Transcatheter embolization of pelvic vessels for control of intractable hemorrhage.

Transcatheter embolization for intractable hemorrhage from pelvic organs was performed in 46 patients. Advantages are discussed regarding different embolic materials, particle size, and embolization directed at branch vessels versus divisional arteries. An optimal approach for managing hemorrhage of different etiologies is developed, taking advantage of (a) selective embolization techniques, (b) pharmacologic manipulation of blood flow, and (c) the ability of embolic material to allow restitution of flow and/or continued collateral perfusion via the precapillary plexus.

Multidisciplinary approach to the diagnosis of hepatoblastoma: ultrasound, nuclear medicine, and angiography.

This report demonstrates the findings of angiographic, nuclear medicine, and ultrasonic examinations in a case of primary liver neoplasm (hepatoblastoma). The use of a multidisciplinary approach pin-pointed the neoplasm and guided a liver biopsy for subsequent tissue diagnosis. The patient is now undergoing radiation therapy, with monitoring by serial ultrasonic examinations.

Angiography in the diagnosis and staging of pelvic neoplasms.

The efficacy of arteriography in the staging of pelvic neoplasms was examined in 447 patients. It was found to be most useful in defining advanced neoplastic disease, improving accuracy of staging in these patients by about 18%. In patients with early pelvic neoplastic disease, arteriography reduced the number of false positives.

Computed radionuclide urogram for assessing acute renal failure.

The computed radionuclide urogram is advocated as a noninvasive diagnostic method for differentiation of the most common prerenal, renal, and postrenal causes of acute renal failure. On the basis of characteristic changes in the effective renal plasma flow rate, the calculated filtration fraction, and the calculated glomerular filtration rate, prerenal conditions such as renal artery stenosis or thrombosis, renal conditions such as acute rejection or acute tubular necrosis, and postrenal conditions such as obstruction or leakage, which are the most common causes of acute renal failure, can be differentiated. In conjunction with morphologic criteria derived from sonograms, a diagnosis with acceptable confidence can be rendered in most instances. Both the computed radionuclide urogram and sonogram are noninvasive and can be used without adverse effects in the presence of azotemia and even anuria. This also makes feasible reexamination at intervals to assess effect of therapy and offer prognostic information.

The management of urinary fistulas and strictures with percutaneous ureteral stent catheters.

A method is described for percutaneous anterograde introduction of a ureteral stent catheter. The experience and treatment of 5 patients are reported: 2 ureterovaginal fistulas, 2 ureterocutaneous fistulas and 1 ureteral stricture. The capability of the technique to place a stent catheter in the presence of an almost complete separation of the ureter or through ureteral segments deformed by extensive cicatricial changes is emphasized. The method is advocated as a most effective alternative to supravesical drainage in patients with ureteral fistulas and impaired healing potential of ureters devitalized by surgical stripping and/or radiation therapy.
