An objective method of pulse diagnosis.

One component of Oriental medical diagnosis is pulse diagnosis. To make this method more objective, three pressure transducers were positioned over the radial artery and the absolute pressure of 50, 100 and 150 g was applied at three positions to simulate the classical pulse diagnosis. The transient pressure changes at the same absolute pressure were recorded. The pressure changes at the three positions were not equal even when the same absolute pressure was applied. Pressures were compared in superficial and deep locations, right and left wrist. The largest deflection was considered as excess and the smallest as depletion in twelve meridians.

Clinical study of objective pulse diagnosis.

The recording of the objective pulse diagnosis was performed and the meridian points were selected from this recording. The proper meridian points (five element points) according to the five element theory and the improper points (non-five element points) were stimulated. Pain threshold was elevated by the stimulation of five element points and not raised by non-five element points in a patient who had nasal polypectomy under acupuncture analgesia and also in 6 of 9 healthy volunteers. Naloxone reversal of elevated pain threshold was also observed in volunteers.

Oriental diagnosis in stomach cancer patients.

A new method of performing Oriental diagnosis using Meridian Imbalance Diagram which, consisting of 23 different ratios, has a highly discriminatory significance. Using this method, Oriental and Western diagnosis in the same patients were compared. To do this we compared the Meridian Imbalance Diagrams of 22 patients entering our hospital for surgical operations with those 19 in a healthy control group. Four significantly different ratios became evident. The postoperative reports showed that 10 of the patients were suffering from stomach cancer. The Oriental diagnosis data of this new group were then compared to the data of the control group. This time, 10 ratios were found to be significantly different. This shows the existence of a pattern of Oriental diagnosis corresponding to a specific Western diagnosis.

Differential Threshold

Effect of cardiac output on pulmonary shunt.

The effect of cardiac output on pulmonary shunt was studied in living lungs of human patients and dogs which had alveoli and blood vessels, and also in artificial lung (bubble-type oxygenator) which had no such structural factors. In spite of such a structural difference, quite similar results were observed in both living and artificial lungs, and this suggests that some common factors other than the change in blood vessels result in the change in pulmonary shunt. It was observed that pulmonary shunt increased as the saturation of mixed venous blood became higher. It was clarified that the oxygen-receiving capacity of blood decreased as the oxygen saturation of mixed venous blood became higher. It was clarified that the oxygen-receiving capacity of blood decreased as the oxygen saturation became higher, i.e., the amount of reduced hemoglobin became less. The reduction in oxygen-receiving capacity of blood results in the reduction in oxygen-transfer efficiency from alveoli to blood and hence pulmonary shunt increases, and this can be regarded as a kind of shunt-like effect. It can be concluded that the change in pulmonary shunt by cardiac output is not due to the change in the lung function but is caused by the change in the oxygen-receiving capacity of the mixed venous blood.

An experimental study on the effect of plasma expanders on blood coagulability.

The effect of various plasma expanders on blood coagulation was studied using the thromboelastography (TEG). Solutions of 5, 10, 20, and 50% concentration of each expander (6% low molecular weight hydroxyethyl-starch (6% Hespander), 3% dextran-40, 6% high molecular weight hydroxyethyl-starch (6 HES), and 6% dextran-70) or solvent (lactated Ringer solution and normal saline) in blood were prepared and their coagulability was examined by TEG. In the low molecular weight plasma expanders (6% Hespander and 3% dextran-40), in general, the coagulability decreased when the concentration was increased. In the high molecular weight plasma expanders (6 HES and 6% dextran-70), the coagulability increased slightly at low and high concentrations and the coagulability was reduced. As a conclusion, at clinically used concentrations of less than 20% in blood, changes in the TEG of the plasma expanders are minimum and have no clinical significance.

Fundamental studies on electric stimulator used for acupuncture analgesia.

In order to perform safe and effective electric stimulation in acupuncture analgesia or therapy, basic knowledge concerning the electric stimulator is required. If direct current is used, a negative square wave with a pulse duration of 0.5 approximately 1.5 msec should be applied to the needle electrode to perform effective stimulation. Negative current is more effective at lower amperage than positive current and the possible corrosion hazard by the positive current can be avoided.

Acupuncture Therapy

The effects of acupuncture needling on the evoked responses of brain, spinal cord and muscle in man.

The effects of acupuncture (needling at the Stomach Meridian-36) on the somatosensory evoked responses from the brain (SER), from the spinalcord (EESG) and on the evoked electromy o-gram (EEMG) were studied in normal healthy man. Although amplitude of the early component of the SER was variable during the course of acupuncture and among the subjects, the inhibition of amplitude was seen during and after acupuncture needling. Late components of SER and the negative and positive waves of the EESG showed a high variability during the course of needling and among the subjects. The H-wave amplitude and H/M ratio were also variable, but decreased during and after the needling. The M-wave showed no significant changes. These results suggest that acupuncture reduced the excitability of the specific sensory pathway.

Acupuncture Therapy

Presynaptic inhibition in man during anesthesia and sleep.

The slow positive (P2) wave of the evoked electrospinogram was recorded from the dorsal epidural space in man. The waveform characteristics of the P2 wave were similar to those of the dorsal cord positive wave (P wave), which is believed to be caused by the primary afferent depolarization (PAD) and to be related to presynaptic inhibitory action in animals. The "second" component of the P2 wave appeared during excitement or following strong stimulation and disappeared after thiamylal administration and during natural slow-wave sleep. The second component, also demonstrated in the P wave of rabbits during ketamine anesthesia, was abolished by spinal transection. Therefore, these second components in man and rabbits may originate from a feedback loop via supraspinal structures. Thus, supraspinal influences might play an important role in the regulation of presynaptic inhibition in the spinal cord of man during wakefulness and anesthesia.
