Possible involvement of protein kinase C in the aberrant regulation of erythropoiesis in polycythemia vera.

To examine the possible involvement of protein kinase C (PKC) in the regulation of aberrant erythropoiesis of polycythemia vera (PV), we investigated the effects of PKC inhibitors on in vitro burst-forming unit of erythroid (BFU-E)-derived colony formation by bone marrow (BM) and peripheral blood (PB) cells obtained from five PV patients. 1-(Isoquinoline-sulfonyl)-2-methylpiperazine dihydrochloride (H-7), an inhibitor of PKC, suppressed the colony formation by BM and PB cells of PV patients in a dose-dependent manner, similar to those in the normal individuals. However, the 50% inhibitory concentrations (IC50) of H-7 in PV BM and PB cells were significantly higher than those in normal BM and PB cells, respectively. The BFU-E-derived colony formation by PV BM and PB cells was also less affected by Staurosporine, another PKC inhibitor, than those in a normal subject. Furthermore, in the study of PV, the IC50 of endogenous colonies formed in the absence of erythropoietin was much higher than that of colonies formed by the stimulation of erythropoietin. By contrast, N-(2-guanidinoethyl)-5-isoquinolinesulfonamide dihydrochloride (HA1004), a cyclic AMP-dependent kinase inhibitor, did not have such inhibitory effects. These findings suggest that PKC, as a second messenger, is involved in the regulation of aberrant erythropoiesis of PV.

Transient sick sinus syndrome associated with immunoblastic lymphadenopathy.

We describe a very rare case of immunoblastic lymphadenopathy (IBL) accompanied with a sick sinus syndrome (SSS) in a 64-year-old male. SSS presented with progression of the IBL and subsided with improvement of the IBL following high dose prednisolone (PSL) therapy. The clinical course of this patient strongly suggested cardiac involvement in IBL and being the cause of the transient SSS. To our knowledge, this is the first case of IBL demonstrating SSS as a possible complication of the disease. This case may provide further information for the treatment of similar patients with IBL.

[A case of adult idiopathic thrombocytopenic purpura associated with atypical mumps virus infection].

A 30-year-old male admitted to out hospital complaing of petechiae. He had attack of fever 2 days before admission, without parotitis and orchitis. Laboratory data showed marked thrombocytopenia. Bone marrow showed normocellularity with an increase of megakaryocytes. Antimumps IgM was positive by the EIA. He was diagnosed as idiopathic thrombocytopenic purpura (ITP) associated with mumps virus infection. After the administration of oral prednisolone and 5 days-infusion of gamma-globulin, platelet count increased rapidly. Prednisolone discontinued within 35 days, as in acute ITP, and he maintained remission. In conclusion, the test for antiviral antibodies is indispensable to exclude acute ITP in adult ITP patients.

Surface reaction between titanium castings and investments.

The selection of investments suitable for titanium castings is critical because the surface-reacted layer greatly affects their mechanical properties. This study analyzed the interfacial zone of titanium castings obtained from various commercial investments and examined the relationship between the surface-reacted layer and the hardness. CP titanium disks (20 mm diameter, 1 mm thick, n = 5) were cast using eight commercial casting systems and investments for titanium casting: two alumina-based investments (T-invest, CD-titan investment), two spinel-based investments (Titanmold, Titavest MZ), three zirconia-based investments (Asahivest-D, Titaniumvest EX, Tycast investment TM), and a magnesia-based investment (Selevest CB). The Vickers hardness of cast disks in cross-section was tested at 50 microns intervals from the surface to 0.5 mm in depth. The cross-section was observed metallurgically, and an elemental mapping measurement of the reacted layer was conducted under the EPMA. The Vickers hardnesses at 50 microns from the surface of the specimens ranged 312 to 624 and were significantly different among the investments (p < 0.01). The thickness of the reacted layer, rich in O, Si, Al, Zr, Mg, and P, ranged from 10 to 60 microns. No correlation was seen between the thickness of reacted layer and the hardness (p > 0.05). However, the specimens cast with alumina-based and zirconia-based investments were harder than the spinel-based and magnesia-based investments at 500 microns. Although the free energies of metal oxide formation of alumina and zirconia were lower than titania, these data suggest that silica, alumina and zirconia were reduced by contact with molten titanium. Accordingly, silicon, aluminum and zirconium diffused to the interior of the titanium castings and changed their mechanical properties.

