Transmission of hepatitis C virus to sexual partners of seropositive patients with bleeding disorders: a rare event.

Sexual transmission of hepatitis C virus (HCV) was studied in 30 partners to anti-HCV positive multitransfused patients with a bleeding disorder. Anti-HCV ELISA C-100 was used as a screening test. Positive results were confirmed with the first generation RIBA test. Indeterminate samples were tested also with the second generation RIBA to verify the positivity. The time of sexual exposure added up was at least 95 years. 29 partners were anti-HCV seronegative. Only 1 partner was anti-HCV indeterminate. Thus sexual transmission of HCV was a rare event.

Treatment of idiopathic thrombocytopenic purpura in adults. Long-term results in a series of 41 patients.

The results of treatment of idiopathic thrombocytopenic purpura (ITP) were analysed in a series of 41 patients 2 to 10 years (median 4 years) after initial treatment. There was one spontaneous remission. A good or fair long-term result was obtained in 11 patients after prednisone treatment, in 20 of 24 patients after splenectomy, and in 3 of 4 patients after immunosuppressive treatment. Short duration of symptoms and rapid response to prednisone had some value in predicting a good long-term result of prednisone treatment. Young age, a good response to prednisone and splenic sequestration of platelets were associated with good results after splenectomy, as was rapid postoperative normalization of platelet counts. These factors were unable to disclose with certainty a refractoriness to splenectomy. Nevertheless splenectomy was regarded to be the treatment of choice in all patients not adequately controlled with corticosteroids.

Absorption of iron from slow-release and rapidly-disintegrating tablets - a comparative study in normal subjects, blood donors and subjects with iron deficiency anaemia.

The absorption of iron from slow-release and rapidly-disintegrating ferrous sulphate tablets has been compared using a double radioiron isotope technique. The studies were performed in 15 normal subjects, 20 blood donors and 10 patients with iron deficiency anaemia. The preparations containing 100 mg of ferrous iron were given twice daily on alternate days for 10 days. The absorption differed significantly between the three groups of subjects, being highest in the anaemic patients and lowest in the normal subjects. In all groups, significantly more iron was absorbed from the slow-release tablets compared to the rapidly-disintegrating tablets. The mean absorption ratios were 1.3 in both normal subjects and blood donors. In patients with iron deficiency anaemia the ratio was 1.4.

Hb Helsinki: a variant with a high oxygen affinity and a substitution at a 2,3-DPG binding site (beta82[EF6] Lys replaced by Met).

A new haemoglobin, Hb Helsinki, in which beta 82-Lys (EF6) is replaced by Met, was found in a Finnish family. It was associated with familial erythrocytosis, and the oxygen affinity of the blood was higher than normal. The oxygen equilibrium curves of purified Hb Helsinki and HbA from the same haemolysate have been determined under vaious conditions. "Stripped' Hb Helsinki was found to show normal cooperativity, slightly low oxygen affinity and a reduced Bohr effect at physiological pH. However, the organic phosphates, 2,3-diphosphoglycerate (2,3-DPG) and inositol hexaphosphate (IHP) had a very small effect on Hb Helsinki, and the 2,3-DPG binding constant of deoxygenated Hb Helsinki is close to that of oxyhaemoglobin A. Thus, the replacement of Lys by Met at position 82 dramatically changes the nature of the central cavity of the tetramer and the effect of 2,3-DPG on the respiratory function of the molecule.

Clinical correlations and significance of orceinpositivity in chronic active hepatitis and primary biliary cirrhosis.

Clinical, biochemical and immunological variables were analyzed in 30 patients with orcein-negative (ON) chronic active hepatitis (CAH), 4 patients with ON primary biliary cirrhosis (PBC), 8 patients with orcein-positive (OP; intracellular copper-binding protein seen histologically in liver biopsy specimens) CAH and 15 patients with OP-PBC. A marked elevation of serum bile acids, alkaline phosphatase, leusine aminopeptidase, gammaglutamyl transpeptidase and cholesterol concentrations, and highly pathological BSP Tm values were characteristic for OP-cases. In addition the faecal fat level was increased and bile acids decreased in OP-cases. Serum levels of IgG or IgM and the occurrence of smooth muscle, mitochondrial or glomerular antibodies were identical in ON- and OP-CAH as well as also in ON- and OP-PBC. 49 patients were treated with a combination of prednisone and azathioprine from 4 to 72 months (mean 22). 26 patients with ON-CAH responded biochemically and morphologically to the treatment. No treatment failures were found in ON-CAH. In contrast treatment failure was confirmed in every treated OP-PBC or OP-CAH. The results suggest that orcein-positivity indicates a poor response to prednisone-azathioprine treatment of CAH.
