[Rational means of the determination of costs and etiological diagnosis of community-acquired pneumonia].

The cost and effectiveness of examinations (sputum staining and culturing, antitest determination for Influenza A and B, RSV, Adenovirus, Chlamydia psittaci and pneumoniae, Coxiella burnetii, Mycoplasma pneumoniae and Legionella pneumophila, and determination for Streptococcus pneumoniae antigen) performed to explore the aetiology of community-acquired pneumonia in the case of 258 hospitalised patients were analysed. The aetiology could be determined in 44.2% of the cases. On the basis of prevailing prices in 1986-88 one pneumonia case with determinable aetiology costs 8111 Forint. The authors have come to the conclusion that in the present epidemiological situation in this country it is not worthwhile to look for so-called non-bacterial microorganisms routinely, because of their rarely occurrence (16.7%) the cost per one positive finding is unrealistically high. Comparing the cost and the practical use the examinations applied the rational choice seems to be to culture the sputum with deep airway origin and to determine the Streptococcus pneumoniae antigen routinely. In the case of suspicion of non-bacterial origin to perform complement fixation test for Mycoplasma pneumoniae and in a severe clinical state to culture the blood is recommended.

[The potentialities and results of the etiologic diagnosis of acute pneumonias].

The paper deals with the results of the joint Soviet-Hungarian study for the determination of etiology of acute pneumonias. Study included 617 patients. The bacteriologic and serologic methods were used to reveal the causative agent of the disease. Prevalence of Streptococcus pneumoniae (33-49%) in the etiologic structure of acute pneumonias was established both in the USSR and Hungary. In many cases pneumonia is also caused by Haemophilus influenzae (21-22%). In 15-31% of the cases the causative agent of pneumonia is gram-negative microorganisms. A protracted course of acute pneumonia is due to the complex associations of microorganisms including S. pneumoniae. A conclusion has been drawn to the effect that the initial antibacterial therapy should be started with administration of antibiotics that act effectively on S. pneumoniae.

[Characteristics of nosocomial pneumonia].

284 patients with pneumonia treated in non-invasive chest wards were analysed in a prospective study. Out of 284 cases 26 proved to be nosocomial pneumonias. Older age, chronic non specific lung diseases, malignancies, peptic ulcus, chronic alcoholism as risk factors for nosocomial pneumonia should be taken into consideration. The pathogen agent was clearly identified in four cases, meanwhile the pathogenity of the agents was suspected at seven patients. By distribution of all pathogens the widely known bacteria are responsible for nosocomial pneumonia were less than one third of cases (in three cases Staphylococcus aureus, in one-one case E. coli and Enterobacter). The possible causes of their observation were analysed. Out of the 15 pathogens Gram-negative bacilli were found in six cases only. Authors suggest to use this observation at the rational choice of empiric treatment.

[Abscessing pneumonia].

Among the patient of a prospective study aimed to recognize the epidemiological and clinical characteristics of pneumonias, cavitating pneumonia occurred in 12.5%, typical lung abscess in 24 cases and necrotizing pneumonia in 11 cases. The illness was secondary in 4 cases. Predisposing factors were present in the majority of the patients. Assumable pathogen was detected with microbiological examinations in 24 cases. 20 patient recovered--19 by antibiotic therapy and one by surgery--were operated on the average in 7 weeks. Complication occurred in 15 patients 7 of them died. The prognosis of cavitating pneumonias is favourable, however the mortality is high in the secondary cases.