Cyclic haemopoiesis at 7- or 8-day intervals.

We report a patient with severe anaemia and cyclic oscillations of reticulocyte and leucocyte counts, as well as serum iron (Fe), unsaturated iron-binding capacity (UIBC), ferritin, C-reactive protein (CRP) levels and temperature, at regular intervals of 7 or 8 d. After treatment with prednisolone, anaemia was corrected and the cyclic oscillations of these parameters ceased; whereas treatment with indomethacin, recombinant granulocyte-colony stimulating factor (G-CSF) and erythropoietin (Epo) were unsuccessful.

Decrease in serum ferritin level in a patient with HCV hepatitis and liver hemosiderosis by interferon-alpha.

We describe here a patient with chronic hepatitis C and liver hemosiderosis whose serum ferritin level was notably reduced by long-term interferon-alpha (IFN alpha) therapy. The decrease of the elevated serum ferritin level was considered to have been mostly obtained by the improvement of liver dysfunction. However, at the beginning of the therapy, in spite of alanine aminotransferase (ALT) improvement, his serum ferritin level increased transiently, and after cessation of IFN alpha therapy, the serum ALT increased again, but the serum ferritin had not increased. This indicates that IFN alpha has an effect on the iron-related measurement, partly due to improvement of hepatic status.

[Effect of combination therapy with sulbactam/cefoperazone and amikacin in the treatment of infectious diseases accompanied by hematological disorders].

We studied the efficacy and safety of a combination therapy with sulbactam/cefoperazone (SBT/CPZ) and amikacin (AMK) on severe infections in the patients with hematological disorders. We administered SBT/CPZ and AMK to 65 patients and analyzed the result of 57 patients, except for 8 drop outs, for evaluation. The overall clinical efficacy of this combination therapy was 64.9% (excellent in 15 patients, good in 22 patients). Thirty seven patients had sepsis and suspected sepsis. The efficacy was 67.6% in these patients. The rate of improvement in the cases (18 patients) with neutrophil counts under 500/mm3 during the administration among 28 patients who had not responded to prior antibiotic treatment was 67.9%. Diarrhea was found in 1 patient as an apparent side effect likely related to this drug regimen. It appears that combination therapy of SBT/CPZ and AMK was a useful regimen in the treatment of infectious diseases complicated with hematological disorders.

A transient rise in plasma beta-endorphin after a traditional 47 degrees C hot-spring bath in Kusatsu-spa, Japan.

To clarify the mechanism of the intoxicating feeling attained after a traditional 47 degrees C hot-spring bath called 'jikan-yu' in Kusatsu-spa, Japan, we examined the change in plasma levels of beta-endorphin and methionine enkephalin in 7 healthy subjects. The mean sublingual temperature rose from 36.8 degrees C to 38.6 degrees C and the plasma beta-endorphin level from 16.2 pg/ml to 49.5 pg/ml 2 minutes after completing a 3-minute bath in 47 degrees C hot-spring water. However, the plasma methionine enkephalin level was not changed. This feeling of intoxication may be explained by the transient rise in plasma beta-endorphin level.

Complete cure of urinary and faecal incontinence after intravenous vitamin B12 therapy in a patient with post-gastrectomy megaloblastic anaemia.

The case of a 59-year-old Japanese woman with post-gastrectomy megaloblastic anaemia having urinary and faecal incontinence and paraesthesia in four extremities is described. While the haematological abnormalities were improved by administration of a total dose of 17 mg of intramuscular mecobalamin, the neurological abnormalities remained unchanged. Five months later, a total dose of 7.5 mg of mecobalamin was injected intravenously over a period of 5 weeks, although the serum level of vitamin B12 was greater than 1180 pmol l-1. Immediately after initiation of the therapy, the urinary and faecal incontinence were gradually improved, and were completely cured within 2 months. The peripheral neuropathy was also ameliorated. The effectiveness of intravenous vitamin B12 injection for the neurological abnormalities due to vitamin B12 deficiency is emphasized.

Effects of long-term running on lymphocyte subpopulations.

