Personality disorder symptoms predict declines in global functioning and quality of life in elderly depressed patients.

The authors evaluated personality disorder symptoms as predictors of change in global functioning and quality of life among elderly depressed patients. Treated elderly patients (N=40) who no longer met RDC criteria for major depression were assessed for personality disorders, depression, global functioning, and quality of life after treatment of the acute episode and at 1-year follow-up. In interaction with persisting or recurrent depression, Cluster B personality disorder symptoms contributed to declines in global functioning and quality of life over a 1-year period. Personality disorder symptoms in elderly patients appear to operate as co-factors that amplify or exacerbate the impact of residual depression on long-term functioning and quality of life.

Personality disorder symptoms and functioning in elderly depressed patients.

The authors evaluated the relationship of personality disorder symptoms to disability and social and interpersonal functioning in geriatric depression. Measures of personality disorder and cognitive, affective, social, interpersonal, medical, socioeconomic factors, and instrumental activities of daily living (IADL) status were administered to 47 elderly patients at various levels of remission from major depression. Total personality disorder scores were inversely associated with IADL, sociability, and presence of a satisfying relationship, both alone and in interaction with depression. The associations between personality disorder and functioning were most prominent in subjects with low residual depression. Symptoms of personality disorder in elderly patients may be associated with disability and impaired social and interpersonal functioning after an acute depressive episode; personality disorder symptoms may also have treatment implications for geriatric depression.

Disability in geriatric depression.

OBJECTIVE: The authors' purpose was to identify the relationship of disability to clinical measures that are part of a comprehensive psychiatric examination of depressed elderly patients. METHOD: The disability of 75 elderly inpatients and outpatients with major depression whose cognitive function ranged from normality to mild dementia was assessed with the Philadelphia Multilevel Assessment Instrument. Age at onset of depression, chronicity of depression, severity of depression, cognitive impairment, medical burden, social support and living environment were assessed with standardized instruments. RESULTS: Impairment in instrumental activities of daily living was significantly associated with advanced age, severity of depression, and medical burden. The relationship of depressive symptoms to impairment in instrumental activities of daily living was not influenced by age or medical burden. Anxiety and depressive ideation as well as retardation and weight loss were significantly associated with impairment in instrumental activities of daily living. Interviewer-rated global disability was associated with advanced age at onset of depression, medical burden, and overall cognitive impairment. Specifically, a disturbance in initiation and perseveration was significantly related to global disability. CONCLUSIONS: Impairment in instrumental activities of daily living appears to be a relatively independent dimension of health status that is related to depressive symptoms, particularly anxiety and depressive ideation as well as retardation and weight loss. Global disability may be associated with impairment in initiation and perseveration and with late onset of depression. These findings provide a basis for studies investigating whether psychotherapy aimed at depressive ideation and rehabilitation efforts focused on instrumental activities of daily living can improve the outcome of geriatric depression.

Measurements of size distribution and density of a pharmaceutical fat emulsion, using field-programmed sedimentation field-flow fractionation (SdFFF).

PURPOSE: The main goal was to establish that sedimentation field-flow fractionation (SdFFF), operated with power based field programming, is effective in the characterization of a commercial emulsion, Medialipide. This emulsion is used clinically for total parenteral nutrition and it is consisted of a mixture of long-chain triglycerides (LCT, soybean oil) with medium-chain triglycerides (MCT) emulsified by phospholipids. METHODS: Different field programming methods were used in the analysis to establish the limits of applicability of the technique. RESULTS: Identical size distribution profiles were obtained under various conditions of the analysis. The density of the droplets was determined by collecting fractions from the SdFFF eluting bands, and analyzing them by photon correlation spectroscopy. The value of density of the oil droplets was changed in the SdFFF data, until best agreement with the PCS values was achieved. The value of density corresponding to the best agreement was considered as the oil density, and it was closed to the weighted average value between soybean and MCT oils. CONCLUSIONS: Field programming extends the capabilities of sedimentation field-flow fractionation in handling and characterizing complex and delicate samples as Medialipide.

Characterization of submicron MCT o/w emulsions using sedimentation field-flow fractionation (FFF) with power field programming.

Sedimentation field-flow fractionation (SdFFF) operated with power-based field programming, was shown to be effective in the characterization of submicron investigational pharmaceutical emulsions. Field programming, in which the decrease of field strength with time gradually decreases the retention of sample components, extends the capabilities of sedimentation field flow fractionation in handling polydisperse and multicomponent samples. The emulsions were made of medium chain triglycerides (MCT) oil in water emulsified by phospholipids. They were analysed by different rates of field decay and different flow rates. Identical size distribution profiles were obtained under all circumstances, using the appropriate stop-flow times. Fractions were collected from the SdFFF eluting bands, and diameters were analysed by photon correlation spectroscopy, showing good agreement with values given by the FFF instrument at high flow rates and low rates of field decay. Accurate and highly reproducible size distribution profiles were obtained under various conditions. The detector response was shown to consist mostly of light scattering and was linear with concentration.

Expressed emotion and glucose control in insulin-dependent diabetes mellitus.

To clarify the meaning of the expressed emotion construct by testing its generalizability beyond psychiatric disorders, the authors measured family expressed emotion and glucose control in 35 diabetic patients. The critical comments component of expressed emotion significantly predicted glucose control, suggesting that the expressed emotion construct is not specific to psychiatric patients.
