Cost accounting in healthcare organizations: who needs it?

There is no question that improved cost information can lead to improved accountability in health care; however, the major proponents of borrowing cost accounting concepts from manufacturing have not gone far enough. They have ignored an important aspect of manufacturing cost accounting that is also a very important prt of hospitals and health care, namely the concern for quality.

The early effects of the prospective payment system on inpatient utilization and the quality of care.

Concern is often expressed that hospitals may be making cost/quality trade-offs under Medicare's prospective payment system. To evaluate the early effects of PPS on inpatient utilization and quality of care, we studied a cohort of 729 U.S. short-term general hospitals from nonwaivered states. We used a linear forecasting model to project 1984 figures based on trends from 1980 to 1983. We found no evidence that the quality of care deteriorated in 1984 for Medicare patients. Consultation rates remained constant, and in-hospital deaths and readmission rates were consistent with previous trends for the Medicare population. Overall, it appears that PPS has reduced hospital utilization without producing deterioration in the quality of care.

Two types of spontaneous contractions in rat myocardium and their possible relation to different Ca release processes.

The potency of putative blockers (lidocaine, dantrolene Na, DAN, SKF 525A) for various forms of Ca2+ release from sarcoplasmic reticulum (SR) against two types of spontaneous contractions induced by 0.5 mM caffeine (caffeine response) or myocardium Ca2+ overload (after-contractions) was tested in rat papillary muscle. There were differences in the sensitivity of spontaneous contractions to the agents employed. The caffeine response was depressed by DAN but not affected by lidocaine. Conversely, conventional diastolic after-contractions were eliminated by lidocaine and SKF 525A but DAN failed to affect them. The two types of spontaneous contractions were suggested to be triggered by different CA2+ release processes supposedly originating from different SR compartments.
