Palpebral myiasis in a Danish traveler caused by the human bot-fly (Dermatobia hominis).

PURPOSE: To demonstrate a case of palpebral myiasis caused by Dermatobia hominis in a Dane traveling in the Brazilian rain forest. METHODS: Surgical removal and histological examination. RESULTS: Within three weeks after returning from the Brazilian rain forest, the patient developed a localized swelling of the left upper eyelid and follicular conjunctivitis. A funnel containing a larva was found between the cilia. The larva was excised together with surrounding inflammatory tissue. The clinical findings as well as zoological and pathological examinations indicated a case of palpebral myiasis caused by Dermatobia hominis. The larva was removed in its third stage four weeks from symptom debut. CONCLUSIONS: Infestation with Dermatobia hominis should be suspected when itching and red swelling of the lid are present in patients who have been to Central and South America.

Retinal detachment after cataract extraction in myopic eyes.

PURPOSE: To determine the incidence of retinal detachment (RD) after cataract extraction in people 40 years of age or older with axial myopia (i.e., axial length > or = 25.5 mm). SETTING: Fifteen Danish eye clinics. METHODS: Two hundred forty-five eyes had cataract extraction performed at 15 eye clinics; 237 eyes had extracapsular cataract extraction (ECCE) and 8 eyes, intracapsular cataract extraction (ICCE). Postoperative data were reported by the practicing ophthalmologists. Mean follow-up was 27 months (range 14 to 32 months). RESULTS: Five RDs occurred in the 245 eyes (2.0%). Excluding the ICCE cases and the two cases of combined cornea transplantation and ECCE, RD occurred in 4 of the 235 eyes that had ECCE (1.7%). The incidence after ECCE with posterior chamber lens implantation was 1.4%. Complete postoperative status was reported on 158 eyes. Forty-eight eyes (30.4%) had a neodymium:YAG capsulotomy and 3 (6.0%) developed an RD 1, 3.5, and 21 months after the capsulotomy. CONCLUSION: The RD incidence after ECCE with posterior chamber lens implantation was low but higher than that in unselected populations. The incidence increased after laser capsulotomy.

Histopathological findings in failed human epikeratophakia lenticules.

Six human epikeratophakia lenticules were removed 1 to 23 months after refractive keratoplasty for aphakia (n = 2) and myopia (n = 4). The myopic group included one epi-lenticule treated with ArF-excimer laser for hyperopia. Reasons for removal were epithelial defect (n = 4), edema of host cornea (n = 1), and overcorrection (n = 1). The epi-lenticule stroma showed centrally a regular fibrillar structure with a low keratocyte density. Electron microscopy (n = 1) revealed preserved cytoplasmic organelles and nuclear appearance. Mononuclear cells were absent. Polysaccharide was demonstrated in the stroma. Bowman's layer was normal without breaks. The epithelium had variable thickness. In the peripheral wound region stromal and epithelial irregularities were evident.

Analyzing the red-shift characteristics of azulenic, naphthyl, other ring-fused and retinyl pigment analogs of bacteriorhodopsin.

Prompted by the near infrared-absorbing properties of some of the azulenic bacteriorhodopsin (bR) analogs, we have analyzed their absorption characteristics along with 11 new related ring-fused analogs and the corresponding Schiff bases (SB) and protonated Schiff bases (PSB). The following three factors are believed to contribute to the total red shift of each of the pigment analogs (sigma RS): perturbation of the basic chromophore (SB shift, delta SB), protonation of the SB (PSB shift, PSBS) and protein perturbation (the opsin shift, OS). For each factor, effects of structural modifications were examined. For the red-shifted pigments, percent OS has been suggested as an alternate way of measuring protein perturbation. Computer-simulated chromophores provided evidence against any explanation involving altered shapes of the binding pocket as a major cause for absorption differences. Implications of the current bR results on preparation of further red-shifted bR and possible application to visual pigment analogs are discussed.

Natural course in fellow eyes of patients with unilateral age-related exudative maculopathy. A fluorescein angiographic 4-year follow-up of 45 patients.

A prospective fluorescein angiographic study of 45 patients with unilateral age-related exudative maculopathy revealed at the 4-year follow-up that 14 (31%) had developed neovascular membranes in the fellow eyes. Age, drusen size and confluence, as well as pigmentary changes of these 14 were compared with the corresponding characteristics of the 31 eyes that remained non-exudative (atrophic). No significant differences were found. Among the 14 eyes that developed exudative maculopathy, visual acuity (Snellen) deteriorated 3-4 steps in 6 eyes (43%), 5-6 steps in 4 eyes (28.5%), and 7-8 steps or more in 4 eyes (28.5%). Among the 31 eyes that remained non-exudative, visual acuity was unchanged in 20 eyes (64%), deteriorated 1-2 steps in 7 eyes (23%), 3-4 steps in 3 eyes (10%), and 5-6 steps in 1 eye (3%). Social blindness (6/60 or less in the better eye) was found among 79% of the patients who had developed exudative maculopathy, whereas the figure was 19% among patients who still had atrophic macular degeneration.

Sjögren's syndrome in patients with multiple sclerosis.

Twenty female patients with clinically multiple sclerosis were evaluated with regard to a possible underlying Sjögren syndrome. Two patients (10%) demonstrated a clinical Sjögren syndrome which does not differ significantly from the prevalence in an unselected population. Dry eye-symptoms occurred among MS-patients and should be subjected to relevant diagnostic procedures.

Topical indometacin, a prostaglandin inhibitor, in acute anterior uveitis. A controlled clinical trial of non-steroid versus steroid anti-inflammatory treatment.

