[Benefits and risks of surgical therapy of acute lateral fibular ligament rupture of the upper ankle joint. Results of a retrospective study].

In 60 patients with an intra-operatively confirmed first-time tear of the lateral ligaments of the ankle joint an examination carried out after a period ranging from 11.6 and 35 months post-op. showed that ligament suturing coupled with immobilisation for 6 weeks was not associated with any major or persistent impairment. The important advantage of the procedure was the possibility of accurately determining the extent of the injury, together with the chance of providing suitable and immediate correction. In 91% of the patient group, a comparison of sides revealed equal or greater stability of the surgically treated ankle joint. Even when occasional temporary, varyingly pronounced, sensory disorders are taken into account, in comparison with the possibilities of conservative therapy, we consider that these results confirm a tear of the lateral ligaments of the ankle to be a justifiable indication for surgery.

Stable patterns generated by activator-inhibitor systems.

The paper is concerned with a discrete morphogenetic model of activator-inhibitor type. The aim is to give a theoretical explanation for what we understand as the first step in pattern formation for a growing object: as long as the object remains small enough, its shape is spatially homogeneous, while passing a critical length results in a spontaneous establishment of a non-homogeneous configuration. We show that, for parameters suitably chosen, there are many stable patterns that can be formed. This fact implies, in particular, a sensitive dependence of the established pattern from the perturbation that causes the object to leave the homogeneous state.

[Scintigraphy using 111-indium-oxine-sulfate-labeled autologous granulocytes compared to scintigraphy using 99-Tc-pyrophosphate in inflammations of the jaw region].

The present paper compares scintigraphy using leucocytes labelled with 111 indium oxine sulphate with conventional 99Tc pyrophosphate scintigraphy for the investigation of inflammatory processes in the region of the jaw bones. It also tries to establish whether progress can be made in the field of differential diagnosis using a combination of these two methods of investigation. It is shown that conventional scintigraphy using 99Tc pyrophosphate represents an approved method of investigation, whereas scintigraphy using autologous leucocytes does not as yet fulfil the expectations that have been placed in it.

[Technic of radioactive labeling of autologous human thrombocytes using 111-indium-oxine and 111-indium-oxine-sulfate and their clinical use].

Using 111indium oxine and 111indium oxine sulphate as platelet labels, maximal labelling efficiency can be achieved already after 3 minutes at an incubation temperature of 37 degrees C. Mean labelling efficiency values of about 90% are reached at a platelet count of greater than 10(9) platelets/ml, but the labelling efficiency is satisfactory also at a rather low platelet count of about 10(6) platelets/ml (71% on average). Platelet labelling with these tracers allows the calculation of platelet half-life in vivo, and gamma-camera imaging of platelet aggregates in thrombosis and renal transplant rejection as well. In the present study, the platelet half-life is significantly shortened in patients with coronary heart disease (n = 15), peripheral vascular disease (n = 13) and primary hyperlipoproteinaemia (n = 32) in comparison with 106 controls. No age dependence of platelet half-life was found in the different groups of patients whereas in the control group, a significant negative correlation between age and platelet half-life was observed.

[Age dependency of platelet half life].

In the last few years many reports have been published concerning shortened platelet half-life in different diseases such as atherosclerosis and hematological disorders. Up to the present moment, however, there is no information available concerning age and sex dependency in healthy people. Therefore, we evaluated platelet half-life in 106 patients, who had been examined because of a suspected atherosclerotic or hematological disorder having been excluded in the sequel. In all the patients we found negative bicycle ergometry and negative sonography of the carotid arteries and the legs. Platelets were labelled with 100 muCi of 111Indium-oxine-sulfate at 37 degrees C for 5 minutes. The mean labelling efficiency was 90%, the recovery about 70% after two hours. We found a statistically significant correlation (r = -0,5395; p less than 0,001 between age and platelet half-life. In male patients we observed a trend towards shortened platelet half-life, however, the differences were not significant.

[Patterns of fetal heart frequency during the second stage of labour and expulsion period--typical patterns, frequency, risk of acidosis and evaluation (author's transl)].

Four patterns of fetal heart frequency with normocardia, two with tachycardia and three with bradycardia are described during the expulsion period. The most frequent patterns are normocardia with decelerations (48%), normocardia with decelerations and final bradycardia (21.1%) and basal bradycardia with decelerations (13.5%). The frequency of acidosis (pH of the umbilical artery less than or equal to 7,200) of these three patterns amounts to 8.3, 24.1 respectively 26.7%. The mean values of pHUA of normocardia with decelerations and final bradycardia, bradycardia, bradycardia with decelerations, tachycardia with decelerations and final bradycardia with prepathologic respectively pathologic Scores (Hammacher's Score, greater than or equal to five points) range from preacidosis to acidosis. Scoring of cardiotocograms seems to be of greater clinical significance than the measurement of deceleration areas.

Acidosis

[Epidemiology and diagnosis of vaginal discharge. First Report (author's transl)].

In 700 patients, 1336 tests of vaginal discharge were performed by culture, wet mount with the phase contrast microscope and the Papanicolaou stain. The diagnosis of yeast is best done by culture. Yeast was found only in 68% of the culture positive cases by the wet mount method and in 47% of the cases by the Papanicolaou stain. Wet mount and Papanicolaou stain results for yeast are therefore only useful if they are positive. In the diagnosis of trichomonas, culture and wet mount are of the same effeciency. The incidence of yeast and trichomonas was dependant upon the age of the patients. In pregnancy and in cases with diabetes, yeast was found more often but no increase of yeast was found in cases on oral contraception. The diagnosis of bacterial mixed infections poses the most difficult problem and can only be improved by expensive culture methods.
