Effect of uremic serum on 1,25-dihydroxyvitamin D3-induced differentiation of human promyelocytic leukemia cell, HL-60.

The mechanism by which resistance to 1,25-(OH)2D3 occurs in patients with chronic renal failure was studied. 1,25-(OH)2D3 causes the induction of differentiation and of 1,25-(OH)2D3-24-hydroxylase activity in the mitochondria of the human promyelocytic leukemia cell line, HL-60, via a steroid hormone-receptor mechanism. Treatment of these cells with 10(-8) M 1,25-(OH)2D3 for 5 days in a medium containing 10% uremic serum from 4 patients with chronic renal failure resulted in maturation of the cells amounting to 30.3 +/- 18.7 (mean +/- SD) and 32.5 +/- 11.2% maturation by the nitroblue tetrazolium reduction assay and the nonspecific esterase assay, respectively. These values were significantly lower than those obtained with 10% normal serum from 3 normal controls (66.6 +/- 12.8 and 58.3 +/- 10.9%, p < 0.02). The occurrence of resistance to 1,25-(OH)2D3 in uremic serum-treated cells was also confirmed when the effect of 1,25-(OH)2D3 was assessed by the induction of the cell's ability to hydroxylate the C-24 position of 1,25-(OH)2[3H]D3. Treatment of HL-60 cells with a mixture of 5% uremic plus 5% normal serum impaired 1,25-(OH)2D3-induced cell differentiation to the levels as those in 10% uremic serum, strongly suggesting the occurrence of a substance(s) having 1,25-(OH)2D3-inhibitory activity in the uremic serum. A significant reduction in 1,25-(OH)2D3 receptor levels was observed in uremic serum-treated cells.(ABSTRACT TRUNCATED AT 250 WORDS)

An adult case of Fanconi syndrome due to a mixture of Chinese crude drugs.

We examined a 35-year-old male case of acquired Fanconi syndrome induced by a mixture of Chinese crude drugs. Renal glycosuria, hypokalemia, hypophosphatemia, metabolic acidosis, a low threshold of tubular bicarbonate excretion, and generalized aminoaciduria were observed after the patient had taken the drugs for 6 months. When he stopped taking them, all laboratory data improved. He took the drugs again on his own judgment, leading to a second bout of Fanconi syndrome. This is the first case in which Chinese crude drugs have been known to cause acquired Fanconi syndrome.

Effect of transforming growth factor-beta on extracellular matrix production by cultured rat mesangial cells.

The effects of transforming growth factor-beta (TGF-beta) on the protein synthesis and gene expression of collagen I (CI), III (CIII), IV (CIV) and laminin (LM) in cultured rat mesangial cells (MCs) were examined to determine whether or not TGF-beta could modulate the formation of extracellular matrix (ECM) constituents, employing immunocytochemistry, enzyme-linked immunoadsorbent assay (ELISA) and Northern blotting. Although TGF-beta did not modulate MC proliferation, it induced slight enhancement of immunoperoxidase staining for CIV in the cytoplasm. ELISA demonstrated a significant increase in the amounts of CIV and LM, a tendency for CIII to increase, and a significantly lower amount of CI at 4 days after TGF-beta administration. Northern blotting showed that the mRNA expression displayed different patterns among the 4 matrix components during a 24-hr period. These findings suggest that a single growth factor, TGF-beta, in vitro modulates the synthesis of individual ECM constituents in MCs differently at the level of both genetic expression and protein production. The specific effects of TGF-beta on the increase in amounts of CIV and LM indicate that TGF-beta may contribute to glomerular remodeling and ECM accumulation in various kinds of glomerular injuries.

[A case of dextrocardia concomitant with tetralogy of Fallot, patent ductus arteriosus and bronchiectasia].

We report a case of dextrocardia concomitant with tetralogy of Fallot (TOF), patent ductus arteriosus (PDA), bronchiectasia, and pulmonary tuberculosis. A 24-years-old man without surgical operation for TOF, whose diagnosis had been made at infancy, was admitted to the Osaka City University Hospital because of massive hemoptysis. The diagnosis of pulmonary tuberculosis was made. He received anti-tuberculotic drugs and bronchial arterial embolization improved his symptoms. A case of mirror-image dextrocardia concomitant with TOF, PDA, and bronchiectasia, as seen in our case, has a low incidence rate, and, moreover, such a case concomitant with pulmonary tuberculosis is rarer still.

Extracellular matrix protein: gene expression and synthesis in cultured rat mesangial cells.