Forty-nine patients were included in a controlled clinical trial comparing the effect of topical non-steroid versus potent steroid preparation in acute anterior non-granulomatous uveitis. Twenty-five patients were randomized to 1% Indometacin and 24 patients to 0.1% Dexametason treatment 6 times a day. There was no difference between the two groups initially. After 7 days of treatment we found significantly less inflammation in the steroid treated group, this significant difference disappearing on day 14. We conclude that whenever adverse reactions to corticosteroid eye drops are suspected or proven, Indometacin eye drops make up a useful alternative to even strong solutions of steroids. Predictive value of Lang two-pencil test, TNO stereotest, and Bagolini glasses. Orthoptic examination of an adult group.

In our comparative study of a Rodenstock sight-screener and a clinical eye examination, an orthoptic examination was included. The present study describes the orthoptic examination of 109 subjects and evaluates the predictive diagnostic value of Lang two-pencil test, TNO stereotest and Bagolini striated glasses. We found 14 orthoptic pathological cases most of which could be revealed by an accurate cover test. The TNO stereotest and the Bagolini glasses have high predictive diagnostic values of negative and even of positive tests. If one of these tests is used as a supplement to the ophthalmological examination and anamnesis, we find it to be a valuable hint of orthoptic normality or abnormality. If an orthoptic pathology is suggested, this must be followed by a complete orthoptic examination concerning diagnosis and treatment.

The value of sight-screening, a comparative study of the Rodenstock sight-screener and a clinical eye examination of 109 adult employees in a hospital.

A rodenstock sight-screener has been used in examinations of more than 20 000 employees in trade and industry by an organisation sponsored by Danish optical companies. In a following report it was stated, that a great deal of these people, because of reduced sight-function, would benefit from further examination by optician or ophthalmologist with prescription of glasses. On that background we undertook an examination of 109 adults working in our hospital. First they were tested with the Rodenstock sight-screener and afterwards they underwent orthoptic and clinical ophthalmological examinations in a blind design. The sight-screening included examination of vision for near and far, stereopsis, phoria, dark adaption and visual fields. We found some difference between our results and those published earlier. Furthermore we found bad clinical accordance. In conclusion, many people will be worried by this kind of sight-screening procedure and only few will benefit from it.

Prostaglandin E2 level in tears during postoperative inflammation of the eye.

The concentration of prostaglandin E2 (PGE2) in tears was measured by a radio immuno assay technique in 25 persons before cataract extraction and during the post-operative period. In addition we studied the clinical signs of inflammation, the tear secretion rate, the content of polymorphonuclear leucocytes (PMNs) in tears, the bacteriology of the conjunctival sac and the central corneal thickness (CCT) during this period. We found pre-operative median level of PGE2 of 82 pg/ml and on the first post-operative day a significant rise in PGE2 concentration despite increased tearing. There was a tendency towards further gradual increase in PGE2 concentration until the 5th day and thereafter a decline to pre-operative level 12 days after surgery. The conjunctiva is proposed as a possible source of the PGE2 in tears. No significant correlation could be demonstrated between PGE2 levels and clinical signs of inflammation, CCT, PMN level, bacteriology or tear secretion rate.

Out-patient cataract surgery. First experiences from a large hospital department.

Out-patient cataract surgery was recently introduced as a routine procedure in our department. Background population, selection criteria and practical-administrative procedures for the first 33 patients are described. 19 females and 14 males, age range 45 to 90 years, were offered ambulatory surgery; 19 had extracapsular surgery and 27 intraocular lenses. Insignificant complications occurred in four cases, one of them related to the out-patient procedure. It definitely seems possible to offer this modality to selected patients without compromising medical ethics, and the results have been met with unanimous satisfaction by the patients.

Prostaglandin E2 in tears.

Prostaglandin E2 (PGE2) concentration in tears was measured before and following cataract extraction, during acute anterior uveitis and in patients with chronic conjunctival complaints. Following cataract extraction a significant increased PGE2 level in tears was observed, returning towards normal level within two weeks. In patients with acute anterior uveitis low levels of PGE2 was found in tears. Half of the patients with chronic conjunctival complaints exhibited high levels of PGE2. Conjunctiva is proposed as a likely source of PGE2 in tears.

Visual evoked response and alcohol intoxication.

Twelve eye healthy test persons were followed for 2 h after alcohol intake by frequent recordings of visually evoked pattern responses (VER's) and simultaneous determinations of the blood alcohol concentrations. VER's were recorded with a stimulus of 14' and 110' check size, and for both check sizes steady state (15 shifts per sec) and transient responses (2 shifts per sec) were analyzed. In the transient responses we found a general increase in latencies after alcohol intake, but only in one stimulus modality (2 shifts per sec, 14' check size) was the deviation statistically significant. The steady state responses showed a large pre-alcoholic variability both in amplitudes and in latencies. No change was discernible after alcohol intake. No correlation was found between the various levels of alcohol concentration and any of the VER parameters. The minor changes of the VER found in this study using a patterned stimulus are discussed and compared to the findings in earlier studies using a flash as well as a patterned stimulus.

Local application of indomethacin in healthy eyes and in post-operative cataract patients.

The effects of locally applied Indomethacin were studied. Ten healthy subjects received 1 drop of a 1% watery solution 4 times a day in their left eye during 2 weeks, the right eye serving as control. Neither changes in visual acuity and refraction nor in the appearance and function of the external and anterior segment of the eye were found during this period. Sixty-four patients underwent surgery for simple senile cataract. Half of the patients, by random selection, received Indomethacin solution locally during the first week. The remaining half received routine treatment with a compound preparation of antibiotics and corticosteroid from the third post-operative day. An inflammatory score was calculated and was found to be significantly reduced in the Indomethacin group during the first week.
