Preoperative localization of lesions of the parathyroid gland using thallium-technetium scintiscanning.

Double tracer scanning of the neck after injection of thallium-210 and technetium pertechnetate was performed upon 33 patients with biochemically proved hyperparathyroidism operated upon during the past 14 months. Operative findings were correlated with the scans. Twenty-two of 29 single lesions of the parathyroid gland were successfully localized. There were ten unsuccessful studies, including three instances in which diffuse hyperplasia was incompletely identified. In one instance of diffuse hyperplasia, the bilateral pathologic finding was diagnosed correctly. Two of three substernal lesions were demonstrated in patients who had undergone previous unsuccessful explorations. An additional substernal lesion was localized in a patient prior to initial exploration. Nineteen of 21 lesions weighing more than 300 milligrams were identified, whereas only 50 per cent of the lesions weighing less than 300 milligrams could be seen. The scans demonstrated 12 of 14 lesions with C-terminal parathyroid hormone levels of more than 900 picograms per milliliter but only 60 per cent of the lesions that produced levels of less than 900 picograms per milliliter. Thallium-technetium scanning is a useful procedure for preoperative localization of parathyroid lesions and may preclude the need for more invasive testing in previously operated upon patients.

Measurement of effective renal plasma flow: a comparison of methods.

We have compared two in vitro methods and three variations of kidney background (BG) subtraction within a gamma camera method (41 examinations, 31 patients) for determination of effective renal plasma flow (ERPF) using 131I orthoiodohippurate (OIH). Method I: plasma samples at 20 and 45 min after OIH injection, ERPF = dose X slope/intercept; Method II: 45-min plasma sample, ERPF = -51.1 + 8.21x + 0.019x2, x = dose/45-min plasma activity/I. Individual kidney and total ERPF were determined from gamma camera (GC) methods using renal uptake 1-2 min after injection. All methods were compared against Method I (previously validated against paraaminohippurate (PAH) clearances). Method II, which requires one blood sample is more accurate than GC methods. GC methods are insensitive to operator variability in placement of renal and BG regions of interest. They may be useful to follow changes in relative or total ERPF, but accurate depth correction of renal data is suggested. In vitro, blood sample-based methods are more accurate.

Diagnosis of acute testicular torsion using radionuclide scanning.

We analyzed retrospectively 99mtechnetium scrotal scan findings in 37 patients with acute scrotal pain. Correlation between scan interpretation, and either surgical findings and/or final clinical diagnosis revealed an accuracy of 89 per cent. Only 1 false negative and 3 false positive results were recorded. Scan data revealed a positive predictive value of 75 per cent, negative predictive value of 96 per cent, sensitivity of 90 per cent and specificity of 89 per cent. An inter-institutional observer study also was conducted to assess reliability of the scan, resulting in an 88 per cent agreement. The testicular scan has been found to be an accurate and reliable method to distinguish between epididymo-orchitis and testicular torsion.

Thyroid uptake of 201thallium and its control by TSH.

Patients injected with 201Thallium (201Tl) for myocardial scanning present good thyroid visualization. Determinations in mice injected with 201Tl indicated a high thyroid/serum concentration ratio (T/S). The 201Tl biological half-life (t 1/2) in serum (30 - 135 s) was much shorter than in thyroid (53 - 55 h) for human subjects and experimental animals. The 1 h 201Tl T/S ratio was comparable to that of 131I and 99mTc, indicating presence of a gradient for 201Tl also. Increase of endogenous TSH induced by propylthiouracil led to a significant rise in in T/S for 99mTc, 131I and 201Tl, whereas TSH inhibition by feeding l-thyroxine led to decrease in T/S for 99mTc and 201Tl. In vitro thyroid/medium concentration ratio (T/M) of 99mTc and 201Tl was decreased after 20' incubation with ouabain, an inhibitor of the Na+, K+, activated ATP-ase. However, perchlorate in vitro or in vivo failed to diminish the 201Tl T/M ratios or to affect the t 1/2 of 201Tl in human subjects, whereas T/M of 201Tl was depressed by KCl addition to the medium.

Neurophysiological study of subacute combined degeneration.

Three patients with subacute combined degeneration of the spinal cord due to vitamin B12 deficiency showed a characteristic pattern of abnormalities on neurophysiological tests which corresponded to the clinical signs and symptoms. Peripheral nerve conduction studies showed an axonal degeneration peripheral neuropathy. Brain stem auditory evoked responses were normal; visual evoked responses were mildly abnormal and somatosensory evoked responses were moderately abnormal. These studies shed light on the pathology of this disorder and can aid in its diagnosis.

Spatial disorientation and the Dyke-Davidoff-Masson syndrome.

A patient with Dyke-Davidoff-Masson Syndrome had a lifelong history of spatial disorientation and visual-spatial cognitive defects demonstrated by psychological tests. We suggest that the abnormalities of behavior and test performance may be related atrophic lesions demonstrated by pneumoencephalography and computerized axial tomography. We consider several explanations to account for the lack of compensation for these cognitive defects.

Amaurosis and primary reticulum cell sarcoma of the brain: clinicopathologic study of a case.

A 63-year-old man developed headache, left hemiparesis, and progressive visual loss. Craniotomy demonstrated reticulum cell sarcoma within the right temporal lobe. Necropsy revealed extensive involvement of optic nerves by primary RCS. Autopsy suggested that local compression of optic nerves by tumor cell infiltrates may produce atrophy in addition to sustained, generalized elevation of intracranial pressure.

A dominant form of adult neuronal ceroidlipofuscinosis (Kufs' disease) with an associated occipital astrocytoma: early diagnosis by cortical biopsy.

A patient with a dominantly inherited form of Kuf's disease and an associated occipital astrocytoma is presented. This is the first reported case in which the diagnosis of Kufs' disease was made by a cortical biopsy several years before its expected clinical onset. The nosology of this disease, and its clinical, genetic, and histopathological characteristics are discussed. The establishment of an early diagnosis by cortical biopsy and its implications are considered.

Somatosensory evoked potentials in the neurological sequelae of treated vitamin B12 deficiency.

10 patients with chronic sequelae from vitamin B12 deficiency showed a characteristic profile of abnormalities of median and peroneal somatosensory evoked potentials: mild to moderately slow peripheral conduction to Erb's point, normal conduction or mild slowing across the cervical portion of the median somatosensory evoked potentials, and absence or severe slowing of impulse propagation along the spinal cord with the peroneal somatosensory evoked potentials. These findings are compared with those of other published series of somatosensory evoked potentials in vitamin B12 deficiency. These results shed light on the chronic physiological and anatomical residuals in the nervous system of patients treated for vitamin B12 deficiency.
