Surgical management and prevention of vaginal vault prolapse.

From 1984 to 1988, we corrected and prevented the prolapse of the vaginal vault of patients who wanted to maintain a satisfactory sexual function. This was done by adopting two different surgical techniques. The first, generally favored technique, was performed upon 179 patients and consisted of a colposuspension to the sacrospinous ligaments. The second technique, a colposacropexis, was performed upon 71 patients, in which abdominal associated pathologic changes required an abdominal surgical approach. Colposacropexis was performed, whenever possible, directly to the anterior longitudinal vertebral ligament or using synthetic materials (Mersilene [polyester fiber], Teflon [polytetrafluoroethylene] and Gore-Tex [expanded, reinforced polytetrafluoroethylene]). Both colposacropexis and vaginal suspension to sacrospinous ligaments have had comparable results in vaginal accommodation and long term fixation. Vaginal approach has a lower incidence of operative complications than the abdominal approach and seems to ensure a lower risk of recurrent cystocele, even if simple and asymptomatic.

[Clinical effects of estriol administration by the vaginal route in physiologic and surgical menopause].

The therapeutic effect of estriol administered by the vaginal route on certain classes of symptoms relating to the climacteric has been assessed in 34 women. The response was generally very satisfactory both as regards neuropsychoendocrine disturbances and as regards urogenital dystrophic signs, in confirmation of the good absorption of the hormone and of its indifferent distribution in the target organs.

Hypogastric vein ligation during oncologic surgery as thromboembolic disease prevention.

Pulmonary embolism and vein thrombosis are frequent and dangerous complications in pelvic surgery. They can be the cause of death in 0.01%-0.87%. The Authors have used, since 1973, as vein thrombosis prophilaxis (VTP): early mobilization, physiotherapy, elastic compressive stocking plus pharmacologic therapy. Since 1975, according to Heidrich and Thomas researches, they have also adopted hypogastric vein ligation (HVL) in high risk patients, because those Authors underlined a primitive responsibility of the hypogastric district (mainly the left one). The Authors mortality rate after the introduction of HVL seemed very low. However, since Jan. '86, they have divided the high risk patients in 3 group (R), in order to collect statistically significant data in the usefulness of the proposed technique.

Effect of quinestrol on plasma and urinary gonadotropins of postmenopausal women.

Ten postmenopausal women were given a 4-mg load of quinestrol by mouth and plasma FSH and LH were serially determined at monthly intervals. By bioassay, total urinary gonadotropins were also measured at the same times. Plasma FSH and apparent total gonadotropin levels in the urine were depressed to very low levels up to 3 months, while plasma LH was slightly affected. Tentative explanations of this discrepancy are presented.
