells to kill the targets.

Natural killer cell activity against a thymoma cell line Thy 121 in bone marrow transplant recipients.

Twenty-one patients with acute and chronic leukemia or severe aplastic anemia were studied for NK activity against a thymoma cell line (Thy 121) before and after allogeneic bone marrow transplantation. The means of the pretransplant and post-transplant levels did not differ from the mean of 134 NK determinations in 67 healthy donors. There was no correlation between pretransplant NK levels and the appearance of graft-versus-host disease. Three weeks following bone marrow transplantation, pretransplant NK levels were observed. The sensitivity of NK cells to interferon was the same as in normal donors both before and after bone marrow transplantation. In contrast to methotrexate, cyclosporin A inhibited NK activity in patients and controls in vitro. In vivo cyclosporin A treatment, however, did not decrease NK levels in bone marrow recipients.

Diagnosis and treatment of meningeal carcinomatosis in ten patients with breast cancer.

Clinical and radiological findings of 10 breast cancer patients with meningeal carcinomatosis were recorded. Special attention was drawn to CSF findings. Initial CSF cytology revealed malignant cells in all cases with a background of inflammatory cells in 7. Oligoclonal bands were found in the CSF of 3 out of 5 cases examined. Repeated CSF control under therapy showed elevation of lactate to be a better diagnostic parameter than lowered glucose. Six patients responded well to CNS radiation therapy and intrathecal chemotherapy. Median duration of survival was 6.4 months.

Mitoxantrone in the treatment of relapsed and refractory acute leukemia.

Twenty-four patients with acute leukemia or blast crisis (BC) of chronic myelocytic leukemia (CML) in relapse or refractory to standard chemotherapy, were eligible for treatment with mitoxantrone. Mitoxantrone (Novantrone; dihydroxyanthracenedione) was administered in a dose of 8-13 mg/m2 on five consecutive days. Five of 20 evaluable patients were induced into complete remission, one patient achieved a partial remission. Side-effects included moderate to severe bone marrow suppression, moderate mucositis and hair loss. No cardiotoxicity was observed. We believe that mitoxantrone is an active agent in the treatment of acute leukemia and suggest further studies in combination chemotherapy.

[Improved clinical course of herpes zoster in immunosuppressed patients treated with fibroblast interferon].

In a randomized study with patients suffering from herpes zoster spread of eruptions was inhibited and segmental pain was reduced rapidly by continuous infusion of daily 0.5 X 10(6) IE fibroblast interferon/kg body weight for 3-5 days. Postherpetic neuralgia was observed more rarely and interferon healing of eruptions was accelerated by interferon compared to the controls.

Participation of the alpha 2(I) chain of bovine skin collagen in the formation of mature crosslinks.

It is shown that regions of unreduced, insoluble cow hide collagen, represented by the peptides alpha 1(I)-CB6, alpha 2(I)-CB4 and the alpha 2(I)-CB3,5, are involved in the formation of unreducible acid-stable and mature-type crosslinks. The characteristic ratio of the CNBr peptides in soluble type I collagen was found to be changed in the insoluble collagen of cow hides. The intensity of the bands of alpha 1(I)-CB6, alpha 2(I)-CB4 and alpha 2(I)-CB3,5, shown by dodecyl sulfate polyacrylamide gel electrophoresis, is significantly reduced in such samples, which indicates an involvement of these peptides in crosslink formation. The purified highly polymeric CNBr peptide fraction was also investigated to confirm the participation of the alpha 2 chain of type I collagen in mature crosslink formation. Chymotryptic digests of such material contain peptides which originate from alpha 2(I)-CB4, alpha 2(I)-CB3,5, and alpha 1(I)-CB6. Finally, acid hydrolysates of crosslinked material were screened carefully for crosslinks down to concentrations of 1 in 1000 amino acids. Only two compounds were detected, one identified as "hydroxyaldol-histidine" and the other an as yet unknown compound. These results indicate that both the alpha 1(I) and the alpha 2(I) chains are involved in mature crosslink formation and that the polymeric CNBr peptide fraction contains components crosslinked by so far uncharacterized, nonreducible crosslinks.

[Comparison of the therapeutic results in treating chronic myelosis with splenic irradiation or busulfan].

In a retrospective study the survival of 111 patients with chronic myeloid leukemia was analyzed. There was a slight superiority of the patients primarily treated with busulfan over those first getting splenic irradiation and busulfan subsequently. This difference was statistically not significant. If spleen size prior to treatment was less than 10 cm below left costal margin, there was also a tendency in favor of primary busulfan therapy. In patients having bigger spleens, neither form of treatment had any advantage.

[Chemotherapy of nonseminomatous testicular tumors. The results of a pilot study with reduced cis-platin dosage].

23 patients with non-seminomatous germ cell tumors of the testis were treated with Cis-Platinum 50 mg/m2 body surface on day 1, Vinblastine 0,4 mg/kg body weight in two doses on days 1 and 2, and Bleomycin 30 mg daily as continuous infusion (days 2-5). 7 patients were treated adjuvant after retroperitoneal lymphadenectomy. One of these had a relapse, he again received the same therapy and now has a complete remission. All other adjuvant treated patients are free of tumor. Of the 16 patients with large retroperitoneal and/or lung metastases, 10 (62%) had a complete remission, 2 of them after resection of persistent metastases after chemotherapy. 4 of the 10 patients with complete remission relapsed. In two patients with relapse a new complete remission could be established by the same chemotherapy. The remission rates achieved with this therapy with a relatively low dose of Cis-Platinum are similar to those obtained with higher doses of this agent. It is possible that there are more relapses, but these respond well to further treatment. Thus, the survival times of the patients treated with this protocol are similar to those reported in the literature. It is concluded that this regimen is an effective treatment in non seminomatous germ cell tumors.

Effect of human fibroblast interferon on natural killer cell activity: stimulation in vitro and inhibition in vivo.

This paper describes the influence of human fibroblast interferon (IFN-beta) on the cytotoxic activity of natural killer cells (NK) in vitro and in vivo using the blood of healthy donors and myeloma patients. IFN-beta stimulates NK activity against all target cells tested in vitro in a dose-dependent way up to 250% of pretreatment values. At higher IFN concentrations, stimulation returned to baseline values. Stimulation was most pronounced in the lowest lymphocyte to target cell ratio. 1- to 2-h preincubation of effector cells with IFN was enough to achieve maximal stimulation. The effector cells of IFN-treated myeloma-patients, or patients with herpes zoster, showed a clear reduction of toxicity against all cells tested during the first infusion, as compared to the pretreatment values.
