[Morphofunctional changes in gastrointestinal tract in patients with psoriasis accompanied by chronic opisthorchosis].

The paper covers clinical, morphological and functional aspects of gastrointestinal tract condition in patients with psoriasis accompanied by chronic opisthorchosis (CO). The authors examined 150 patients with psoriasis accompanied by CO, 100 patients having psoriasis without helminthiasis, 100 patients with CO and 30 healthy people. The gastric secretion was evaluated by means of the fractional test (both phases) with histamine stimulation; other diagnostic procedures included carbohydrate absorption evaluation (5-gram D-Xylose absorption test) and Kamer test of fat absorption. The morphological condition of the gastric and intestinal mucosa was investigated by means of light and electron microscopy. The study revealed gastric secretory dysfunction and malabsorption in small and large intestines in patients with psoriasis and CO, clarified the relation between the duration of psoriasis and opisthorchosis and gastric secretory dysfunction and determined dependence of small intestine malabsorption on such factors as stage, severity, degree of skin involvement and duration of psoriasis. The authors also established interrelation between the above malfunctions and gastric and intestinal structural abnormalities.

[Impact of age and type of behavior on the course of ulcer disease].

AIM: To study features of ulcer disease (UD) course with reference to a biological age, type of behavior, quality of life for recommendations on psychological rehabilitation of these patients. MATERIAL AND METHODS: Examination of 111 gastroduodenal UD patients determined type of behavior, correlation of biological age (BA) and due biological age (DBA), quality of life (QL). RESULTS: An unfavourable BA/DBA correlation was found in 71.1% cases, in which patients over 45 years of age had a larger ulcer defect size. Behavior type A with shorter scarring was observed in 30.9% patients. Lower QL was explained by sleep disorders, pain, emotional reactions, the demand in continuous treatment, diet, limited physical exercise. UD patients have a low QL index and a high index of the disease severity. These parameters are higher in unfavorable correlations between BA and DBA and in patients with type A behavior. CONCLUSION: UD patients have behavioral peculiarities dependent on BA and affecting the disease course and QL.

[Clinical effectiveness of dehelmintization in patients with psoriasis accompanied by chronic opisthorchosis].

The purpose of the study was to prove the appropriateness of dehelmintization in patients with psoriasis accompanied by chronic opisthorchosis (CO). The authors examined 150 patients with psoriasis accompanied by CO, 100 patients having psoriasis without helminthiasis, 100 patients with psoriasis and 30 healthy individuals. Gastric secretion was evaluated by means of the fractional test (both phases) with histamine stimulation; other diagnostic procedures included carbohydrate absorption evaluation (5-gram D-Xylose absorption test), Kamer test of fat absorption and evaluation of small intestine bioelectric activity by means of electromyography. The patients were followed up within 2 to 3 years. The study found negative dynamics in the parameters of gastric secretion, fat and D-xylose absorption and small intestine bioelectric activity in patients with psoriasis and CO within the 2-3-year follow-up, while the group of dehelmintized patients displayed significant improvement of these parameters. Thus, effective dehelmintization allowed improvement of alimentary tract functional condition and the clinical course of psoriasis.

[Absorptional function of the small bowel in patients with chronic obstructive bronchitis].

The purpose of the study was to evaluate intestinal absorption of fats, proteins and carbohydrates in patients with chronic obstructive bronchitis (COB) depending on the functional condition of the lungs, and clinical manifestation of its disorder. Fat, protein and carbohydrate absorption was studied in 68 COB patients. Lung ventilation was evaluated by means of spirography, pulmonary gas exchange--via measurement of paO2 of arterialized capillary blood by Astrup method. Fat absorption was evaluated using Kamer method and radioindication with 131I-trioleate-glycerine and 131I-oleic acid; protein absorption was determined using 131I-albumin; carbohydrate absorption was determined by d-xylose test. The study demonstrated that deterioration of respiratory and ventilatory pulmonary functions in patients with COB impaired fat, protein and carbohydrate absorption: paO2 > or = 70 mmHg and ventilatory insufficiency (VI) I were associated with an significant reduction of fat absorption by 49% on the average, protein--by 23%, carbohydrates--by 15%; paO2 = 70-40 mmHg and VI II-III--with reduction of these parameters by 104%, 69%, and 41%, respectively. The results show a direct correlation between body weight (BW) deficit and the reduction of fat, protein, and carbohydrate absorption (r = 0.549, 0.707, and 0.482, respectively) in COB patients. The most prominent was the correlation between BW deficit and the reduction of protein absorption. Thus the study of intestinal absorption of fats, proteins and carbohydrates in patients with COB demonstrates not only reduction of absorption, correlating with the degree of the impairment of ventilatory and respiratory pulmonary functions, but also direct correlations between BW deficit and reduction of fat, protein and carbohydrate absorption.

