[Interdigestive gallbladder volume and and meal-induced gallbladder emptying in patients with diabetes].

The aim of the study was to find whether the type of diabetes and the duration of this disease causes any influence on gallbladder (GB) emptying, after exclusion of possible effect of obesity and a hyperglycaemia over 7 mmol/l (126 mg%) on this phenomenon. The investigations were carried out in 44 subjects with diabetes mellitus, among them 22 with type I and 22 with type II. These groups were divided according to disease duration: shorter or longer than 3 years. Two control groups (CI, CII) comprised each 20 healthy subjects. The age of CI and CII corresponded to the age of diabetes groups type I and II, respectively. All subgroups as well as the control groups were matched according to sex percentage structure, body mass index (BMI) and body surface (BS). In groups with type I and II diabetes, the number of cases with diabetic neuropathy was comparable. There was not found any significant (p > 0.05) influence of diabetes on the interdigestive GB volume, neither non-standardized nor standardized with regard to standard BS (1.73 m2). In comparison to the control groups, after a test meal, there was in all diabetic groups a statistically significant (0.001 > p < 0.05) reduction of GB emptying observed. This diminution was most expressed in the group with long-lasting type I diabetes.

Studies on immunomodulatory properties of isoniazid. Influence of isoniazid on responsiveness of peripheral blood mononuclear cells to interleukin-2.

Peripheral blood mononuclear cells which were "resting" i.e. interleukin-2-receptor-negative were stimulated to proliferate by interleukin-2 in vitro. It was found that isoniazid at concentration of 10(-5)-10(-6) M increased the peak proliferation of "resting" cells but had no influence on the kinetics of proliferation. The mechanism of this effect of isoniazid remains unclear.

Serum amino-terminal propeptide of type III procollagen in patients with hyperthyroidism or hypothyroidism during therapy.

Changes in serum amino-terminal type III procollagen propeptide (PIIIPN) and hydroxyproline were compared with indices of thyroid function in 33 patients with hyperthyroidism and 16 patients with hypothyroidism before and after treatment. Control values were obtained from 26 healthy individuals. Hyperthyroidism was shown to be associated with an increased level of PIIIPN and hypothyroidism with a decreased PIIIPN level. An increase in PIIIPN was found in 100% of hyperthyroid patients and a decrease in 31% of those with hypothyroidism, while hydroxyproline increased in only 54% of patients with hyperthyroidism and its level was decreased in only 6% of patients with hypothyroidism. Correlation between PIIIPN and thyroxine and triiodothyronine levels was shown. The results obtained suggest that PIIIPN is a valuable index of tissue response to thyroid hormones.

Influence of chronic mercury poisoning upon the connective tissue in rats. I. Effect of mercuric chloride on glycosaminoglycan levels in tissues, serum and urine.

Rats were intoxicated with mercuric chloride (1mg/kg b.w.) daily, for 12 weeks. A decrease in total glycosaminoglycan content was shown in the skin, the lungs, the liver and the heart muscle. These changes were accompanied by a slight alteration of the glycosaminoglycan pattern, a decrease in hyaluronic acid in the skin, the lungs and the heart muscle and an enhancement of heparan sulphate level in the kidneys. In serum of mercury-intoxicated rats, an increase in total glycosaminoglycan levels was shown. This enhancement was caused by elevation of almost all fractions. Urine output of glycosaminoglycans was higher in mercury-treated animals as compared to the controls.

Rats were intoxicated with mercuric chloride (1 mg/kg b.w.) daily, for 12 weeks. An increase of total collagen and elastin content was found in the skin, the lungs, the liver, the kidneys and the heart muscle. The increase resulted from the elevated level of soluble collagen. These changes were accompanied by elevated hydroxyproline level in serum and urine. It is concluded that chronic intoxication with mercury leads to disturbed composition of the connective tissue matrix.

[Effect of misoprostol, a synthetic prostaglandin E analog, on levels of serum C-peptide in serum and glucose tolerance].

The aim of the study was to investigate whether oral application of misoprostol (MI), which is an analogue of prostaglandin E1, does change the secretion of insulin and the blood glucose level. The investigations were carried out in 15 subjects, aged 21-40 yrs, with a gastric or duodenal ulcer and without any disturbances of the digestive tract motility. In 7 of them (group A), without stated diabetes a single oral dose of 400 mg MI versus placebo (PL) and 15 min later 75 g glucose p.o., was applied in randomized order, on different days, in fasting state. In 8 subjects with mild diabetes type II (gr B) MI versus PL and after 15 min. i.v. 1.0 mg glucagon was similarly administered. In both groups the concentrations of glucose and C-peptide in blood were determined. In comparison to PL, the application of MI did not cause any statistically significant differences of C-peptide in serum and blood glucose levels neither before and after oral glucose loading nor after i.v. administration of glucagon. Statistically not significant were also the differences in AUC (p > 0.05).

[Professor Jiri Syllaba: life and achievements of an honorary member of the Polish Society of Internal Medicine].

Professor Jiri Syllaba, an eminent Czech internist, was born on March 14, 1902 in Prague. He graduated from the Medical Faculty of the Charles University in 1926, and became an assistant at the Institute of Physiology in Prague. He carried out his research in Cambridge, London and Paris (1928-29). He was interested in blood gases and acid-base equilibrium. In 1935, he completed his habilitation thesis on cardiac output. After another period of research in Paris, he continued his work at the 2nd Department of Internal Medicine in Prague. During the Second World War, Professor Jiri Syllaba was arrested and imprisoned in the nazi camp. After war, he became the head of the 2nd Department of Internal Medicine at the Charles University. He worked at this post for 25 years, and further he worked in Diabetic Outpatient Clinic until 1992. Professor Jiri Syllaba is an author of 7 books, 250 papers and 80 congress abstracts. His research were focused on several aspects of internal medicine including diabetic coma and nephropathy. Professor Jiri Syllaba was granted the honorary membership of the Polish Society of Internal Medicine on October 16, 1970. Since early years of his professional carrier, he has being cooperating with Polish internists.

[Clinical and therapeutic aspects of brucellosis].

Brucellosis is an infectious zoonosis. It is a occupational disease of stock-breeders, workers of slaughter houses and veterinarians. Brucellosis is characterized by a variety of clinical symptoms and involves several organs of the body. Skeletal manifestations often facilitate diagnosis. Difficulties in diagnosis of brucellosis in Poland are resulted from an increased amount of low-symptomatic or asymptomatic cases, a decrease in acute brucellosis and mild course of chronic disease. Therapy is related to the stage of the disease, antibiotics are required in acute brucellosis. Symptomatic treatment with special emphasis to neurological and psychiatric management is used in patients with chronic brucellosis. Early made diagnosis is important because undiagnosed disease leads to severe disability and loss of work ability.