Thirteen healthy members (11 males and 2 females; mean age 19.8 years) of an athletic club underwent a long-term exercise program (five 8-km runnings per week for 40 weeks) as regular training for their club. The circulating lymphocyte number, T lymphocyte percentage, and OKT4/OKT8 ratio did not change through the 40-week training period. However, suppressor/inducer (Leu3a+Leu8+) and suppressor T cells (Leu2a+Leu15+) significantly increased. In contrast, helper (Leu3a+Leu8-) and cytotoxic T cells (Leu2a+Leu15-) significantly decreased. On the other hand, there was no change in three males (mean age 18.7 years) without the long-term exercise program. These results suggest that long-term running may induce some change in lymphocyte subpopulations.

Restoration of abnormally high CD4/CD8 ratio and low natural killer cell activity by vitamin B12 therapy in a patient with post-gastrectomy megaloblastic anemia.

A patient with post-gastrectomy megaloblastic anemia is described. Initially, the CD4/CD8 ratio of blood cells was abnormally high and the level of natural killer cell activity was markedly low. These abnormalities were, however, restored by vitamin B12 treatment. Taken together with our prior report on a patient with pernicious anemia in whom the decreased level of CD8-positive blood cells was corrected by vitamin B12 therapy, it may be considered that the reversible CD4/CD8 imbalance of blood cells noted in some cases of pernicious anemia is not pathognomonic for the disease but secondary to vitamin B12 deficiency.

A long-term survivor with malignant thymoma accompanied with pure red cell aplasia.

A 56-year-old woman with malignant thymoma lived for 19 years after diagnosis and died of superior vena cava syndrome. An autopsy showed invasion of the thymoma to the right lung and metastasis to the left lung. The pathologic diagnosis was epithelial thymoma (spindle cell dominant) accompanied with pure red cell aplasia (PRCA). However, this was well controlled with steroid therapy. She was the longest survivor with malignant thymoma at this time. Her long-term survival may be the result of good control of the PRCA.

Nonsecretion of myeloma protein in spite of an increase in tumor burden by chemotherapy.

A unique case of IgA kappa myeloma is presented. While the myeloma cells had secreted a large quantity of IgA kappa monoclonal protein, they were induced to stop secreting the monoclonal protein by cyclophosphamide and vincristine, in spite of a remarkable increase in tumor burden. The absence of intracytoplasmic IgA kappa was clearly evidenced by the immunofluorescence technique using anti-IgA and anti-kappa monoclonal antibodies.

Antineoplastic Combined Chemotherapy Protocols

[Cerebral infarction and high serum levels of muscle-derived enzymes associated with abrupt increase in hematocrit in a patient with secondary erythrocytosis].

A 70-year-old man was admitted to our hospital because of fever and progressive dyspnea in December 1989. He was already diagnosed as having erythrocytosis secondary to pulmonary fibrosis 4 years previously and the values of his hematocrit (Ht) were maintained between 44.5 and 62.9% by repeated phlebotomy. Immediately after admission, severe diarrhea developed and the Ht value was 61.5%. Around 1:30 a.m. of the 3rd hospital day, he developed disturbance of consciousness. In addition, the serum levels of LDH, CPK, aldolase, and myoglobin of muscle origin increased markedly and the Ht value showed 78.5%. While the level of consciousness was gradually restored by 600 ml phlebotomy and 1,500 ml saline infusion, dysarthria and hemiplegia became evident. The Ht value early in the morning of the 3rd hospital day was reduced to 59.4%. Although cranial CT and MRI performed 74 days and 15 months, respectively, after the onset of the symptoms failed to reveal any abnormal shadow, he was clinically suspected to have cerebral infarction. These findings emphasize that abrupt increase in Ht or blood viscosity is a possible factor triggering cerebral infarction, and adequate control of Ht value is recommended for the prevention of such a condition in the aged.

Relief of severe diabetic truncal pain with mexiletine.

A 60 year-old Japanese man having a 20-year history of diabetes mellitus presented with truncal neuropathy. The severe pain, especially its nocturnal exacerbation, prevented him from sleeping and eating, resulting in rapid emaciation. However, he recovered from the condition with oral mexiletine, which dramatically controlled the pain.
