[Possibilities of chemotherapy in patients with bronchial cancer].

The introduction of chemotherapy in the treatment of small cell lung cancer made this a potentially curable disease. Despite intensive efforts, non-small cell lung cancer, however, remains refractory to chemotherapy. In patients with small cell lung cancer efforts are concentrating on increasing the cure rates. In patients with non-small lung cancer efforts are directed at achieving an optimal combination of palliative action and quality of life.

[Chemotherapy of non-small cell bronchial cancer with mitomycin C, vindesine and cisplatin. Results of a phase II study].

Forty-five patients (39 male, 6 female) with inoperable non-small-cell lung cancer were treated with combined mitomycin C, vindesine and cisplatin. All patients had measurable disease and had no previously received chemotherapy. The performance status of all patients was over 60%. Twenty-one patients had limited, 24 extensive disease. The overall remission rate was 53.3% (3 complete and 21 partial remissions) with a median remission duration of 9 months. Adeno- and squamous cell carcinoma responded to the chemotherapy with a remission rate of 63% (7 out of 11 patients) and 58% (14 out of 24 patients), but there were only 30% responders in large cell carcinoma (3 out of 10 patients). The median survival time for responders was 12 months, for those with no change and for patients with progressive disease 6 months. Myelosuppression and renal toxicity of the combination was generally not a treatment problem; subjective tolerance, however, (gastrointestinal upset) was poor.

[Hemoperfusion--a therapeutic alternative to early treatment of acute lindane poisoning].

Rapid lowering of serum concentrations to subtoxic values was seen in a 16-year-old girl with acute intoxication with the organochloroinsecticide gamma-hexachlorocyclohexane to subtoxic values using haemoperfusion over amberlite XAD-4. In vitro, an extraction coefficient of 0,99 for gamma-hexachlorocyclohexane using amberlite XAD-4 was determined. On the basis of this experience haemoperfusion is recommended as effective treatment in the early phase of acute gamma-hexachlorocyclohexane intoxication.

Mechanical and biochemical alterations in guinea-pig tracheae caused by sensitisation.

In order to test the hypothesis of beta-adrenergic receptor blockade in asthma bronchiale directly at the site of the asthmatic reaction, the tracheobronchial tree, we searched for mechanical and biochemical differences between isolated tracheae of actively sensitized and normal guinea pigs. The dose-response curves of terbutaline on isolated tracheae of actively sensitized guinea pigs were significantly shifted to the right in comparison with control trachea (ED50 11.8 X 10(-8) and 5.2 X 10(-8) M respectively). The sensitized organs dilated significantly slower than did controls (0.186 and 0.350 cm H2O/s, respectively) after addition of terbutaline, whereas the dilation velocity after addition of theophylline did not differ. The stimulation with fenoterol led to significantly lower amounts of cAMP in sensitized than in control tracheae (24.3 and 33.2 pmol/mg protein, respectively). The Km1 and Km2 of cAMP-phosphodiesterases of tracheal homogenates did not differ in both groups. Beta-adrenergic receptor binding studies with 3H-dihydro-alprenolol did not reveal a remarkable difference between sensitized and normal tracheae. The results indicate, that sensitization led to a partial blockade of the beta-adrenergic receptor adenylatecyclase system. However, the exact site of this effect remains obscure.

Multiple pulmonary (hamartomatous?) leiomyomas. Light and electron microscopic study.

The light and electron microscopical features of the lung tumors in a case of multiple pulmonary leiomyomas are described. The differential diagnosis of leiomyomatous tumors of the lung is discussed. They have to be differentiated from lymphangio-leiomyomatosis of the lungs. In the literature, multiple pulmonary leiomyomas are generally considered to be metastases from low grade uterine leiomyosarcoma or to be hamartomatous lung tumors. This is suggested by the glandular structures both within the tumor and on the surface. However, our ultrastructural observations showed these epithelia to have features of granular pneumocytes (type II), in particular they contain lamellar bodies and possess microvilli on their surface. Their formation is considered to be a secondary reaction of alveolar lining cells to tumor growth. A possible origin of multiple pulmonary leiomyomas from the contractile system of the lung acini (contractile interstitial cells) is discussed.

[Multiple pulmonary leiomyomatosis hamartoma (author's transl)].

The multiple pulmonary leiomyomatosis hamartoma (MPLH) is rare. Typical is the difference between the marked pulmonary alterations on chest X-ray film and the poor symptomatology. The (MPLH) is observed nearly only in women. The diagnosis is linked to pathological-histological findings.

Cyclic nucleotide metabolism in experimental bronchial constriction in rabbits.

In anesthetized, spontaneously breathing rabbits, intratracheally administered isotonic saline, acetylcholine (Ach) and histamine (His) induced an increase of airway resistance up to 183, 571, and 312%, respectively, compared with untreated controls. This was accompanied by a decrease of the arterial pO2. Bilateral cervical vagotomy led to nearly complete inhibition of the saline- and Ach-induced bronchoconstriction, whereas the His effect was only reduced to 202%. Vagotomy did not improve the arterial pO2. While the bronchoconstrictive effects of isotonic saline and Ach revealed a significant linear correlation with the increased cGMP/cAMP ratio, His did not cause an alteration of the cGMP/cAMP ratio to a degree that corresponds to its bronchoconstrictive activity. These data indicate that (1) bronchoconstriction-inducing agents may act (a) by stimulation of the parasympathetic nerve or (b) by contracting smooth muscles directly; (2) alveolar ventilation is not regulated by the parasympathetic nerve, and (3) bronchoconstriction induced by parasympathetic stimulation is associated with or mediated by an increase of the cGMP/cAMP ratio.
