Bilomas: a new approach to the diagnosis and treatment.

Biloma, a newly described entity, appears to have gained a heightened clinical interest recently. During the past 18 months we have diagnosed 9 abnormal biliary collections. We discuss here the role of sonography and radionuclide cholescintigraphy in the evaluation of bilomas. The diagnosis of all cases was confirmed by ultrasound-guided percutaneous needle aspiration. Subsequently 7 patients were treated with catheter drainage, obviating the need for any surgery.

Interventional ultrasound in detection and treatment of renal inflammatory disease.

Fifty-one patients with acute inflammatory renal disease were examined with ultrasound. The spectrum of findings seen in acute pyelonephritis is described, with emphasis on the evolution of the disease from acute focal inflammation to perinephric extension. There were 26 cases of acute bacterial nephritis, 15 cases of abscess formation, and 10 cases of pyohydronephrosis. Pathologic correlation was obtained. The impact of percutaneous needle puncture on the diagnosis and treatment of all stages of renal disease is discussed. Prompt diagnosis can be made, and specific antibiotic therapy can be instituted on the basis of information obtained by ultrasound examination and percutaneous aspiration. If necessary, treatment may be aided by ultrasonically guided placement of drainage catheters. Serial examination will show resolution of the disease, and it avoids exposure of the patient to ionizing radiation and intravenous contrast agents.

Liver abscess: sonography in diagnosis and treatment.

Over 24 months, 22 patients with intrahepatic abscesses were diagnosed by sonography. The sonographic appearance of the abscesses was nonspecific and varied from anechoic lesions to highly echogenic solid masses. Septations, fluid-fluid interfaces, and debris were also observed. In 19 of 22 patients, the diagnosis was confirmed by needle aspiration. Subsequently, 12 patients were treated with percutaneous catheter drainage; in 10 patients this therapy obviated the need for further surgery.

Alkaline ribonuclease from rye germ cytosol.

1. Alkaline ribonuclease (pH optimum 7.6) was isolated from rye (Secale cereale L) germ cytosol and partially purified; the preparation was devoid of other nucleolytic activities. 2. The enzyme is a typical endonuclease hydrolysing all phosphodiester bonds in RNA, yielding ultimately purine and pyrimidine nucleoside 2',3'-cyclic phosphates and the corresponding 3'-phosphates. Upon extensive digestion of synthetic polyribonucleotides, pyrimidine, but not purine, nucleoside 3'-phosphates are formed. The enzyme does not hydrolyse synthetic purine cyclic nucleotides. 3. The enzyme does not depolymerize double-stranded complexes of poly(A) and poly(U). 4. Susceptibility to photooxidation and inhibition by 2-hydroxy-5-nitrobenzyl bromide and N-bromosuccinimide implies the involvement of tryptophan residue in the active centre of the enzyme.

Endoscopic pulsed dye laser lithotripsy of gallbladder calculi in vivo.

We have evaluated the efficacy and safety of pulsed dye laser lithotripsy of gallbladder calculi using a percutaneous endoscopic technique in a porcine model. Fragmentation was readily achieved in vivo. Using a combination of laser lithotripsy and saline lavage, complete removal of all stone debris was feasible through a 24 F tract (N = 3). However, the degree of fragmentation required rendered removal through a smaller tract inefficient, a mean 53% of stone mass being retrievable through a 16 F tract (N = 11). Repeated laser activation at 1 mm from the gallbladder mucosa produced minimal injury, regardless of pulse energy. When the laser fiber was pressed against the mucosa, perforation of the gallbladder was possible at therapeutic pulse energy, but this did not lead to clinical sequelae. We conclude that the pulsed dye laser is a safe and effective means of fragmenting gallbladder calculi in vivo.

MRI isolation of infected renal cyst in autosomal dominant polycystic kidney disease.

The use of gadolinium-enhanced magnetic resonance (MR) to isolate an infected renal cyst in the setting of autosomal dominant polycystic kidney disease (ADPKD) has not been previously described. A case in which T1- and T2-weighted and gadolinium-enhanced MR images were used to identify a single purulent cyst in a patient with ADPKD is herein presented. We suggest that gadolinium-enhanced MR be considered useful in the evaluation of ADPKD patients with suspected infected cyst.
