Use of antipsychotic drugs in nursing homes: current compliance with OBRA regulations.

OBJECTIVE: To examine the degree and patterns of compliance with the Omnibus Budget Reconciliation Act (OBRA) regulations regarding the use of antipsychotic drugs in nursing homes. DESIGN: Retrospective chart review of all resident records. PARTICIPANTS: Eight nursing homes: five community, two county-owned, and one university-affiliated Veterans Administration facility. MEASUREMENTS: A structured assessment instrument to track compliance with each aspect of the OBRA regulations regarding antipsychotic drug use. RESULTS: A total of 1573 nursing home residents' pharmacy records were reviewed between August 1994 and March 1996. Two hundred seventy-nine residents were actively taking antipsychotic medications (prevalence = 17.7%). Mean compliance greater than 70% was found for (1) appropriate diagnostic indication (mean = 70.9%), (2) dosage within recommended limits (mean = 90.1%), and 3) documented appropriate target symptoms (mean = 90.4%). Dosages were more likely to exceed limits in those patients with histories of major mental illness, particularly schizophrenia. CONCLUSIONS: Nursing homes were better able to comply with those guidelines that are most specific. Educational interventions now need to focus on behavioral interventions, monitoring of adverse effects, and efficacy. These data are useful in establishing threshold levels of performance and can be used by nursing homes for continuous quality improvement. OBRA continues to impact neuroleptic drug prescribing practices in nursing homes significantly.

A silicate inclusion in Puente del Zacate, a IIIA iron meteorite.

The IIIA and IIIB iron meteorites are considered to have formed in the cores of asteroids. A silicate inclusion within the IIIA meteorite Puente del Zacate consisting of olivine (Fa4), low-calcium pyroxene (Fs6Wo1), chromium diopside (Fs3Wo47), plagioclase (An14Or4), graphite, troilite, chromite, daubreelite, and iron metal resembles inclusions in IAB iron meteorites. The oxygen isotopic composition of the Puente del Zacate inclusion is like chromite and phosphate inclusions in other IIIA and IIIB irons. The Puente del Zacate inclusion may have been derived from the lower mantle of the IIIAB parent asteroid.

Viability and morphology of aortic and pulmonary homografts. A comparative study.

In view of possible clinical use of the pulmonary homograft for right ventricular outflow tract reconstruction a comparative study with the aortic counterpart was performed. Samples of aortic and pulmonary walls from 10 cadaveric hearts were assessed for viability and morphologic characteristics before and after storage in nutrient-antibiotic solution. The viability, as evaluated by an autoradiographic technique, was similar in both aortic and pulmonary specimens at the time of dissection, after 2 weeks, and after 4 weeks of storage. The histologic examination showed no changes in the structure of the media in all samples up to 4 weeks of storage. The total calcium content per gram of tissue in the pulmonary media was on an average less than half of that in the aortic counterpart. We conclude that the pulmonary homograft is preserved the same as the aortic homograft and, accordingly, it becomes available for clinical application. Moreover, a lesser content of elastic tissue and a lower amount of total calcium may, in all likelihood, make the pulmonary wall less prone to calcification.

Leukotriene D4 relaxes canine renal and superior mesenteric arteries.

To characterize the influences of leukotriene D4 on regional vascular smooth muscle, effects of leukotriene D4 on vasomotor tone of canine renal and superior mesenteric arterial rings were determined. Vascular smooth muscle tone was measured with isometric force transducers. After tone had been induced with norepinephrine, leukotriene D4, in concentrations of 10(-8) M to 10(-7) M, produced dose-dependent relaxation of renal and superior mesenteric arterial rings. Leukotriene D4-induced relaxation was observed only in those ring preparations in which care had been taken to avoid damaging the luminal surface. Acetylcholine (10(-7) M) also decreased tone in these same ring segments. Neither acetylcholine nor leukotriene D4 altered tone of arterial rings after the endothelium had been intentionally disrupted by rubbing with a cotton-tipped applicator. Nitroglycerin (10(-6) M) relaxed rings both before and after rubbing the intimal surface. These results demonstrate that leukotriene D4 possesses the capacity to relax canine superior mesenteric and renal arterial rings in an endothelial-dependent manner. Because relaxation of renal and superior mesenteric arterial rings in response to leukotriene D4 was not altered after incubation with indomethacin (10(-5) M), the observed endothelial-dependent relaxation induced by leukotriene D4 did not appear to be related to release of a cyclooxygenase metabolite(s). In contrast, FPL 55712 (10(-5) M) attenuated the relaxation produced by leukotriene D4, suggesting that this response was a receptor-linked consequence.

Depression, physical health and somatic complaints in the elderly: a study of the Zung Self-Rating Depression Scale.

The relationships between depression, measured by the Zung Self-Rating Depression Scale (SDS), somatic symptoms based on self-reports, and health based on medical evaluations, were investigated in 60 depressed older persons (median age 64.5 years) in relatively good physical health. No relationship was found between health ratings and depression scores, but a significant association emerged between a Somatic Symptom subscale, specifically the single item of fatiguability, physicians' ratings of health, and depression scores. Results of factor analyses of the Zung Self-Rating Depression Scale were similar to those reported in previous studies and indicated a factor structure apparently based on the positioning of the positively and negatively worded items. It is suggested, therefore, that further investigations be carried out to evaluate the possible response bias on this scale.

Gerovital-H3: a clinical trial as an antidepressant.

Twenty-five volunteers, all in their fifth decade or beyond, all with mild to moderate, nonpsychotic depression of at least several months' duration participated in a double-blind study of Gerovital vs placebo. There was no significant difference between the Gerovital and placebo groups; both groups showed significant improvement on self-rating as well as observer rating scales.