[Immunoregulatory cytokines and chronization of type C hepatitis: matching of clinical and immunological manifestations].

The study demonstrates significant changes of cytokine-producing ability of mononuclear blood leucocytes in type 'hepatitis, correlating with the degree of hepatic lesion. The results showed that various morphologic types of chronic virus type C hepatitis were associated with disbalance in production of pro- and antiinflammatory cytokines, i.e. decrease of tumor necrosis factor (TNF)-alpha concentration and a significant rise of interleukin (IL)-4 and IL-10 concentations, which were the most prominent in cases of severe hepatic fibrosis and high histologic activity of hepatitis. The study established a direct correlation between increase of the ability of mononuclear blood leucocytes to synthesize IL-4, and the progress of the pathologic process.

[The course of chronic glomerulonephritis in chronic opisthorchiasis].

AIM: To characterize clinical, functional and morphological features of chronic glomerulonephritis (CGN) running with chronic opisthorchiasis (CO) and to justify dehelminthization. MATERIAL AND METHODS: Clinical, functional and morphological examinations of the kidneys, immunological characteristics were studied in 100 patients with primary CGN and CO (group 1), 30 patients with CGN free of CO (group 2) and 40 patients with long-term CO. RESULTS: CGN in CO runs with frequent rise of creatinine, glomerular filtration and canal reabsorption fall. Pathogenetic therapy with addition of pulse cyclophosphamide in a dose 10 mg/kg and conduction of dehelminthization a year later lead to long-term remission and inhibition of nephrosclerosis development. CONCLUSION: Clinicofunctional and morphological characteristics of the kidneys in mixed pathology necessitate addition of immunosuppressor cyclophosphamide in a dose 10 mg/kg and dehelminthization in combined treatment of patients with glomerulonephritis and opisthorchiasis.

[Peculiarities of duodenal ulcer with concomitant bronchial asthma (clinical and pathomorphological aspects)].

AIM: To study incidence of gastroduodenal ulcer (GDU) in bronchial asthma and clinicomorphological features of ulcer disease in BA in long-term use of systemic corticosteroids (SCS) and without SCS. MATERIAL AND METHODS: The trial covered 297 BA patients. They have undergone clinicoendoscopic examination of the gastroduodenal zone (GDZ) for duodenal ulcer with assessment of clinical gastroenterological symptoms and morphological analysis of biopsies of gastric and duodenal mucosa using histological, histochemical and morphometric methods. RESULTS: BA patients were found to have high incidence of duodenal ulcer (21.9%). Morphologically, they had active immune inflammation with hyperergic component. The results of the study did not confirm the opinion about a key role of SCS in development of GDU in BA. Small doses of SCS in patients with BA and GDU do not deteriorate ulcerogenesis. CONCLUSION: One of the factors determining combination of ulcer disease with BA may be chronic hyperergic inflammation of GDZ mucosa stimulating realization of predisposition to ulcer and its chronic recurrent course.

[The specific features of the course of chronic viral hepatitis concurrent with chronic opisthorchiasis].

A hundred and one patients with chronic viral hepatitis (CVH) in the phase of replication were examined. Group 1 comprised 53 patients with CVH in combination with chronic opisthorchiasis. Group 2 (a comparison group) included 48 patients with CVH without opisthorchiasis. According to the etiology of hepatitis, each group was divided into 3 subgroups: subgroups 1a and 2a consisted of patients with CVHB; 1b and 2b included those with CVHC, and 1c and 2c comprised those with CVHB+C. The clinical, functional, and morphological status of the liver was studied and hepatic tissue was qualitatively and quantitatively assessed in all the examinees. The findings suggest that Opisthorchis is an additional factor that aggravates hepatic damage in patients with CVH in the replicative phase of infection.

[Effect of the phytohepatoprotective agent maxar on the hepatic morphofunctional status in patients with chronic hepatitis].

The paper provides evidence for that as compared with the well-known drug carsil, the new Russian phytohepatoprotective agent maxar has a therapeutic efficiency in patients with mild and moderate chronic hepatitis (CH) of viral and alcoholic etiology. Maxar has been found to produce a more pronounced hepatoprotective effect, to a greater extent, in CH of alcoholic etiology.

[Psychovegetative features in patients with biliary dyskinesia].

The aim was to study psychovegetative disorders in hypomotor and hypermotor forms of biliary diskinesia (BDK) in young patients. Psychological status and vegetative nervous function were studied in 62 patients with hypoBDK, 58 patients with hyperBDK and 33 control patients. According to AMPI, Spilberger's, Aizek's, Heck-Hess' tests, the psychological status of the patients from both groups was similar, while vegetative status in the hypoBDK group at rest and active orthostasis had a sympathetic trend. In hyperBDK group at rest sympathetic impacts enhanced, in stress these impacts intensified reaching higher values than in the control group. For the treatment of various forms of BDK, it is recommended to correct psychovegetative deviations with consideration of diskinesia forms.

