Surgical and prosthetic considerations in the management of orbital tumors.

Since tumors of the globe and ocular adnexa, other than skin cancers of the eyelid, are uncommon, most oncologic specialists have little opportunity to become experienced in the management of eye tumors. In general, a malignancy that does not penetrate a bony orbital wall can be resected with an adequate margin by inclusion of the adjacent wall and periorbita. The decision to save or sacrifice a globe when a malignancy penetrates the bone but not the periorbita should be based on such factors as the tumor histology, contralateral vision, the probability of permanent diplopia, and the need for irradiation in doses that would destroy vision. Because of the complications involved with irradiation of orbital tumors, surgical resection has been the treatment of choice. In addition, orbital exenteration presents a unique challenge to the maxillofacial prosthodontist. Prosthetic restoration of the orbit is often complicated by the extent of resection, tissue response, and method of retention.

A new method of measuring dimensional change.

A new method of measuring linear dimensional change in denture base resins is presented and its precision in determining distances between two points was investigated. Chrome steel ball bearings 1.57 mm in diameter were invested in a stone mold and were used to record measurements by a reflection from the surface of the ball bearings. The accuracy of this method was determined by performing repeated measurements between two points in a stone mold. Random measurements were made to an accuracy of 0.002 mm. The dimensional change of light-cured, heat-cured, and self-cured denture base resins was also determined with this method, followed by an analysis of variance of the data. The findings support the accuracy and reproducibility of this new method in recording linear measurements. The dimensional changes of all three resins were different from each other after the specimens were processed and stored in distilled water for 7 days at 37 degrees C.

Dimensional change in maxillary prosthetic obturators.

Dimensional stability of obturator bases and changes in occlusal relationships are areas of clinical importance in the oral rehabilitation of maxillectomy patients. The dimensional change in maxillary prosthetic obturators constructed with three different denture base resins was investigated. The results indicate that a light-polymerized material had the least percent of change between base and teeth positions.

Provisional restoration for a patient with cleft lip and palate: a clinical report.

A method for designing and fitting a provisional restoration for a patient with a cleft lip and palate was described. The provisional prosthesis provides an alternative treatment option that allows the dentist to plan the definitive restoration while providing the patient with a temporary but esthetic and functional restoration.

Prosthetic and surgical management of osteogenic sarcoma of the maxilla.

Maxillary surgical defects resulting from resection of oral neoplasms vary in size from small perforations of the hard and soft palate to complete removal of these structures. Osteogenic sarcoma is of unknown etiology and is a rapidly growing tumor that may produce pain, paresthesia, and anesthesia. The recommended treatment of radical resection often results in defects that produce significant orofacial disfigurement. These defects, regardless of size, present significant functional disability, with compromised esthetics, mastication, and deglutition. Restoration with a maxillary obturator prosthesis can reestablish the physical separation between oral and nasal cavities and, in soft palate defects, enable normal palatopharyngeal function. Most patients can be rehabilitated successfully with restoration of speech and swallowing to normal levels and significant improvement in appearance.