Production of extracellular matrix (ECM) by mesangial cells (MCs) contributes to progressive glomerulosclerosis. Our previous immunocytochemical studies on ECM production by MCs [1] were extended by examining the gene expression and protein synthesis using Northern blotting and ELISA. ELISA demonstrated that fetal calf serum (FCS) stimulated the synthesis of collagen I (CI), III (CIII) and IV (CIV), and laminin (LM). At and after confluence, the amounts of CIII, CIV and LM per DNA amount began to decrease, whereas that of CI did not. Northern blotting indicated that the mRNA expressions for CIV and LM were transiently increased by FCS. The mRNA expressions for CI and CIII decreased until confluence and then returned to the initial levels. These results suggest that FCS and cellular confluence affected the synthesis of ECM at both the level of gene expression and protein synthesis. The mRNA expressions for CIV and LM paralleled the protein synthesis and appeared tightly regulated. CI and CIII synthesis and mRNA expression were not coordinate and were not regulated in the same manner as the basement membrane matrix components.

Decreased bone mineral density in diabetic patients on hemodialysis.

Renal osteodystrophy in hemodialyzed patients with DM-HD shows different features from that in non-DM,HD. Two studies were done. One was a comparison of BMD in 30 non-DM,HD patients and 30 DM-HD patients. The second was a comparison of possible factors affecting calcium metabolism in the higher and lower BMD groups (n = 20/21) in the DM-HD patients. BMD was measured by dual-energy X-ray absorptiometry (DEXA; Hologic QDR 1,000/W) in the third lumbar vertebra (L3), head, pelvis, and whole body. The BMDs of the DM-HD group were lower in these areas and whole body than that in the non-DM,HD group. A significant difference was found in the head BMD (p less than 0.05). In the second study, factors which may contribute to the differences in BMD were compared in the DM-HD patients divided into higher and lower BMD of the head. The group with higher head BMD had a value 110% of the mean value or more. Clinical and biochemical test results (age, the time since the first dialysis, body weight, the degree of obesity, height, serum calcium, serum phosphate, serum aluminum, serum c-PTH level and the dose of 1 alpha-OH-D3) were compared. The degree of obesity of the patients with higher BMD was significantly larger than that with lower BMD (p less than 0.005).

Water loading tests both in supine and upright positions in three cases of idiopathic edema.

Three cases of idiopathic edema are reported with the results of water loading tests. Free water clearance and fractional sodium excretion revealed that three cases were different in water and sodium retention both in supine and upright positions. Aldosterone, plasma renin activity and antidiuretic hormone seemed to little to correspond to water and sodium retention; whereas atrial natriuretic peptide markedly increased in the upright position in all the cases. These data suggest that three cases cannot be explained by a single factor, but that enhanced reduction of venous return to the heart in the upright position may be a common feature in all three cases.

Formation of extracellular matrix by cultured rat mesangial cells.

Formation of extracellular matrix (ECM) by mesangial cells (MCs) contributes to progressive glomerulosclerosis. The authors investigated the production and distribution of ECM constituents by cultured rat MCs, using immunocytochemistry and immunoelectron microscopy. Staining for all ECM constituents increased after serum feeding. Localization was strictly intracellular until confluency, when extracellular deposition of collagen IV and laminin appeared, followed by fibronectin and collagen III. In parallel, the intracellular staining for these proteins diminished markedly. Neither extracellular deposition nor intracellular loss was observed for collagen I and thrombospondin. On surfaces coated with collagen IV or laminin, extracellular deposition of ECM constituents clearly preceded confluency. These results indicate that synthesis of ECM constituents parallels MC growth, and that extracellular deposition of ECM occurs at cell-cell contact. Collagen IV or laminin secreted by MCs in the substratum accelerates production and facilitates secretion of other ECM constituents in an autocrine fashion.

Acquired cutis laxa concomitant with nephrotic syndrome.

A 41-year-old woman developed laxity and wrinkling of the skin. This process spread slowly to involve the skin of almost the entire body, without any previous inflammatory skin disorder. The skin of her face, neck, and trunk hung in loose folds, leading to a prematurely aged appearance. One year later she noticed edema on her face and legs. Laboratory studies disclosed low C3 and CH50 serum levels and proteinuria. Skin biopsy specimens revealed extensive loss and fragmentation of dermal elastic fibers. A renal biopsy specimen showed membranoproliferative glomerulonephritis (type 2), and an immunofluorescent study disclosed C3 and IgG deposition in the mesangial matrix and along the glomerular basement membrane. We propose that, in this case, cutis laxa may be related to an abnormal immune response.

