[Total serum and non-conjugated estriol in the 3d trimester of pregnancy. Inter-individual, intra-individual variations during the day and from one day to another. Study of at-risk pregnancies].

It is possible to estimate the daily rate of oestriol production by the fetoplacental unit to monitor pregnancy by radio-immunoassay of the concentrations of unconjugated and total oestriol in the serum. The usefulness of these two parameters was compared by comparing variations from day to day in the same individuals and between them and other individuals, as well as over 24 hours. We think the best choice of test for biochemical monitoring of at-risk pregnancies is the estimation of unconjugated oestriol because there is a lower day to day variation in the same individual as well as throughout the 24 hours.

[Ectopic pregnancy: diagnostic reliability of a new hCG-test (Neo-Pregnosticon)].

We report about the diagnostic reliability of a new commercially available human chorionic gonadotropin (hCG) test, a simple hemagglutination test (discriminatory value: 75 U/I), which can be easily performed in any laboratory (Neo-Pregnosticon). Urine samples of 457 patients with unexplained lower abdominal pain and/or abnormal bleeding were analysed. The diagnostic sensitivity and specificity of the test gave values of 0.993 and 1.0, respectively. The predictive value for positive results was 1.0, and for negative results 0.997. By means of this test, we could determine hCG in urine of 40 out of 41 patients with ectopic pregnancy, in contrast to the currently used urine test (Pregnosticon, discriminatory value: 1000 U/I) which was positive in only about 50% of the cases. Due to its accuracy, this simple test will probably replace in the future the more laborious radioimmunological methods for determination of hCG.

Synthesis of immunogenic C-6 derivatives of 2-methoxyestrone and 2-methoxyestradiol-17 beta and characterization of the corresponding antisera.

The synthesis, purification and structural confirmation of the 6-(carboxymethoxyimino) derivatives of 2-methoxyestrone and 2-methoxyestradiol-17 beta are described. These derivatives were coupled to bovine serum albumin by the mixed anhydride method, and rabbits were immunized with the product. The resulting antisera showed high affinity and specificity for 2-methoxyestrone and 2-methoxyestradiol-17 beta, respectively, with low cross reactivities to structurally related estrogens.

Concentrations of 2-hydroxyoestrogens in human sera measured by a heterologous immunoassay with an 125I-labelled ligand.

A heterologous immunoassay for 2-hydroxyoestrogens has been established in which antibodies raised against 2-hydroxyoestradiol-17-succinyl-BSA serve as binding protein and 2-hydroxyoestrone-17-cmo-[125I]iodohistamine as radioligand. Lipophilic serum components competing for binding sites in this system were defined as total 2-hydroxyoestrogens'. The underlying assumption of specificity was supported by the pattern of cross-reactivity evaluated with structural related steroids and o-diphenols and by the fact, that an additional chromatography of the serum extracts preceding the competing reaction had little if any effect. Sensitivity: 2.8 +/- 1 pg/tube; accuracy: Y = 0.91x + 2.2; r = 0.989; precision: 5.8% intra-assay; 6.5% inter-assay. The following concentrations (+/- standard deviation) were found in the sera of healthy subjects. Young men: 29 +/- 5 pg/ml (n = 11); women follicular phase: 32 +/- 8 pg/ml (n = 25); luteal phase: 53 +/- 13 pg/ml (n = 23); postmenopausal women: 13 +/- 4 pg/ml (n = 10); pregnant women 11th--20th week: 70 +/- 16 mg/ml (n = 64); 36th--40th week: 240 +/- 23 pg/ml (n = 40); newborn cord blood: 604 +/- 43 pg/ml (n = 48).

[Critical comments on the calculation of receptor analyses (author's transl)].

The main literature concerning the processing of binding data is reviewed. Practical conclusions for the measurement of association constants and binding site concentrations are considered from the standpoint of basic binding theory in connection with basic information theory. The theoretical minimum errors in the association constant and the binding site concentration are discussed as a function of the saturation fraction of the most dilute component. Various plotting forms and the error arising from various rearrangements of the basic binding equation are discussed. For fitting procedure several heuristic models are mentioned and it is recommended to start from the most general Adair function. The necessity of well defined criteria for quality of fit in computer programming for iterative curve fitting procedures is emphasized. The most frequent, however avoidable, errors are mentioned.

[The value of alpha-1-fetoprotein determinations in the amniotic-fluid of patients with hydramnios in the diagnosis of fetal anomalies (author's transl)].

Little is known about the influence of hydramnios on the alphafetoprotein (AFP)-concentration in the amniotic fluid. We therefore investigated in 19 patients with hydramnios the diagnostic value of the sonographic evidence and the AFP-concentration in the amniotic fluid in relation to prenatal diagnosis of fetal malformations in two independent series. An anlysis of the AFP-concentration in the amniotic fluid can corroborate the diagnosis of a malformation made by sonography and influence decisions concerning the management of pregnancy and delivery. If ultra-sonic tests show no anomalies, significantly increased AFP-concentrations in the amniotic fluid of patients with hydramnions can merely confirm a suspicious of an existing malformation and influence decisions on further diagnostic steps.

[Biochemistry and prepared diagnosis of lung maturation (author's transl)].

The composition and structure of acellular alveolar material are described. Evidences as summarized that 1,2-dipalmityl-sn-glycero-phosphoryl-choline, depending of the presence of other components of the hypophasis, determines the surface tension of the air-alveolus interface. The foetal maturation of the lung is defined according to the Avery-Clements-Gluck theory, as a stabilisation of the alveolar lecthin production; and the dependence of the relevant enzyme activities on the stage of the pregnancy is discussed. Attention is drawn to the possibility of enzyme induction by medication; the biochemical and clinical aspects of the acceleration of lung maturation following glucocorticoid therapy are treated in detail. Finally, methods for the prepartal and postpartal characterisation of lung maturation, and especially the biochemical methods for the determination of lecithin in amniotic fluid, tracheal, and gastric secretion are discussed.
