[Optic disc drusen--case report].

A case of giant optic disc drusen is presented. Diagnostic difficulties and a role of fluorescein angiography, CT and static perimetry in diagnosing these alterations are described.

[Tuberous sclerosis--case report].

A case of 37-year-old man with tuberous sclerosis is presented. Classic triad (Vogt) which consists of mental handicap, epilepsy and adenoma sebaceum around a nose and cheeks was observed. Ophthalmoscopy showed bilateral retinal astrocytomas with mulberrylike appearance. During 6 years follow-up small progression of ocular changes (findings) was noticed. CT and NMR of the head, ultrasound of the eye fluorescein and ICG angiography are presented.

[Corneal topography before and after radial keratotomy].

PURPOSE: Evaluation of corneal topography of patients examined before and after radial keratotomy. MATERIAL AND METHODS: There were 23 patients (40 eyes) examined at our department. The corneal shape was evaluated by use of EYE-SYS company equipment. RESULTS: Postoperatively marked flattening of the central cornea was noticed with hypercorrection which decreased during postoperative period up to six months. Good postoperative results persisted after 18 months. Additional corneal incisions during RK decreased astigmatism effectively. CONCLUSION: Examination and corneal shape analysis before and after RK enabled an objective and quantitative evaluation of this procedure.

[The effect of corneal incision method on astigmatism after cataract extraction].

PURPOSE: To assess the effect of the type of corneal incision and its closure on astigmatism after cataract extraction. MATERIAL AND METHODS: Induced astigmatism was evaluated in 94 eyes after cataract extraction with IOL implantation 7 days, 1 month, 3 months, 6 months and 12 months after operation. 3 groups: were compared I--after extracapsular extraction with corneo-scleral incision (10 h-2 h) and continuous cross-like suture, II--after phacoemulsifications with scleral tunnel incision (3.3 mm) without suture. RESULTS: The highest induced astigmatism was observed 7 days after surgery in patients with the longest corneo-scleral incision with suture (group I). Induced astigmatism gradually decreased in time. In group after phacoemulsification especially with no suture low astigmatism with prompt stabilization was observed. CONCLUSIONS: The length and type of closure of the incision have an essential effect on induced astigmatism. Small incision, especially without suture induces minimal early postoperative astigmatism which remains stabile.

[Personal observations with the use of soft contact lenses after radial keratotomy].

PURPOSE: The evaluation of conjunctiva and cornea of patients wearing soft contact lenses after RK. MATERIAL AND METHODS: Between the years 1991-1995 RK was performed in 88 patients. Nine of them (10.2%) used soft contact lenses for correction of residual refractive error. All these people were carefully examined. RESULTS: The only complication was superficial neovascularisation concerning mainly peripheral parts of the corneal incisions. The long incisions reaching limbus and early application of contact lenses after RK promote peripheral neovascularisation of the cornea. CONCLUSIONS: Soft contact lenses should not be recommended after RK if corneal incisions reach the limbus. However, in other cases they can be used with careful ophthalmological supervision.

[Tissue plasminogen activator (t-PA) in aqueous humor of patients with cataract].

PURPOSE: The purpose of the examination was evaluation of tissue plasminogen activator (t-PA) concentration and plasminogen activator inhibitor (PAI) activity in aqueous humor of the eye and plasma of blood in patients operated on cataract. MATERIAL AND METHODS: In aqueous humor and citrate plasma of 24 patients undergoing cataract surgery the concentration of t-PA (Imulyse Biopool) and the activity of PAI-1 (Spectrolyse Biopool) were determined. RESULTS: In aqueous humor of 90% patients the concentration of t-PA-Ag in mean value 1.5 +/- 2.7 ng/ml was ca 20% of the level present in plasma. In aqueous humor of nearly all patients no PAI-1 activity could be detected. In a little group of persons because of small amounts of aqueous humor, other fibrinolytic parameters were also estimated and the presence of urokinase-type plasminogen activator (u-PA), alfa-2 antiplasmin (alfa-2AP) and plasmin-alfa-2 antiplasmin (PAP) complexes but no activity of protein C were observed. CONCLUSION: The presence of PAP complexes in aqueous humor indicates on intensive plasminogenesis in vivo. Our examinations confirm the role of fibrinolytic process in physiological and pathological processes occurring in the eye.