Systemic amyloidosis terminating in cardiac insufficiency.

An autopsy case of systemic amyloidosis is presented. The patient, a 57-year-old male, died of congestive heart failure. Autopsy revealed systemic amyloidosis which involved the heart, liver, spleen, kidneys, lymph nodes, tongue, prostate, rectum, and small blood vessels of various organs. In the heart, amyloid deposited diffusely in the myocardium, subendocardial tissue, and blood vessel walls. A lot of vacuolated cardiac muscle fibers were noted. Electron microscopic examination of the myocardium revealed that amyloid fibrils surrounded most of the cardiac muscle fibers in the very close vicinity of their basement membranes. Fashion of the amyloid deposition throughout various organs and the mechanism of heart failure are discussed.

Polyamines in 1 alpha, 25-dihydroxycholecalciferol-induced differentiation of human promyelocytic leukemia cells, HL-60.

The human promyelocytic leukemia cell line, HL-60, differentiated into macrophage/monocytes in the presence of 1 alpha,25-dihydroxycholecalciferol [1 alpha,25(OH)2D3], as assessed by the percentage of morphologically mature cells and their ability to reduce nitroblue tetrazolium. In this study of the mechanism involved, the activities of ornithine decarboxylase and spermidine/spermine-N1-acetyltransferase (SAT), the rate-limiting enzymes of polyamine metabolism, as well as the cellular levels of polyamine were measured. ODC activity reached a peak 24 h after the addition of 1 alpha,25(OH)2D3 and then decreased, while SAT activity gradually increased as differentiation commenced. An increase in putrescine and decreases in spermidine and spermine were also observed. Addition of alpha-difluoromethylornithine, an irreversible inhibitor of ODC, with or without methylglyoxalbis(guanylhydrazone), an inhibitor of S-adenosylmethionine decarboxylase, caused no effect on 1 alpha,25(OH)2D3-induced cell differentiation, although the cellular levels of putrescine and spermidine decreased markedly. Addition of alpha-difluoromethylornithine markedly suppressed cell proliferation; this effect was reversed by the addition of exogenous putrescine. Addition of exogenous spermidine or spermine to overcome activation of SAT also had no effect on 1 alpha,25(OH)2D3-induced cell differentiation. These results suggest both that polyamine metabolism is not important in 1 alpha,25(OH)2D3-induced differentiation of HL-60 cells, but that it is intimately involved in the proliferation of these cells.

Malignant chondroid syringoma. Report of a case with widespread metastasis and review of pertinent literature.

The authors report the autopsy case of a 73-year-old man with widespread, metastatic, malignant chondroid syringoma, with a long clinical history. Three years after resection of the primary tumor on the back, the tumor recurred at the same site, metastasized to the cervical lymph nodes 7 years later, and finally, after 13 years exhibited widespread metastases. Histologically, the tumor showed cords and nests of cuboidal or polygonal tumor cells with little cellular pleomorphism and few mitoses surrounded by a mucoid matrix. Histochemically, the matrix contained hyaluronic acid and sulfated acid mucopolysaccharides, such as chondroitin sulfate A and/or C. To the knowledge of the authors there have been nine reported cases of malignant chondroid syringoma in the English literature, four of which had distant metastasis. A review of the literature reveals that malignant chondroid syringoma is more common in women and occurs most often in trunk and extremities, which is in contrast to its benign counterpart, its histologic appearances vary greatly, and it may even have benign appearances.

Diabetes mellitus increases the severity of anemia in non-dialyzed patients with renal failure.

We investigated whether the presence of diabetes mellitus (DM) was related to the severity of the anemia observed in patients with renal failure who were not receiving dialysis. Forty patients were examined, (19 with long-term type II DM (DM-CRF), 21 with renal failure due to other causes (non-DM-CRF)). The two groups did not differ significantly as to age, sex, serum creatinine or erythropoietin. Hemoglobin was significantly (p < 0.005) lower in the DM-CRF patients (9.5 +/- 2.1 g/dl) than in the non-DM-CRF patients (11.2 +/- 2.0 g/dl). Multiple regression analysis indicated that higher serum creatinine levels and the presence of DM were independent risk factors for anemia (R2 = 0.494, p < 0.001). DM appeared to be a risk factor for the severity of anemia in patients with renal failure who were not receiving dialysis.