["Hematological masks" of chronic viral hepatitis].

AIM: To reveal the structural features of the red blood cell membrane in patients with prolonged persistence of hepatitis B virus (HBV) and hepatitis C virus (HCV). MATERIALS AND METHODS: 61 patients with moderate and mild chronic viral hepatitis B and C were examined. A control group comprised 22 healthy donors. Fluorescence of the red blood cell membrane was performed, by using the following probes: pyrene, phenylnaphthylamine, 1-anilinonaphthalene-8-sulfonate. The activity of the ion-transporting enzyme Na+, K(+)-ATPase was determined. RESULTS: Increases in the microviscosity of the lipid bilayer, periprotein lipid environment, as well as the structural modification of the surface layers of the membrane were found in patients with chronic hepatitis B and C. The activity of the membrane-bound enzyme Na+, K(+)-ATPase was statistically significantly decreased as compared with that in healthy donors. CONCLUSION: In HBV and HCV infection, the impairments of the red blood system are highly diversified: they involve bone marrow compartment of an erythron, as earlier shown, and its peripheral one. Molecular disorganization of the red blood cell membrane fails to present with obvious red blood cell dysfunctions. Despite the multifactorial property of their occurrence, impairments of the red blood system are mediated and corrected by many regulatory mechanisms at different organizational levels and may be compensated.

[Hormonal and immunological aspects of renal lesions in patients with chronic alcoholism].

AIM: To analyze a relationship of the level of sex hormones to the tension of immunological processes in definite renal diseases in patients with chronic alcoholism. MATERIALS AND METHODS: 43 patients with chronic alcoholism who also suffered from chronic renal disease were examined. The levels of plasma testosterone and estradiol were determined by radioimmunoassay using labeled 3H-hormones, as described by V. S. Goncharov and G. V. Katsiya. The immunity was assessed by determining the activity of secreted interleukin-1 in the cultured peripheral monocytes of the patients and in the cultured renal glomerular mesangial cells by radioimmunoassay according to the procedure of S. Mizel and O. V. Gabrilevskaya. The obtained renal biopsy specimens were examined by electron microscopy immunoassay, as described by A. A. Ivanov. The activity of lysosomal enzymes was studied. RESULTS: Activation of lysosomal enzymes and increased synthesis of interleukin-1 by monocytes and mesangial cells were observed in patients with chronic alcoholism and renal diseases with testosterone deficiency and excess estradiol in the plasma. There was a correlation of the level of estradiol with that of interleukin-1 and with the activity of cathepsin D. CONCLUSION: With hypofunction of androgens, estrogens stipulate the tension of immune reactions, which finally leads to renal lesion in patients with chronic alcoholism.

[Clinical and immunologic correlations of gastroduodenal lesions in patients with bronchial asthma].

To study local immunity of the stomach in bronchial asthma (BA) and the role of some immunity factors deficiency in development of gastroduodenal disturbances in BA patients, we have examined 271 male and female patients aged 18 to 60 years with exogenic and mixed BA of different severity. Gastroduodenal erosions were endoscopically diagnosed in 61 (22.5%) patients. We discovered that gastroduodenal pathology in BA develops in local deficiency of secretory IgA (sIgA) and high content of IgG and CIC in the gastric mucus. We registered a dose-dependent effect of long-term oral therapy with systemic glucocorticoids (SGC) in BA on gastroduodenal pathology and local immunity. Physiological doses reduce concentrations of IgG and CIC in gastric mucus and, by attenuation of immunocomplex reactions in gastric mucosa, decreased the rate of gastroduodenal erosions. Treatment with supraphysiological doses of SGC breaks specific defense of gastric mucosa and makes erosions more frequent. Thus, the above derangement of humoral immunity of the stomach in patients with non-hormone-dependent BA has common manifestations and direction with local immunity of the respiratory airways in BA suggesting involvement of "common immune system of the mucosas" in BA.

[Morphological validation of peloidobalneotherapy intensification in patients with duodenal ulcer].

AIM: To study effectiveness of short courses of intensive peloidobalneotherapy in duodenal ulcer (DU). MATERIAL AND METHODS: Esophagogastroduodenoscopy and biopsy before and after the treatment course were made in 124 DU patients. Depending on timing of moor applications, pine and pearl baths, the patients were divided into 2 groups. Patients of group 1 received procedures daily with interval 4-5 hours (intensive course), patients of group 2 were treated each other day. RESULTS: 3-4 weeks of the treatment produced a response in both groups but in group 2 there were no signs of active inflammation in gastric and duodenum mucosa. 2 years later, patients of group 1 had histological evidence of chronic inflammation exacerbation. The latter was absent in group 2. CONCLUSION: An intensive course of peloidobalneotherapy results in attenuation of chronic gastroduodenal inflammation in DU patients.
